It describes recent advances in knowledge of the chemistry associated with irrigants and delivery systems, which is of vital importance given that chemical intervention is now considered one of the most important measures in eliminating planktonic microbes and biofilms from the infected tooth. Recommendations are made regarding concentrations, exposure times and optimal sequences. Possible complications related to the use of the different solutions are highlighted, with guidance on response. In addition, clinical protocols are suggested on the basis of both clinical experience and the results of past and ongoing research. Throughout, a practical, clinically oriented approach is adopted that will assist the practitioner in ensuring successful endodontic treatment.

La préparation mécanique seule, permettant la mise en forme des canaux, est insuffisante et doit impérativement être complétée par une préparation chimique: l'irrigation. C'est une étape incontournable qui va conditionner la réussite du traitement endodontique Elle doit permettre le nettoyage et la désinfection de l'ensemble du réseau canalaire. De nombreuses solutions d'irrigation sont ainsi proposées au chirurgien-dentiste. Elles comportent chacune leurs avantages et inconvénients car aucune ne permet de remplir à elle seule les propriétés de la solution idéale. De plus, la complexité anatomique du système endo canalaire et la multiplicité des espèces bactériennes en présence complique cette étape. Divers systèmes ont ainsi été mis au point pour faciliter et optimiser cette irrigation. Certains d'entre eux permettent également d'activer la solution dans le but de potentialiser ses effets. L'objectif de ce travail est de rappeler l'importance de l'irrigation en endodontie en précisant le rôle des différentes solutions d'irrigation mises à notre disposition. Il détaille également les différents systèmes et méthodes mises en œuvre pour rendre cette irrigation la plus efficace et ainsi réaliser le traitement endodontique dans les meilleures conditions.

Available in PDF, EPUB and Kindle. Book excerpt: Introduction : la désinfection du système intra-canalaire au cours d'un traitement endodontique est essentiellement assurée par l'irrigation. Actuellement, différents systèmes s'offrent aux praticiens afin d'optimiser l'efficacité des solutions d'irrigation canalaires. Très peu d'études s'intéressent à l'utilisation de ces différents systèmes en pratique libérale. Matériels et méthode : Un questionnaire a été envoyé par courriels aux Conseils de l'Ordre départementaux constituants la Nouvelle-Aquitaine. De plus, il a été diffusé via les réseaux sociaux sur des groupes privés réservés aux chirurgiens-dentistes. Résultats: 164 questionnaires ont pu être exploités. Notre échantillon est représentatif de la population en Nouvelle- Aquitaine en ce qui concerne le sexe, mais il ne l'est pas pour l'année de diplôme ainsi que pour la répartition géographique. 34% des praticiens ont recours à un système d'activation. Les systèmes les plus fréquemment utilisés sont les ultrasons. La majorité des praticiens n'ayant pas recours à l'activation l'explique par un manque de connaissance et de formation. Discussion : les fréquences d'utilisation obtenues sont nettement supérieures à l'unique étude s'intéressant au recours à un système d'activation, menée en Arabie Saoudite. La participation à une formation continue sur l'activation, ainsi que l'apparition sur le marché d'un système simple et peu coûteux sembleraient augmenter la fréquence d'utilisation d'un système d'activation. Conclusion: chaque système d'activation présente ses avantages et ses inconvénients. Bien que l'activation de la solution d'irrigation ne soit pas encore ancrée dans les pratiques professionnelles, il est intéressant de voir que la majorité des praticiens ayant participé à cette étude sont intéressés par une formation sur l'irrigation.
