Life-long nutrition studies are not possible in humans because of their long survival. Studies over two to six years in healthy adult humans have, however, shown that a 20% reduction in food or calorie intake slows many indices of normal and disease-related aging. Thus, it is widely believed that long-term reduction in calorie or food intake will delay the onset of age-related diseases such as heart disease, diabetes and cancer, and so prolong life. Over the last 20 or more years there has been a progressive rise in food intake in many countries of the world, accompanied by a rising incidence of obesity. Thus our increasing food and calorie intake has been linked to the rising incidence of cardiovascular disease and diabetes in early adult life. Scientists have established that caloric intake, macronutrient composition of the diet, basal and resting metabolic rate, thermic response to food, energy expenditure associated with movement and physical activity, and preferential storage of the surplus of calories as fat or lean tissues are critical determinants of energy balance and body weight. While much has been learned, the field is poised for major advances with the advent of a variety of imaging techniques, progress in quantitative and molecular genetics, use of transgenic rodent models and of breeding experiments with informative inbred strains, availability of stable isotopes for metabolic and behavioral studies, and a growing number of useful experimental animal and human models. This volume takes an integrative approach to obesity. While the effect of physical activity and exercise on cancer may be partially mediated through obesity control, it is clear that considerable research is required and is ongoing at both the molecular and clinical levels to better understand the associated mechanisms and to develop optimal exercise strategies. This volume will contain chapters on the effect of exercise on biological pathways in tumor growth, state art exercise strategies and cutting edge research focused on different cancers and patient groups. It will provide an important volume in this series on energy balance and cancer and a basis for ongoing research, experimental approaches and application of evidence based practices to clinical care for patients with cancer.
