Book excerpt: Research demonstrates that there is a high rate of non-compliance with regard to the usage of Personal Protective Equipment (PPE) in the healthcare setting. For example, a study undertaken in a U.S. Emergency Room (ER) observed 88 ER personnel via a video camera. During the study 304 procedures were observed. In spite of the high risk environment, 22% of the procedures were performed without any or inadequate eye protection and 32.2% were completed without a mask. In a separate peri-operative study in the U.S. that focused on Post Anesthesia Care Unit (PACU) Nurses the study revealed that only 31.3% of the nurses would use PPE for low-risk patients and 68.8% for high-risk patients and only 75% of the nurses would consent to wear PPE if they were actually anticipating contact with blood. 83.3% conceded that they would use goggles if they knew for sure that the patient was HIV positive (Neo, Edward, and Mills, 2012). These statistics are ubiquitous across the healthcare system in the United States. Studies have been performed comparing Personal Protective Equipment on-line didactic only training to on-line didactic training with an added interactive training component. In one particular study the resultant post-training testing of the concepts that included an interactive component demonstrated a far better retention rate at one month post training when students were asked to perform an unannounced demonstration (Williams and Carnahan, 2013). Healthcare Acquired Infections (HAIs) are prevalent in the United States and according to the Centers for Disease Control 1 in 25 patients on average, per day, acquired a hospital-based infection in 2011. This statistic adds up to 722,000 HAIs annually of which approximately 75,000 of those patients lose their lives to the infection or the sequelae of the process (""CDC HAIs: Data & Statistics,"" 2014). Non-compliance with the use of Personal Protective Equipment can put the healthcare worker at risk of exposure to infectious agents as well as put the patient at risk of contracting an infection. This research study proposal is being conducted to determine if adding an interactive training component to an existing annual on-line didactic only training module will improve compliance with the use of Personal Protective Equipment. The research process will involve an unannounced baseline testing of current knowledge retention of a current on-line only didactic training module, followed up by a new interactive training session with the final step will being an unannounced third testing session approximately one to two months post interactive instructional sessions. The proposed research project timeframe is expected to last 6 months.

Book excerpt: Background: Due to the high potential of transferring infectious diseases and/or organisms among patients, themselves, and the community, healthcare workers (HCWs) must be knowledgeable and confident in selecting the appropriate type of personal protective equipment (PPE), and the use in technique when putting on (donning) and removing (doffing) PPE based on the level of isolation precautions required for the patient being cared for. Project Purpose: The purpose of this project is to determine whether assessing the knowledge and actual practice with observing, and utilizing an innovative approach of video and educational tools to isolation precautions would improve the consistency of compliance with PPE selection and use among registered nurses (RNs) and nursing assistants (NAs) on a medical-surgical unit. Methodology: The theoretical framework incorporated in the study was Schon's Theory of Reflective Practice. A pre-intervention questionnaire was distributed, and pre- and post-intervention observations were conducted to evaluate PPE selection and use by RNs and NAs. Results: The pre-intervention data from both the knowledge questionnaire and the observations showed various inconsistencies in RNs' and NAs' PPE technique and selection choice. A comparison of the pre- and post-intervention observation data showed that there was significant increase in all four analyzed categories--hand hygiene (45% to 70%), selection of PPE (79% to 80%), sequence of putting on PPE (70% to 85%), and sequence of removing PPE (76% to 85%). Recommendations: Future research studies should plan for a longer period of time to assess and collect pre- and post-intervention data, and include a larger sample. An expanded research project should also examine the correlation between HCWs' compliance rates with PPE and isolation precautions, and the incidence of healthcare-associated infections (HAIs). Surgical teams caring for patients undergoing complex surgical procedures may create an opportunity for surgical errors impacting patient safety. The purpose of this project is to improve consistent standardize Surgical Safety Checklist (SSC) use and briefings in the orthopedic perioperative care setting in a large Midwestern hospital, thereby supporting a culture of safety through staff engagement and a team-based communication approach. Watson’s Theory of Human Caring guided this project through the theoretical concepts of presence and faith. An initial implementation of the SSC and briefings revealed a decrease in compliance, teamwork, and communication and staff engagement. Introduction of a team training programing, Team Strategies and Tools to Enhance Performance and Patient Safety TeamSTEPPS provided improved communication and enabled more efficient surgical teams, thus improving the compliance of both the SSC and briefings. Further evaluation of compliance and development of a model of care translated the technical and safety processes involved in the operating room into an enhanced patient experience. Conclusions derived from this project suggest that SSCs and briefings provide the standard of care to foster patient safety, teamwork, and communication to aid in the prevention of surgical errors.

Book excerpt: Confronted with worldwide evidence of substantial public health harm due to inadequate patient safety, the World Health Assembly (WHA) in 2002 adopted a resolution (WHA55.18) urging countries to strengthen the safety of health care and monitoring systems. The resolution also requested that WHO take a lead in setting global norms and standards and supporting country efforts in preparing patient safety policies and practices. In May 2004, the WHA approved the creation of an international alliance to improve patient safety globally; WHO Patient Safety was launched the following October. For the first time, heads of agencies, policy-makers and patient groups from around the world came together to advance attainment of the goal of "First, do no harm" and to reduce the adverse consequences of unsafe health care. The purpose of WHO Patient Safety is to facilitate patient safety policy and practice. It is concentrating its actions on focused safety campaigns called Global Patient Safety Challenges, coordinating Patients for Patient Safety, developing a standard taxonomy, designing tools for research policy and assessment, identifying solutions for patient safety, and developing reporting and learning initiatives aimed at producing 'best practice' guidelines. Together these efforts could save millions of lives by improving basic health care and halting the diversion of resources from other productive uses. The Global Patient Safety Challenge, brings together the expertise of specialists to improve the safety of care. The area chosen for the first Challenge in 2005-2006, was infection associated with health care. This campaign established simple, clear standards for hand hygiene, an educational campaign and WHO's first Guidelines on Hand Hygiene in Health Care. The problem area selected for the second Global Patient Safety Challenge, in 2007-2008, was the safety of surgical care. Preparation of these Guidelines for Safe Surgery followed the steps recommended by WHO. The groundwork for the project began in autumn 2006 and included an international consultation meeting held in January 2007 attended by experts from around the world. Following this meeting, expert working groups were created to systematically review the available scientific evidence, to write the guidelines document and to facilitate discussion among the working group members in order to formulate the recommendations. A steering group consisting of the Programme Lead, project team members and the chairs of the four working groups, signed off on the content and recommendations in the guidelines document. Nearly 100 international experts contributed to the document (see end). The guidelines were pilot tested in each of the six WHO regions--an essential part of the Challenge--to obtain local information on the resources required to comply with the recommendations and information on the feasibility, validity, reliability and cost-effectiveness of the interventions.
