I have written it because I have not been able to find any small work upon the subject which does not contain glaring errors. The correct books are too large; the small books are out of date and repeat such well-worn myths as that the aortic second sound is normally louder than the pulmonic second, that aortic regurgitant murmurs are usually best heard in the sec ond right interspace, that a hypertrophied left auricle can produce dulness and pulsation near the left sternal border, that systolic re traction at the cardiac apex means adherent pericardium, that epi gastric pulsation denotes hypertrophy of the right ventricle, etc. Further, none of the smaller text-books contains any adequate ao count of muscle sounds, of pulrnonary atelectasis, or of adherent pericardium. To record the well-known but often forgotten truth on such matters as these has seemed to me of importance in small books as well as in encyclopaedic treatises. Medicine, I have been anxious to place a similar means Of overcoming them within the reach Of all, free from the cost of time and labour at which the advantage was procured for myself. My original drawings were but roughly made in the hurry of a student's life - they have therefore been cast aside, and their plan, only, retained in this work, the plates of which are taken-from fresh specimens of disease, and have been executed by a skilful and practised artist. In the pages which treat of acoustics, I have attempted to collect, in a simple and intelligent form, the principles upon which the physical diagnosis of disease within the chest is based, freely illustrating them with examples, in order more closely to connect the theories of the science with the practice of the art. The succeeding chapters are principally occupied with dissertations on the plates, and directions for educating the organs of sense to appreciate the physical signs of disease. My endeavour has been throughout to produce a short, simple, and strictly practical work, which should, if pos sible, omit nothing absolutely essential to a fundamental knowledge of the subject, and yet contain nothing that could safely be omitted. I must confess, however, that the difficulties I have encountered on my way leave me no hope of having kept this happy middle course without digressions on one side or the, Other. When I have ventured to make definite statements on subjects fairly open to discussion, I trust to be acquitted of presumption, on the grounds that these subjects could not be left unnoticed, while the Objects and limits of my book precluded the introduction of lengthy arguments. These signs, as representing certain physical conditions, are generally available, and their char acters relating to pitch and quality are highly significant. The names exaggerated bronchial whisper, whispering bronchophony or bronchophonic whisper, and cavernous whisper, although, perhaps, not, intrinsically, the best which might have been devised, have the advantage of corresponding with the names commonly applied to correlative signs produced by the loud voice. The author would state, as a feature of the work, the recognition of the principle that the constancy of association of certain ab normal sounds with certain physical conditions constitutes the only reliable proof of the validity of the former as representing the latter. It is inconsistent with this principle to undertake to determine a priori signs to which certain physical conditions should give rise, and still more, on the other hand, to infer the existence of certain i physical conditions from certain abnormal sounds. At the request of many of his pupils, he published a series of lectures during the years 1840 - 1, in the Medi cal Examiner, which, with some additional lectures, were collected into the first edition of the present work. The form of lectures in the first edition was retained as the most convenient; some of them were condensed by a careful hand from the oral lectures of the author; the greater part, however, were written by himself, forming the substance of the course. They necessarily were less full than the lectures as delivered, and the illustrations were of course excluded; but they comprised the most important parts of the subject, such as are most worthy of being remembered, and seem essential to a knowledge of the subject. In the present edition the work is divided into the usual form of chapters, arranged according to the subjects of which they treat. The sounds, healthy and morbid, constitute what are known as physical signs. Frequently, for the sake of brevity, the term signs, without the word physical, is used to denote these sounds. Conven tionally, physical signs, or signs, are terms employed in a sense of contradistinction to the term symptoms. The signs are distinguished, of course, as normal or healthy, and abnormal or morbid. The sounds which constitute signs represent certain physical conditions pertaining to the chest. The normal or healthy signs represent physical conditions existing when the organs are not affected by disease; the abnormal or morbid signs represent physical conditions which are deviations from those of health, being incident to the various diseases of the chest. The physical conditions represented by Signs may be distinguished as normal or healthy, and abnormal or morbid conditions.
