Book excerpt: Estimates indicate that as many as 1 in 4 Americans will experience a mental health problem or will misuse alcohol or drugs in their lifetimes. These disorders are among the most highly stigmatized health conditions in the United States, and they remain barriers to full participation in society in areas as basic as education, housing, and employment. Improving the lives of people with mental health and substance abuse disorders has been a priority in the United States for more than 50 years. The Community Mental Health Act of 1963 is considered a major turning point in America's efforts to improve behavioral healthcare. It ushered in an era of optimism and hope and laid the groundwork for the consumer movement and new models of recovery. The consumer movement gave voice to people with mental and substance use disorders and brought their perspectives and experience into national discussions about mental health. However over the same 50-year period, positive change in American public attitudes and beliefs about mental and substance use disorders has lagged behind these advances. Stigma is a complex social phenomenon based on a relationship between an attribute and a stereotype that assigns undesirable labels, qualities, and behaviors to a person with that attribute. Labeled individuals are then socially devalued, which leads to inequality and discrimination. This report contributes to national efforts to understand and change attitudes, beliefs and behaviors that can lead to stigma and discrimination. Changing stigma in a lasting way will require coordinated efforts, which are based on the best possible evidence, supported at the national level with multiyear funding, and planned and implemented by an effective coalition of representative stakeholders. Ending Discrimination Against People with Mental and Substance Use Disorders: The Evidence for Stigma Change explores stigma and discrimination faced by individuals with mental or substance use disorders and recommends effective strategies for reducing stigma and encouraging people to seek treatment and other supportive services. New advances in our fundamental understanding of the causal mec- nisms involved in the development ofaddiction may be necessary to advance our success in developing new forms of prevention and treatment for alcohol and drug abuse. The work in this book builds on the work of numerous previous investi- tors who have been drawn to investigate this topic. As you will notice in the extensive reference list, there have been hundreds of articles published on this topic. Although each of these references has added a small piece to our und- standing of the relationship between personality and alcohol abuse, the majority of these studies have been done on clinical samples and often involved no control groups or poorly matched control groups. Several important previous longitu- nal investigations have been conducted,but these investigations have usually not included general population samples or comprehensivepersonality test batteries.

Book excerpt: The Substance Abuse and Mental Health Services Administration (SAMHSA) estimated that in the year 2002, 19.5 million Americans, which includes individuals 12 years of age and older, were current users of illicit drugs. The purpose of this study was to examine the relationship between personality traits, as operationalized by the NEO-PI-R subscales, and self-identified drug of choice. While there is a significant body of literature substantiating the existence of personality types, little research has investigated their relationship to substance use. The study represented a correlational design. Male participants in recovery, ranging in age from 18 to 65, and living in a large Northeastern metropolitan area, were recruited via 12-step program meetings to complete two instruments - the NEO-PI-R and the Substance Abuse History Inventory (modified). Participants were stratified by drug of choice. MANOVA was used to assess variations in personality traits (dependent variable) by drug of choice. The overall MANOVA was statistically significant, Pillai's Trace = 3.40 and partial [eta] 2 = .85, suggesting that drug of choice had a significant effect on the multivariate combination of personality trait scores. Those identifying CNS stimulant as their drug of choice scored higher on the Extraversion domain than did CNS depressant or opioid users; those referring to themselves as CNS depressant or opioid users scored higher on Neuroticism than did CNS stimulant users. This knowledge could assist clinicians to motivate clients to seek stimulating activities that do not involved CNS stimulants. Implications for social change include the possibility of drug use and abuse interventions that are more effective and thus reduce the individual and societal costs associated with substance abuse.

The participants to the study reported in this book were invited to respond to standardised self-report measures of alcohol and drug use, impulsivity and aggressiveness. Although they do not suggest causation, the computed outcomes from the data collected revealed that impulsiveness was significantly and positively correlated with self-reported drug and alcohol use. The association between the various measures of impulsivity and drug and alcohol use persisted, while no relationship was observed between aggressiveness and drug and alcohol use among the participants. In a stepwise linear regression analysis based on the Alcohol Use Disorders Identification Test and the Drug Abuse Screening Test, self-reported drug and alcohol use was highly related to impulsiveness, with the sensation seeking and lack of premeditation components significantly contributing to the prediction. The results are discussed in terms of the need for impulsivity-targeted interventions for individuals who are at a high risk of drug and alcohol misuse.

Book excerpt: Zuckerman received his Ph.D. in psychology from New York University, Graduate School of Arts and Science in 1954 with a specialization in clinical psychology. After graduation, he worked for three years as a clinical psychologist in state hospitals in Norwich, Connecticut and Indianapolis, Indiana. While in the latter position the Institute for Psychiatric Research was opened in the same medical center where he was working as a clinical psychologist. He obtained a position there with a joint appointment in the department of psychiatry. This was his first interdisciplinary experience with other researchers in psychiatry, biochemistry, psychopharmacology, and psychology. His first research areas were personality assessment and the relation between parental attitudes and psychopathology. During this time, he developed the first real trait-state test for affects, starting with the Affect Adjective Check List for anxiety and then broadening it to a three-factor trait-state test including anxiety, depression, and hostility (Multiple Affect Adjective Check List). Later, positive affect scales were added. Toward the end of his years at the institute, the first reports of the effects of sensory deprivation appeared and he began his own experiments in this field. These experiments, supported by grants from NIMH, occupied him for the next 10 years during his time at Brooklyn College, Adelphi University, and the research labs at Albert Einstein Medical Center in Philadelphia. This last job was his second interdisciplinary experience working in close collaboration with Harold Persky who added measures of hormonal changes to the sensory deprivation experiments. He collaborated with Persky in studies of hormonal changes during experimentally (hypnotically) induced emotions. During his time at Einstein, he established relationships with other principal investigators in the area of sensory deprivation and they collaborated on the book Sensory Deprivation: 15 years of research edited by John Zubek (1969). His chapter on theoretical constructs contained the idea of using individual differences in optimal levels of stimulation and arousal as an explanation for some of the variations in response to sensory deprivation. The first sensation seeking scale (SSS) had been developed in the early 1960's based on these constructs. At the time of his move to the University of Delaware in 1969, he turned his full attention to the SSS as the operational measure of the optimal level constructs. This was the time of the drug and sexual revolutions on and off campuses and research relating experience in these areas to the basic trait paid off and is continuing to this day in many laboratories. Two books have been written on this topic: Sensation Seeking: Beyond the Optimal Level of Arousal, 1979; Behavioral Expressions and Biosocial Bases of Sensation Seeking, 1994. Research on sensation seeking in America and countries around the world continues at an unabated level of journal articles, several hundred appearing since the 1994 book on the subject.
