This book is designed to inform this discussion by providing an analytical basis for the discussion of options, a series of policy options which could be considered in moving forward, and the economic and financial implications of these options. In doing so, it is hoped that this book will help stimulate the discussion of options and help Tanzania develop a health financing strategy which meets its long-term needs. Health financing in Tanzania is currently highly fragmented, with many different sources of funds and programs directed at specific population groups. Despite this, many people still do not access health services because of financial barriers, and this burden falls disproportionately on the poor. This book looks at the current situation with respect to health financing as well as the experience in other countries to address health financing for the poor and the population generally, using a common analytical framework. The book then explores a number of options in the areas of revenue generation, pooling of funds, purchasing and service provision, and also looks at the regulatory and political environment, making specific recommendations which can be considered in each of these areas. The focus of these recommendations is particularly on improving financial health protection for the poor. The economic, financial and service delivery implications are then examined, using several different scenarios for extending pre-paid health insurance coverage to the population. Making Health Financing Work for the Poor in Tanzania will be of interest to readers working in the areas of health care and public health, social protection, and social analysis and policy, in Tanzania and in other countries aiming for improvements in their health financing systems.

In 2009, the number of most vulnerable children was estimated at two million. The rights of these children to social services including health which is under threat in light of increasing poverty, over-stretched extended families and insufficient mechanisms to ensure policy implementation. This is a qualitative study conducted in two wards of Morogoro Municipal that included 35 participants who were interviewed to obtain the information from them to build on the arguments and conclusions made in the literature. The study reveals that, policies that protect the poor are in place, yet the challenge remains at the implementation level. The socio-economic status of most households affects their accessibility to health services, implementation of user fees systems have remain to affect the poor. The challenges around the quality of services have been revealed in terms of inadequacy of drugs, human resources for health.

Despite this positive trend in the economy, poverty rates have not decreased accordingly; more than one-fourth of Tanzania's 53 million inhabitants live below the poverty line and almost 10 percent live in extreme poverty. The health sector has been identified as a policy priority area in Tanzania. The main purpose of this case study is to describe Tanzania's efforts to promote universal health coverage (UHC) inclusive of the poor, and to identify challenges and opportunities for the health system to advance on this path in a coherent and integrated fashion. Given the large number of interventions implemented in the health sector, efforts were selected based on their potential to address the challenges to the equitable access to health services, namely the poor quality of health services and the limited financial protection. The paper is organized as follows: section one gives introduction. Section two describes supply-side efforts and features the devolution of health services, the primary health care (PHC) strengthening program, and results-based financing. Section three analyzes Tanzania's social protection program and synthesizes the country's experience with user fees, as well as their impact on financial protection. Section four features the community health funds, Tanzania's most explicit effort to increase financial protection in health. Section five assesses the implementation of these initiatives. Section six highlights some opportunities to include the poor and address the challenges to pro-poor UHC in Tanzania.

Book excerpt: Universal health coverage (UHC) has been recognized by the World Health Organization as a key element in reducing social inequality and a critical component of sustainable development and poverty reduction. In most of the world UHC is sought through a combination of public and private-sector health care systems. In most low- and middle-income countries health systems are evolving to increasingly rely on the private sector because the public sector lacks the infrastructure and staff to meet all health care needs. With growing individual assets available for private-sector expenditure, patients often seek better access to technology, staff, and medicines. However, in low-income countries nearly 50 percent of health care financing is out-of-pocket. With the expected increase in the overall fraction of care provided through the private sector, these expenditures can be financially catastrophic for individuals in the informal workforce. In the global workforce of approximately 3 billion people, only 10 to 15 percent are estimated to have some type of access to occupational health services. The informal workforce is growing worldwide, and the degree to which its occupational health needs are satisfied depends on the capabilities of the general health care system. In July 2014, the Institute of Medicine held a workshop on approaches to universal health coverage and occupational health and safety for informal sector workers in developing countries. This report summarizes the presentations and discussions from this workshop. Approaches to Universal Health Coverage and Occupational Health and Safety for the Informal Workforce in Developing Countries identifies best practices and lessons learned for the informal workforce in developing countries in the financing of health care with respect to health care delivery models that are especially suitable to meeting a population's needs for a variety of occupational health issues, including the prevention of or mitigation of hazardous risks and the costs of providing medical and rehabilitation services and other benefits to various types of workers within this population. These experiences and lessons learned may be useful for stakeholders in moving the discussions, policies, and mechanisms forward to increase equitable access to quality health services without financial hardship for the informal workforce.
