Book excerpt: Alors que l'implication bactérienne dans l'étiologie de la maladie parodontale a été validée par le plus grand nombre, un nouveau paradigme a été proposé. Il suppose qu'une contamination parasitaire du biofilm parodontal causerait une amibiase délétère pour le parodonte. Les parasites mis en cause sont deux amibes : Entamoeba gingivalis et Trichomonas tenax. Leur identification repose aujourd'hui sur la combinaison de techniques microscopique et PCR avec chacune leurs avantages et inconvénients. Leur présence dans la cavité buccale étant confirmée, des études ont été menées pour montrer leur pathogénicité sur le parodonte et leur implication dans la maladie parodontale. Cependant, l'absence de validation des postulats de Koch revisités ne le permet pas à l'heure actuelle. Malgré cette absence de validation scientifique et en parallèle des traitements parodontaux conventionnels à haut niveau de preuve, des traitements anti-parasitaires ont été proposés. Le protocole « Bonner-Dunoyé » publié en 2015 en fait partie et promet de « guérir » la parodontite. Ce protocole prévoit un suivi mensuel des patients avec des instructions répétées aux méthodes d'hygiène, l'étude microscopique de leur flore buccale à la recherche des amibes mises en cause et l'usage entre autres : d'eau oxygénée, de bicarbonate de soude salé et d'antibiotiques locaux et systémiques. L'utilité et la validité scientifique de chaque étape de ce protocole sera décryptée dans ce travail afin d'en déterminer sa pertinence en 2021.

Néanmoins, depuis plus d'un siècle, des recherches ont permis d'identifier deux parasites buccaux, Entamoeba gingivalis et Trichomonas tenax, chez des sujets atteints de maladies parodontales. L'objectif de cette thèse était donc de rechercher un lien de causalité éventuel entre la présence de ces parasites et l'apparition de ces maladies. Les données que nous avons recueillies montrent qu'effectivement Entamoeba gingivalis et Trichomonas tenax sont fréquemment rencontrés en présence de gingivite ou de parodontite. Cependant, ces parasites sont également présents chez un pourcentage non négligeable de sujets sains sans que l'on puisse savoir si ces sujets ont un risque plus important de développer une maladie parodontale. Ces données ne nous permettent donc pas de conclure quant à l'implication d'Entamoeba gingivalis et/ou Trichomonas tenax dans l'apparition des maladies parodontales. Pour répondre définitivement à cette question, il est indispensable de poursuivre la recherche de facteurs de virulence chez ces deux parasites et de réaliser de nouvelles études épidémiologiques standardisées.

Après le recueil des données du patient et sondage des poches parodontales, des échantillons ont été prélevés au sein de deux poches différentes. Nous avons mis en culture les bactéries et avons réalisé des PCR afin d'identifier Entamoeba Gingivalis et Trichomonas Tenax. Une prévalence de 14,28 % pour Entamoeba Gingivalis et de 23,8 % pour Trichomonas Tenax a été obtenue. Les résultats montrent qu'il est possible que ces parasites soient impliqués dans l'étiopathogénie parodontale, bien que non indispensables. D'autres études devraient approfondir ce potentiel rôle, afin d'améliorer la compréhension de la maladie.

Les prélèvements ont été examinés en microscopie optique sans préparation et analysés en biologie moléculaire. Résultats : Methanobacter oralis a été détecté sur 4 / 12 (33%) prélèvements réalisés, dont 3 chez les patients présentant des poches parodontales. Un protozoaire a été observé en microscopie optique dans 1 prélèvement où la biologie moléculaire n'a pas permis de retrouver d'archaea. Il n'a pas été possible de différencier Trichomonas tenax ou Entamoeba gingivalis. Discussion et conclusion : Le faible nombre d'échantillons n'a pas permis de tirer des conclusions statistiquement significatives. En outre, le faible nombre de protozoaires observé nous pousse à réévaluer nos techniques de détection.

Il ressort que Trichomonas tenax n'intervient pas dans la genèse de cette maladie. Néanmoins, E. gingivolis peut être un des nombreux éléments infectieux responsables. Il n'a pas été possible de mettre en évidence le fait que cette amibe puisse être seule responsable de la parodontite. Actuellement, le diagnostic repose sur la clinique et la quantification des bactéries par qPCR au niveau des lésions du parodonte. Les traitements obsolètes et actuels via les recommandations de l'AFSSAP sont détaillés. Le traitement et la prophylaxie de la maladie parodontale sont approchés à l'aide de trois cas cliniques de gravité progressive où le matériel distribué et conseillé en officine est mis en exergue.

Une analyse systématique de la littérature des dix dernières années sur le lien entre Entamoeba gingivalis et maladies parodontales est proposée. Seulement sept études d'intérêt ont été identifiées. Les résultats indiquent une forte corrélation du parasite avec les maladies parodontales. De plus, sa capacité à influer sur son milieu de façon agressive en fait un candidat très intéressant à étudier pour mieux comprendre les maladies parodontales. Toutefois, le faible nombre d'études et leur qualité méthodologique faible ne permet pas de conclure quant au rôle précis de E. gingivalis dans les maladies parodontales.
