Book excerpt: Our knowledge of postoperative thromboembolic complications has increased enormously over the past 2 decades, particularly where diag nosis and prophylaxis are concerned. The 125 I-fibrinogen method of diagnosing thrombosis has completely changed our concept of the frequency, occurrence, and natural course of thrombosis, and it has formed the basis of most thromboprophylactic studies. Concurrently with the development of this diagnostic method, two methods for the prophylaxis of thrombosis have come into vo gue, namely low-dose heparin and dextran. Both these methods were tested in very extensive studies during the seventies, and their value has been unequivocally proved, for reducing both the frequency of thrombosis with and without symptoms, and the frequency of fatal pulmonary embolism. Thromboprophylaxis is not particularly common in surgery; how ever, and its general use is far from uncontested. It has been argued that not only does it complicate surgical activities and make them mo re expensive, but it also involves an unacceptable number of other complications.

It has been correctly called by S. Sherry a major breakthrough in medicine. Although thrombo embolism was first recognized by Laennec in 1819 and defined by Virchow in 1846, its development was not well understood and its prevention escaped our best efforts until now. This all-too-com mon, sudden, unexpected and unwanted form of morbidity and mortality, always referred to in the surgical patient as postoper ative pulmonary thromboembolism, has now become the major complication of all surgery. However, it occurs with equal fre quency in hospitalized non-operative patients as well, so the latter are also in need of this prophylaxis if this calamity is to be avoided. The mass ofliterature generated in the past few years has produced some confusion as to which of a number of methods of heparin prophylaxis gives the best results. It is the intention of this book to help clarify the situation and thereby resolve the problem by offering a procedure of subcutaneous heparin prophylaxis which is acceptable, safe and simple to administer. The first ESWL treatment in the United States with the Dornier H~ device occurred at the Methodist Hospital of Indiana, and by the end of 1984, In spite of the rapidly the United StatesESWL study group had accrued over2,5()() ESWL treatments. accumulated experience at the six institutions involved in the FDA trial of the Dornier HM] device, other urologists in this country and around the world had little opportunity to gain knowledge about the utilization of this revolutionary technique. For this reason, the Methodist Hospital of Indiana organized the first symposium on shock wave lithotripsy in February of1985. Interest in this meeting was intense, as approval of the Dornier device had occurred only a few weeks earlier in December of 1984. Because of the success of this initial meeting, subsequent meetings have been held annually in Indianapolis. Following the third annual symposium on extracorporeal shock wave lithotripsy in March of 1987, a number of participants and attendees requested that the information presented at the meeting be made available. Therefore, plans were made to publish the proceedings of the next meeting which occurred March 5 and 6, 1988. The Methodist Hospi tal ofIndiana' s 4th Symposium on Shock Wave Lithotripsy: State of the Art was the best attended meeting to date with over 650 registrants from 36 states and 24 countries.
