It is based on the research carried out under the umbrella of the EU-funded project “Mobility of Health Professionals (MoHProf). The partners involved in the MoHProf project gathered evidence from 25 countries around basic questions and knowledge gaps relating to the international migration of health professionals, which involved an analysis of migration flows and evaluation of policies addressing migration. This book provides a comprehensive description and analysis of the mobility streams, the motives and driving forces behind them and the impact on and challenges for health systems and draws conclusions and provides recommendations for future strategic planning, monitoring and the management of mobility of health professionals as well as further research and policy development needs.

It provides studies on a variety of social policy fields in which different, but also the same, IOs operate. The countries' response has been a momentous achievement. Now, a third big step is being taken with the publication of this book. It underlines WHO's unwavering commitment to supporting the implementation of the Code and provides a wide range of detailed examples from the countries themselves of how they are tackling the many complex issues involved. It provides not just numerous insights into progress but also gives other countries valuable guidance and recommendations on how they, too, can implement the Code. Book excerpt: Since 2004, the U.S. government has supported the global response to HIV/AIDS through the President's Emergency Plan for AIDS Relief (PEPFAR). The Republic of Rwanda, a PEPFAR partner country since the initiative began, has made gains in its HIV response, including increased access to and coverage of antiretroviral therapy and decreased HIV prevalence. However, a persistent shortage in human resources for health (HRH) affects the health of people living with HIV and the entire Rwandan population. Recognizing HRH capabilities as a foundational challenge for the health system and the response to HIV, the Government of Rwanda worked with PEPFAR and other partners to develop a program to strengthen institutional capacity in health professional education and thereby increase the production of high-quality health workers. The Program was fully managed by the Government of Rwanda and was designed to run from 2011 through 2019. PEPFAR initiated funding in 2012. In 2015, PEPFAR adopted a new strategy focused on high-burden geographic areas and key populations, resulting in a reconfiguration of its HIV portfolio in Rwanda and a decision to cease funding the Program, which was determined no longer core to its programming strategy. The last disbursement for the Program from PEPFAR was in 2017. Evaluation of PEPFAR's Contribution (2012-2017) to Rwanda's Human Resources for Health Program describes PEPFAR-supported HRH activities in Rwanda in relation to programmatic priorities, outputs, and outcomes and examines, to the extent feasible, the impact on HRH and HIV-related outcomes. The HRH Program more than tripled the country's physician specialist workforce and produced major increases in the numbers and qualifications of nurses and midwives. Partnerships between U.S. institutions and the University of Rwanda introduced new programs, upgraded curricula, and improved the quality of teaching and training for health professionals. Growing the number, skills, and competencies of health workers contributed to direct and indirect improvements in the quality of HIV care. Based on the successes and challenges of the HRH program, the report recommends that future investments in health professional education be designed within a more comprehensive approach to human resources for health and institutional capacity building, which would strengthen the health system to meet both HIV-specific and more general health needs. The recommendations offer an aspirational framework to reimagine how partnerships are formed, how investments are made, and how the effects of those investments are documented.

Worker shortage, skill mix imbalance, maldistribution, negative work environments, and weak knowledge bases challenge nearly all countries. Especially in the poorest countries, the workforce is under assault by a triple threat of HIV/AIDS, out-migration, and inadequate investment. Effective country strategies should be launched and backed by international reinforcement. These include urgently mobilizing one million more health workers for Africa, and focusing efforts on the unremunerated community-level health workers, the majority of whom are women. Ultimately, the crisis in human resources is a shared problem requiring shared responsibility for cooperative action. Alliances for action are recommended to strengthen the performance of all actors while expanding space and energy for new ones.

At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: This book looks at the migration and work experiences of six women who have migrated to Australia from China; Zimbabwe; South Korea; the United Kingdom; India and the Philippines. It sets their journeys out into three distinct periods of migration, including the first period of their lives when they reflect on their experiences growing up with their immediate families and the factors that encouraged them to gravitate towards a nursing career. The second period covers time when each of these women begin to think about where their career in nursing might taken them. During this phase, these women take their first steps to leave their home country and migrate to Australia, often after several countries in between. The final section allows the reader to understand how these women initially experienced Australia when they first arrived and how they faced challenges both personally and professionally after arrival in their new place to call home. The discussions within these three sections cover both professional and personal/familial reflections, where differences in nursing identity between sending and destination country is discussed alongside the adjustments that the women needed to make to overcome loneliness and to successfully integrate into new organizational environments. Each chapter analyses migration as a life course, which considers why nurses leave their home country and find a new place to call home. Furthermore, if they find themselves thinking about returning to their country of birth; how or if they maintain transnational links, and how identity and ethnicity shape these responses. These life trajectories are underscored by an historical context setting of nursing migration to Australia in the opening chapter offering unique insights into the changing process of migration, accreditation, registration and settlement of nurses in Australia. Focusing on agribusiness and rural development, as well as the othering of international migrants and the shifting boundaries of belonging in rural spaces, the chapters in this book examine how globalisation, with migration being a constitutive feature, influences different rural contexts in the ‘Global North’ and the impact this has on migrant populations. Chapters demonstrate the harsh lived experiences/realities characterised by mental health issues and emotional labour for migrants, occupational health and safety issues in the workplace and experiences of exclusion and racism from ‘host’ communities. These chapters taken together identify a rural-migration nexus where the relationship between international migration and localised rural spaces are mutually constitutive.
