Book excerpt: Cette thèse s'attache à étudier la qualité des diagnostics portés aux urgences au cours de l'année 2004, au sein du Centre Hospitalier Général de l'hôpital de Verdun. L'état actuel des services d'urgences en France y est décrit, ainsi qu'un bref rappel historique et évolutif. La fréquentation et l'activité de ces services ne cessent de croître. De ces constatations découle une nécessité constante d'évaluation de la qualité du travail effectué, par l'intermédiaire d'Evaluation des Pratiques Professionnelles. L'étude porte sur 657 patients passés par les urgences au cours de l'année 2004 et hospitalisés dans un service de médecine interne à orientation digestive, diabétologique, oncologique et néphrologique. Elle s'attache à comparer les diagnostics posés à la sortie de ces deux services; les discordances diagnostiques sont isolées après plusieurs étapes, puis classées par mécanismes de discordance. Cette étude est complétée par l'analyse des conséquences de ces discordances, à savoir erreur d'orientation et retard thérapeutique. Au final, l'étude retrouve 41 discordances diagnostiques, soit 6,1 %. Parmi celles-ci, on retrouve 6 erreurs d'orientation, et 15 retards d'initiation du traitement. L'objectif n'est pas de se contenter de ces résultats, mais de chercher les causes de ces erreurs afin de les éviter par la suite. C'est en équipe avec les autres services que le travail des médecins urgentistes pourra s'améliorer, en particulier avec les médecins internistes, dans l'objectif d'une prise en charge globale et pluridisciplinaire des patients.

Book excerpt: L'objectif de notre étude est d'évaluer la fiabilité du diagnostic posé lors du passage dans la salle d'accueil des urgences vitales. Après analyse des dossiers médicaux aux urgences et des courriers de sortie d'hospitalisation, nous avons établi un taux de concordance diagnostique entre les diagnostics principaux posés suite au passage dans la SAUV et les diagnostics établis suite à l'hospitalisation. 512 dossiers ont été traités, 349 ont été retenus pour l'exploitation de ce travail. Le taux de concordance diagnostique global est de 94.2% tout cadres nosologiques confondus, il existe cependant 4.6 % de vrai discordant. Durant l'hospitalisation 16% des diagnostics ont pu être affinés par des examens complémentaires spécifiques ne pouvant être fait au service d'urgence, ou pouvant être différés. Le diagnostic posé à la SAUV semble donc fiable dans une majorité des cas, les erreurs diagnostiques concernent essentiellement les étiologies d'une dyspnée aiguë. Une formation continue des médecins urgentistes en pneumologie et cardiologie pourrait optimiser certains diagnostics, ainsi que la sensibilisation à la prise en charge de ces pathologies. Pour améliorer encore la pertinence diagnostic, un rappel sur les principes du codage semble nécessaire afin de limiter le nombre de diagnostic symptomatique. Enfin, il pourrait être envisager une étude prospective sur ce sujet, débutant de l'appel au SAMU jusqu'à la sortie d'hospitalisation, dans le but d'évaluer l'ensemble de la chaîne de soins.

It is written primarily for public health actors. As the social determinants of mental health and psychosocial problems occur across sectors, half of the tools in the accompanying toolkit cover MHPSS assessment issues relevant to other sectors as well as the health sector.

But it is not only its frequency that makes it a prime dataset for such research, it is also that pottery embeds many dimensions of the human experience, ranging from the purely technical to the eminently symbolic. The aim of this book is to provide a cutting-edge theoretical and methodological framework, as well as a practical guide, for archaeologists, students and researchers to study ceramic assemblages. As opposed to the conventional typological approach, which focuses on vessel shape and assumed function with the main goal of establishing a chronological sequence, the proposed framework is based on the technological approach. Such an approach utilizes the concept of chaîne opératoire, which is geared to an anthropological interpretation of archaeological objects. The author offers a sound theoretical background accompanied by an original research strategy whose presentation is at the heart of this book. This research strategy is presented in successive chapters that are geared to explain not only how to study archaeological assemblages, but also why the proposed methods are essential for achieving ambitious interpretive goals. In the heated debate on the equation stating that “pots equal people”, which is a rather fuzzy reference to assumed relationships between (mostly) ethnic groups and pottery, technology enables us to propose with conviction the equation “pots equal potters”. In this way, a well-founded history of potters is able to achieve a much better cultural and anthropological understanding of ancient societies.​
