Book excerpt: Dietary Risk Assessment in the WIC Program reviews methods used to determine dietary risk based on failure to meet Dietary Guidelines for applicants to the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC). Applicants to the WIC program must be at nutritional risk to be eligible for program benefits. Although "dietary risk" is only one of five nutrition risk categories, it is the category most commonly reported among WIC applicants. This book documents that nearly all low-income women in the childbearing years and children 2 years and over are at risk because their diets fail to meet the recommended numbers of servings of the food guide pyramid. The committee recommends that all women and children (ages 2-4 years) who meet the eligibility requirements based on income, categorical and residency status also be presumed to meet the requirement of nutrition risk. By presuming that all who meet the categorical and income eligibility requirements are at dietary risk, WIC retains its potential for preventing and correcting nutrition-related problems while avoiding serious misclassification errors that could lead to denial of services for eligible individuals.

The context is a global one; as the developed world faces a problem with overconsumption and chronic diseases, the developing world is addressing the double burden of hunger and over consumption. Even in the developed world, nation states are facing the rise of modern malnutrition which is over consumption, but also the re-emergence of hunger as there are growing levels of poverty and inequality due to the financial crises. Food insecurity is in many people’s minds associated with hunger, and while this is true the modern food system has introduced new complexities to food insecurity with the growth of micro-nutrient inequalities. Hunger and obesity are not being faced by two different groups but often the same group or cohort. These are features of modern malnutrition that are often not recognized. A critical examination of food poverty and food security is undertaken, with a view to clarifying taken-for-granted assumptions in present discourses. The book addresses food charity and the rise of solutions such as foodbanks as appropriate social responses. The final chapters explore the solutions from real life situations. The concluding chapter from the editors draws together the issues and locates solutions within a food policy framework of the total food system. The various definitions of food insecurity will are examined. Hunger and its modern manifestations (hunger and obesity) is another focus, with particular explorations of developed and developing countries experiences. Some of the chapters cover how food poverty/insecurity is being addressed and provide examples of work in progress.

Low income is statistically linked with a variety of poor outcomes for children, from low birth weight and poor nutrition in infancy to increased chances of academic failure, emotional distress, and unwed childbirth in adolescence. To address these problems it is not enough to know that money makes a difference; we need to understand how. Consequences of Growing Up Poor is an extensive and illuminating examination of the paths through which economic deprivation damages children at all stages of their development. In Consequences of Growing Up Poor, developmental psychologists, economists, and sociologists revisit a large body of studies to answer specific questions about how low income puts children at risk intellectually, emotionally, and physically. Many of their investigations demonstrate that although income clearly creates disadvantages, it does so selectively and in a wide variety of ways. Low-income preschoolers exhibit poorer cognitive and verbal skills because they are generally exposed to fewer toys, books, and other stimulating experiences in the home. Poor parents also tend to rely on home-based child care, where the quality and amount of attention children receive is inferior to that of professional facilities. In later years, conflict between economically stressed parents increases anxiety and weakens self-esteem in their teenaged children. Although they share economic hardships, the home lives of poor children are not homogenous. Consequences of Growing Up Poor investigates whether such family conditions as the marital status, education, and involvement of parents mitigate the ill effects of poverty. Consequences of Growing Up Poor also looks at the importance of timing: Does being poor have a different impact on preschoolers, children, and adolescents? When are children most vulnerable to poverty? Some contributors find that poverty in the prenatal or early childhood years appears to be particularly detrimental to cognitive development and physical health. Others offer evidence that lower income has a stronger negative effect during adolescence than in childhood or adulthood. Based on their findings, the editors and contributors to Consequences of Growing Up Poor recommend more sharply focused child welfare policies targeted to specific eras and conditions of poor children's lives. They also weigh the relative need for income supplements, child care subsidies, and home interventions. Consequences of Growing Up Poor describes the extent and causes of hardships for poor children, defines the interaction between income and family, and offers solutions to improve young lives. Book excerpt: This year’s report presents evidence that the absolute number of people who suffer from hunger continues to slowly increase. The report also highlights that food insecurity is more than just hunger. For the first time, the report provides evidence that many people in the world, even if not hungry, experience moderate food insecurity as they face uncertainties about their ability to obtain food and are forced to compromise on the quality and/or quantity of the food they consume. This phenomenon is observed globally, not only in low- and middle-income countries but also in high income countries. The report also shows that the world is not on track to meet global nutrition targets, including those on low birthweight and on reducing stunting among children under five years. Moreover, overweight and obesity continue to increase in all regions, particularly among school-age children and adults. The report stresses that no region is exempt from the epidemic of overweight and obesity, underscoring the necessity of multifaceted, multisectoral approaches to halt and reverse these worrying trends. In light of the fragile state of the world economy, the report presents new evidence confirming that hunger has been on the rise for many countries where the economy has slowed down or contracted. Unpacking the links between economic slowdowns and downturns and food insecurity and malnutrition, the report contends that the effects of the former on the latter can only be offset by addressing the root causes of hunger and malnutrition: poverty, inequality and marginalization.

The global financial crisis that began in 2008 compounded the burden of high food prices, exacerbating the problems of hunger and malnutrition in developing countries. The tandem food price and economic crises struck amidst the massive, chronic problem of hunger and undernutrition in developing countries. National governments and international actors have taken a variety of steps to mitigate the negative effects of increased food prices on particular groups. The recent abrupt increase in food prices, in tandem with the current global economic crisis, threatens progress already made in these areas, and could inhibit future efforts. The Institute of Medicine held a workshop, summarized in this volume, to describe the dynamic technological, agricultural, and economic issues contributing to the food price increases of 2007 and 2008 and their impacts on health and nutrition in resource-poor regions. The compounding effects of the current global economic downturn on nutrition motivated additional discussions on these dual crises, their impacts on the nutritional status of vulnerable populations, and opportunities to mitigate their negative nutritional effects.

Despite this progress and achievement, a number of regressive aspects also continue to plague many parts of the world and blight the lives of men and women in various ways. The manifestation and intensity of these aspects varies between regions and between countries within regions. Poverty, hunger, malnutrition and mortality are so inexorably intertwined that they reinforce each other and hinder human development and economic growth. Understanding the relationship between poverty, nutrition and mortality is a complex one and this relationship vary according to the social, economic and cultural environment. In order to further our knowledge on these important issues, a seminar titled “The Impact of Mortality as Both a Determinant and a Consequence of Poverty and Hunger: A Contribution to Achieving the First Millennium Development Goal (Eradicate Extreme Poverty and Hunger)” was held between February 23 and 25, 2005 in Trivandrum, India. The seminar was organised jointly by CICRED and the Centre for Development Studies (CDS) with financial support from UNFPA This book presents some of the papers offered at that conference with additional papers 'invited' in order to cover other regions and to fill the gap in certain issues. This volume covers Asia, the Pacific and Africa where nearly 90 percent of the world's hungry people live. About 2 out of 5 under-five deaths occur in Sub-Saharan Africa and almost half of the world's undernourished children live in South Asia.

A combination of income growth and nutrition interventions are therefore suggested to adequately tackle this issue, yet evidence to support this claim is often not available, especially for African settings. The authors evaluate the joint contribution of income growth and nutrition interventions toward the reduction of malnutrition. Using a four-round panel data set from northwestern Tanzania they estimate the determinants of a child's nutritional status, including household income and the presence of nutrition interventions in the community. The results show that better nutrition is associated with higher income, and that nutrition interventions have a substantial beneficial effect. Policy simulations make clear that if one intends to halve malnutrition rates by 2015 (the Millennium Development Goals objective), income growth will have to be complemented by large-scale program interventions.

Poor-quality diets have become a major driver for overweight and obesity and associated non-communicable diseases such as diabetes, heart diseases, and some types of cancers. Conflict and climate vulnerability have been identified as major obstacles to reaching Sustainable Development Goal targets related to malnutrition by 2030. In 2019, economic downturns and slowdowns hindered efforts even further. More recently, the COVID-19 crisis has imposed even harsher conditions to countries.Poverty and malnutrition are inevitably linked, and therefore addressing one can help address the other. Given that most of the world’s extremely poor people and stunted children live primarily in rural areas and rely mostly on agriculture, the agriculture and food systems approach can offer an opportunity to reduce both poverty and malnutrition. The food systems approach places equal emphasis on both the supply and demand dimensions that are critical for ensuring healthier diets and better nutrition for poor and vulnerable groups.This special issue of Policy in Focus is dedicated to answering a crucial question: How can a food systems approach be used to design and implement policies and investments that reach those most vulnerable to poverty, hunger, malnutrition, and suboptimal diets? We hope that the contributions contained in this volume, by leading academics and development practitioners, exploring the linkages between nutrition, food systems, and poverty, can help stakeholders and policymakers make inroads towards the promotion of food and nutrition security and the reduction of rural poverty.
