Along with the challenges faced by all medicinal products there is an added challenge due to the lack of raw material and the social problems with regards to their acquisition. Obtaining them depends on blood and plasma donations, which are insufficient. Europeans as a whole fail to even cover 40 % of their needs in plasma for fractionation. The aspiration that all donations shall be voluntary, free and unpaid, is an altruistic objective, which reality, unfortunately, contradicts every day. Governments and all social Agencies should promote to the maximum its actions in order to achieve the greatest number of donation each day. Even from a humanitarian or ethical viewpoint nobody can deny a possible donor accepting incentives. This incentive discussion, plus the concept of self-sufficiency of countries and more are discussed in this volume.

At the same time competitiveness and innovation must be stimulated. Without innovation in pharmaceuticals, the incurable diseases of today will remain incurable. Competitiveness drives innovation and innovation saves lives. Increased competitiveness of the pharmaceutical sector will not only better protect public health, but will also create high quality jobs and create growth. In this context the implementation of the G10 recommendations, particularly regarding the pricing and reimbursement of medicines by Member States, remains a considerable challenge.

Book excerpt: How best to manage risk involving multi-valued human biological materials is the overarching theme of this book, which draws on the sourcing and supply of blood as a case study. Blood has ethical, social, scientific and commercial value. This multi-valuing process presents challenges in terms of managing risk, therefore making it ultimately a matter for political responsibility. This is highlighted through an examination of the circumstances that led to HIV blood contamination episodes in the US, England and France, as well as their consequences. The roles of scientific expertise and innovation in managing risks to the blood system are also analysed, as is the increased use of precautionary and legal strategies in the post-HIV blood contamination era. Finally, consideration is given to a range of policy and legal strategies that should underpin effective risk governance involving multi-valued human biological materials.

Bremer Preface The industrial world today is divided between two camps: a culture based on the principles of Total Quality Management (TQM), developed in the Far East, and one still strongly influenced by the origins of "Scientific Management", intro duced in the West by F.W. Taylor and others at the turn of the century. This divergence will be shown to have arisen in the last forty years, long enough for a new generation of managers and corresponding culture to emerge. The two cul tures are so deeply entrenched that it is difficult for one to change to the other. However, there is strong evidence to support the contention that people-oriented TQM is superior, and those companies clinging to Taylor models now face diffi cult decisions. Actions by Taylor-companies to move to TQM rnight weH be hindered rather than helped by applying present Quality Assurance Standards, developed by Taylor-oriented national and international Standards Institutions.

This symposium is co-sponsored by the World Health Organization and is being held under the auspices of the ISBT and the Secretary General of the Council of Europe, Mr Walter Schwimmer. The patronage was granted with great pleasure for several reasons. First of all, Dutch experts are very active in our Committees and have largely contributed in developing the Council of Europe principles in the blood area. Secondly, the Council of Europe is active today in the area of blood transfusion due to a tragic event, which occurred in 1953 in the Netherlands; following a flooding many of the blood products given for assistance' could not be used due to incompatibilities and differences in labelling. Some words to present the Council of Europe since the organisation is sometimes confused with institutions ofthe European Union: The organisation has been founded in 1949 to establish the principles of democracy and rule of law all over Europe. Since 1989, the year of the fall of the Berlin wall and the opening up of the iron curtain, these principles could be extended to the countries of Central and Eastern Europe. Today this makes the Council of Europe the only pan-European organisation with 41 Member States thus representing more than 750 million people.

The aim of the Guides, as the title implies, is to facilitate the use of international human rights standards by their systematic presentation. Rather than reproducing full texts of various instruments, substantive standards are grouped according to subject-matter in order to enable users to quickly and easily locate the topic they may be looking for. A detailed index, with references to the many international instruments which address the same issue, reinforces this thematic approach. The choice of human rights and health for the second volume in this series highlights the aim of the Guides: to map out the entire range of human rights and fundamental freedoms as they relate to a specific topic. The sheer size of this volume illustrates the number and variety of human rights standards relevant for health. Many of these standards have been generated by organizations dealing with health rather than human rights, and quite a few are found under medical ethics rather than human rights. Subsuming medical ethics under international human rights law is a novel development, pioneered by the Council of Europe.
