Book excerpt: From the crude battlefield surgery of Revolutionary times to the birth of modern clinical medicine, the nineteenth century witnessed impressive developments in the medical sciences and a concomitant growth in the prestige of the medical practitioner. In France this phenomenon had important implications for literature as writers scrambled to give legitimacy to their enterprise by allying themselves with science. Overflowing its traditional banks, medical discourse inundated the field of French literature, particularly in the realist and naturalist movements. The literati's enthrallment with medicine and their subservient adoption of a medical model in the creation of their plots and characters have not previously been seriously questioned. In Medical Examinations, Mary Donaldson-Evans corrects this oversight. Exploring six novels and two short stories published during the Second Empire and the early Third Republic, she argues that there was a growing resistance to medicine's linguistic and professional hegemony, a resistance fraught with ideological implications. Tainted by a subtle?and sometimes not so subtle?anti-Semitism, some of the fiction of this period adopts counterdiscursive strategies to tar the physician with his own brush. As these essays illustrate, the development of bioethics in Germany does not follow a linear line of progressiveness, but rather retains a sense of the traditional ethos of the guild. Book excerpt: An evidence-based guide for doctors diagnosing, testing, and treating children with PANDAS (Pediatric Autoimmune Neuropsychiatric Disorder Associated with Strep infections). In the early 1990s, a group of researchers at the National Institute of Mental Health began collecting data on children who had developed neuropsychiatric disorders (OCD and tics) following infections. They found evidence that antineuronal antibodies had developed in some of these children which attacked the basal ganglia region of the brain. MRI and PET scans in these children demonstrated inflammatory changes in the basal ganglia as well. In 1997, the researchers published the first article to describe this syndrome that they named PANDAS (Pediatric Autoimmune Neuropsychiatric Disorder Associated with Strep infections). In PANDAS, an autoimmune attack on the brain occurs following a Strep infection. PANS (Pediatric Acute-onset Neuropsychiatric Syndrome) is a broader term that also includes cases following exposure to other infections, toxins, and even stress. Clinicians treating children with PANS and PANDAS have found that antibiotics targeted at the offending organisms, steroids, and IVIG results in marked improvement and occasionally complete remission of the neuropsychiatric symptoms. Disturbing symptoms consistent with many DSM-5 psychiatric disorders manifest in patients with PANS and PANDAS—yet we know that there is a biologic basis for the changes in these children. As a result, these disorders require us, as physicians, to view mental illness in an entirely new way. Resistance to this change in paradigm has made PANS and PANDAS difficult for clinicians to diagnose, unbearable for parents to endure, and controversial for scholars to accept. As such, there is no recognized standard of care. We have written this work in an effort to change that. This is a textbook by physicians for physicians. It was written to bring back some of the art of medicine to physicians caring for a group of children and families who really need it. PANS and PANDAS are complex disorders that demand a rich, multifaceted response with novel treatment approaches. The material in this book is assembled from the peer-reviewed medical literature, in combination with over thirty years of clinical experience caring for the sickest patients, both in and out of the hospital. Book excerpt: While the writings of early modern medical practitioners habitually touch on performance and ceremony, few illuminate them as clearly as the Protestant physicians Felix Platter and Thomas Platter the Younger, who studied in Montpellier and practiced in their birth town of Basle, or the Catholic physician Hippolytus Guarinonius, who was born in Trent, trained in Padua and practiced in Hall near Innsbruck. During his student years and brilliant career as early modern Basle's most distinguished municipal, court and academic physician, Felix Platter built up a wide network of private, religious and aristocratic patients. His published medical treatises and private journal record his professional encounters with them as a healer. They also offer numerous vivid accounts of theatrical events experienced by Platter as a scholar, student and gifted semi-professional musician, and during his Grand Tour and long medical career. Here Felix Platter's accounts, many unavailable in translation, are examined together with relevant extracts from the journals of his younger brother Thomas Platter, and Guarinonius's medical and religious treatises. Thomas Platter is known to Shakespeare scholars as the Swiss Grand Tourist who recorded a 1599 London performance of Julius Caesar, and Guarinonius's descriptions of quack performances represent the earliest substantial written record of commedia dell'arte lazzi, or comic stage business. These three physicians' records of ceremony, festival, theatre, and marketplace diversions are examined in detail, with particular emphasis on the reactions of 'respectable' medical practitioners to healing performers and the performance of healing. Taken as a whole, their writings contribute to our understanding of many aspects of European theatrical culture and its complex interfaces with early modern healthcare: in carnival and other routine manifestations of the Christian festive year, in the extraordinary performance and ceremony of court festivals, and above all in the rarely welcomed intrusions of quacks and other itinerant performers.

Book excerpt: Women physicians in nineteenth-century America faced a unique challenge in gaining acceptance to the medical field as it began its transformation into a professional institution. The profession had begun to increasingly insist on masculine traits as signs of competency. Not only were these traits inaccessible to women according to nineteenth-century gender ideology, but showing competence as a medical professional was not enough. Whether women could or should be physicians hinged mostly on maintaining their femininity while displaying the newly established standard traits of successful practitioners of medicine. Women Physicians and Professional Ethos provides a unique example of how women influenced both popular and medical discourse. This volume is especially notable because it considers the work of African American and American Indian women professionals. Drawing on a range of books, articles, and speeches, Carolyn Skinner analyzes the rhetorical practices of nineteenth-century American women physicians. She redefines ethos in a way that reflects the persuasive efforts of women who claimed the authority and expertise of the physician with great difficulty. Descriptions of ethos have traditionally been based on masculine communication and behavior, leaving women’s rhetorical situations largely unaccounted for. Skinner’s feminist model considers the constraints imposed by material resources and social position, the reciprocity between speaker and audience, the effect of one rhetor’s choices on the options available to others, the connections between ethos and genre, the potential for ethos to be developed and used collectively by similarly situated people, and the role ethos plays in promoting social change. Extending recent theorizations of ethos as a spatial, ecological, and potentially communal concept, Skinneridentifies nineteenth-century women physicians’ rhetorical strategies and outlines a feminist model of ethos that gives readers a more nuanced understanding of how this mode of persuasion operates for all speakers and writers.
