Book excerpt: This paper examines current trends in Positron Emission Mammography (PEM) instrumentation and the performance tradeoffs inherent in them. The most common geometry is a pair of parallel planes of detector modules. They subtend a larger solid angle around the breast than conventional PET cameras, and so have both higher efficiency and lower cost. Extensions to this geometry include encircling the breast, measuring the depth of interaction (DOI), and dual-modality imaging (PEM and x-ray mammography, as well as PEM and x-ray guided biopsy). The ultimate utility of PEM may not be decided by instrument performance, but by biological and medical factors, such as the patient to patient variation in radiotracer uptake or the as yet undetermined role of PEM in breast cancer diagnosis and treatment.

Book excerpt: Abstract: The Positron Emission Mammography imaging system (PEMi) provides a novel nuclear diagnosis method dedicated for breast imaging. With a better resolution than whole body PET, PEMi can detect millimeter-sized breast tumors. To address the requirement of semi-quantitative analysis with a radiotracer concentration map of the breast, a new attenuation correction method based on a three-dimensional seeded region growing image segmentation (3DSRG-AC) method has been developed. The method gives a 3D connected region as the segmentation result instead of image slices. The continuity property of the segmentation result makes this new method free of activity variation of breast tissues. The threshold value chosen is the key process for the segmentation method. The first valley in the grey level histogram of the reconstruction image is set as the lower threshold, which works well in clinical application. Results show that attenuation correction for PEMi improves the image quality and the quantitative accuracy of radioactivity distribution determination. Attenuation correction also improves the probability of detecting small and early breast tumors.

Book excerpt: Image reconstruction for positron emission mammography (PEM) with the breast positioned between two parallel, planar detectors is usually performed by backprojection to image planes. Three important factors affecting PEM image reconstruction by backprojection are investigated: (1) image uniformity (flood) corrections, (2) image sampling (pixel size) and (3) count allocation methods. An analytic expression for uniformity correction is developed that incorporates factors for spatial-dependent detector sensitivity and geometric effects from acceptance angle limits on coincidence events. There is good agreement between experimental floods from a PEM system with a pixellated detector and numerical simulations. The analytic uniformity corrections are successfully applied to image reconstruction of compressed breast phantoms and reduce the necessity for flood scans at different image planes. Experimental and simulated compressed breast phantom studies show that lesion contrast is improved when the image pixel size is half of, rather than equal to, the detector pixel size, though this occurs at the expense of some additional image noise. In PEM reconstruction counts usually are allocated to the pixel in the image plane intersected by the line of response (LOR) between the centers of the detection pixels. An alternate count allocation method is investigated that distributes counts to image pixels in proportion to the area of the tube of response (TOR) connecting the detection pixels that they overlay in the image plane. This TOR method eliminates some image artifacts that occur with the LOR method and increases tumor signal-to-noise ratios at the expense of a slight decrease in tumor contrast. Analysis of image uniformity, image sampling and count allocation methods in PEM image reconstruction points to ways of improving image formation. Further work is required to optimize image reconstruction parameters for particular detection or quantitation tasks.

Book excerpt: Positron emission mammography (PEM) of F-18 fluorodeoxyglucose (FbG) uptake in breast tumors with dedicated detectors typically has been accomplished with two planar detectors in a fixed position with the breast under compression. The potential use of PEM imaging at two detector positions to guide stereotactic breast biopsy has motivated us to use PEM coincidence data acquired at two or more detector positions together in a single image reconstruction. Multiple angle PEM acquisition and iterative image reconstruction were investigated using point source and compressed breast phantom acquisitions with 5, 9, 12 and 15 mm diameter spheres and a simulated tumor:background activity concentration ratio of 6:1. Image reconstruction was performed with an iterative MLEM algorithm that used coincidence events between any two detector pixels on opposed detector heads at each detector position. This present study compared two acquisition protocols: 2 angle acquisition with detector angular positions of -15 and +15 degrees and 11 angle acquisition with detector positions spaced at 3 degree increments over the range -15 to +15 degrees. Three-dimensional image resolution was assessed for the point source acquisitions, and contrast and signal-to-noise metrics were evaluated for the compressed breast phantom with different simulated tumor sizes. Radial and tangential resolutions were similar for the two protocols, while normal resolution was better for the 2 angle acquisition. Analysis is complicated by the asymmetric point spread functions. Signal- to-noise vs. contrast tradeoffs were better for 11 angle acquisition for the smallest visible 9 mm sphere, while tradeoff results were mixed for the larger and more easily visible 12 mm and 15 mm diameter spheres. Additional study is needed to better understand the performance of limited angle tomography for PEM. PEM tomography experiments with complete angular sampling are planned.

We have developed a list mode likelihood reconstruction algorithm for this scanner. Here we theoretically study the lesion detection and quantitation. The lesion detectability is studied theoretically using computer observers. We found that for the zero-order quadratic prior, the region of interest observer can achieve the performance of the prewhitening observer with a properly selected smoothing parameter. We also study the lesion quantitation using the test statistic of the region of interest observer. The theoretical expressions for the bias, variance, and ensemble mean squared error of the quantitation are derived. Computer simulations show that the theoretical predictions are in good agreement with the Monte Carlo results for both lesion detection and quantitation.

Book excerpt: The PET/X breast imaging system is designed to be a compact positron emission tomography (PET) scanner with a rectangular field of view (FOV) that can be mounted directly over the conventional mammography scanner. This thesis studies the prototyping of this dedicated breast cancer scanner that combines PET and mammography, using mostly computer modeling methods, while the hardware of data acquisition system is being concurrently developed. Apart from processing scripts and functions, two major simulation software packages are used: SimSET and CatSim. The former models PET emission process and the latter models mammography projection. This thesis delves into the details of using these packages and presents results of applying them to the PET/X system. By studying reconstructed images using simulated PET data, this thesis characterizes the proposed PET/X scanner and estimates its performance upper bound. Under several idealized detector conditions, the PET/X scanner exceeds the design goal of detecting a 20% change of radiotracer concentration with high sensitivity and specificity. This thesis also explores the use of X-ray (mammography) images for necessary attenuation correction of the PET data. Preliminary result shows that the X-ray-based attenuation correction (XAC) method has potential to be a surrogate for ground truth attenuation correction for the PET/X system.
