Book excerpt: The initial objective of this work was to reduce stress incontinence surgery from a major surgical procedure (requiring up to ten days in hospital) to a minor day-care operation. From the beginning it was clear that the two major impediments to achieving this goal were post operative pain and urinary retention. Addressing these problems became a long and winding road and culminated in the Integral Theory. The IVS 'tension-free' tape operation was inspired by Dr Robert Zacharin's anatomical studies. Though Zacharin suggested that the ligaments and muscles around the urethra were important for urinary continence control, he did not say how. The observation that implanted foreign materials created scar tissue led to the hypothesis that a plastic tape inserted in the position of the pubourethrallig ament, would leave behind sufficient scar tissue to reinforce that ligament, which would then anchor the muscles for urethral closure. In September 1986, two prototype Intravaginal Sling operations were per formed. A Mersilene tape was inserted with neither tension nor elevation, in the position of the pubourethral ligament. Restoration of continence was immediate and both patients were discharged on the day following surgery without require ment for catheterization. There was minimal pain, and immediate restoration of continence. After six weeks the tapes were removed. Both patients were still conti nent at last review 10 years later. The results appeared to confirm the importance of a midurethral anchoring point.

The tropical diseases such as malaria and yellow fever plagued them just as they had the earlier French effort. The housing stock left behind by the French was dilapidated and inadequate. About a hundred sets of beautifully drafted architectural plans left by the French came in handy for locating drains, etc., as the Americans made repairs to existing buildings. Some workers found insect ridden rooms in adjacent towns while others lived in tents or thatched huts near construction sites. Not wanting to endanger the lives of their families, most men left their wives and children behind. What started out as a cesspool of disease and loneliness eventually emerged as a little piece of paradise for its Canal Zone residents. This book tells some of the stories of the various townsites scattered along the fifty miles of the Panama Canal Zone between the Atlantic and the Pacific Oceans. It also shares the fond memories of a few of its residents whose hometowns have changed since the Panama Canal was turned over to Panama on December 31, 1999, and the Canal Zone as they knew it was no more.

Book excerpt: Using data for Germany and 23 other economies in Eastern and Western Europe, this paper estimates the monetary returns to education acquired under communism more than 10 years after the fall of the Berlin Wall. We show that, in the 2000s, Eastern European workers who completed their education under communism earned in the 2000s similar returns to their education as did workers belonging to the same age cohorts who studied in Western Europe. This might suggest that education under communism is still as valuable as education attained in Western Europe. However, individuals educated under communism are more likely than their Western counterparts to be unemployed, retired or disabled, and therefore to earn lower or zero returns to their education. Moreover, when we allow the returns to pre- and post-secondary education to differ, we find that senior males who have attained only primary or secondary education under communism are penalized in the post-transition Eastern European labour markets, and that those who have completed post-secondary education under communism enjoy in these markets higher payoffs to their education than similarly educated Western European individuals who are employed in the West.
