The problem has elicited a variety of societal and public policy responses over the years, concomitant with fluctuations in the economy and changes in the demographics of and attitudes toward poor and disenfranchised citizens. In recent decades, federal agencies, nonprofit organizations, and the philanthropic community have worked hard to develop and implement programs to solve the challenges of homelessness, and progress has been made. However, much more remains to be done. Importantly, the results of various efforts, and especially the efforts to reduce homelessness among veterans in recent years, have shown that the problem of homelessness can be successfully addressed. Although a number of programs have been developed to meet the needs of persons experiencing homelessness, this report focuses on one particular type of intervention: permanent supportive housing (PSH). Permanent Supportive Housing focuses on the impact of PSH on health care outcomes and its cost-effectiveness. The report also addresses policy and program barriers that affect the ability to bring the PSH and other housing models to scale to address housing and health care needs.

Act, it is important to evaluate the effectiveness of these programs. TH has been an important element of HUD¿s efforts to respond to the housing needs of homeless families and individuals through a continuum of care. This study examines whether TH makes a difference in the lives of the families it serves and whether it is more effective for some homeless people than others. This study follows 179 families in 36 TH programs within five communities for one year after leaving the program. TH programs, and certain characteristics of the programs, were found to be associated with positive outcomes. Illustrations.

Available in PDF, EPUB and Kindle. Book excerpt: This qualitative study examines the lived experience of homelessness from the perspective of women in permanent supportive housing programs. Fifteen formerly homeless women residing in permanent supportive housing, along with eleven service providers, were interviewed to determine patterns of service utilization and unique aspects of women's experience of homelessness. Findings suggest that women's experience of homelessness is characterized by increased vulnerability compared with their male peers and often impacted by the presence of children. This vulnerability appears to influence women's decisions to access shelter and services, with women reporting far less "on the street" homelessness and more willingness to enter and remain in shelter programs than their male counterparts. Women with children present have greater access to mainstream benefits and use these resources, along with limited support from family, to maintain stability while in permanent supportive housing. Participants did not observe significant differences in the services needed by male and female clients, but noted that gender, and investment in traditional gender roles, influences how these services are perceived by recipients. Men were believed to feel more stigmatized by the experience of requesting assistance and more reluctant to seek help than their female counterparts. Women demonstrated greater willingness to access services, but many faced barriers related to a diminished sense of self-efficacy, along with concerns related to securing transportation and childcare. The majority of women interviewed viewed permanent supportive housing as a temporary home that allowed them the time and space to pursue educational, employment, and parenting goals absent the financial pressures they experienced while independently housed. These women, particularly those younger than 35, were confident in their ability to achieve self-sufficiency within one to five years. Older residents expressed somewhat more modest goals related to managing disabilities and navigating pension systems. Women and families are a unique population in permanent supportive housing and face multiple barriers to self-sufficiency. Practitioners and policymakers must begin to recognize these differences in designing programming that addresses this population's specific needs in permanent supportive housing.

Book excerpt: The "Supportive Housing for Homeless Families: Foster Care Outcomes and Best Practices" report describes the outcome evaluation of Cottage Housing Incorporated's Serna Village program in Sacramento, California. Serna Village is a supportive housing program serving homeless families. Outcomes from the program illustrate that it is possible to end recidivism into the child welfare system for homeless families by providing them with permanent housing and comprehensive support services. Although homeless and marginally housed families have high rates of involvement in the child welfare system, this study found that permanent and stable housing, social support and case management services can prevent these disenfranchised families from re-entering the foster care system. The intervention of supportive housing--housing and services focused on the unique needs of adults and their children exiting homelessness--may break the cycle of abuse and neglect among these families. Conducted in 2011, this study involved a sample of 293 children and youth from approximately 150 families who lived with one or more parents in Cottage Housing Incorporated's Serna Village between 2002 and 2009, the first seven years of the program. The findings from this study indicate that comprehensive supportive housing programs following a best-practice model can provide homeless parents and their children with stable living for a significant period of time. Supportive housing programs also may give homeless parents an opportunity to find and maintain employment, work on their education, save a substantial amount of money for move-out costs, learn daily living skills, experience a real-world living situation and prevent re-entry into the child welfare system. The outcomes from this study may help inform policymakers and child welfare administrators with recommendations to better assist marginalized families and save valuable funding dollars. Book excerpt: With U.S. health care costs projected to grow at an average rate of 5.5 percent per year from 2018 to 2027, or 0.8 percentage points faster than the gross domestic product, and reach nearly $6.0 trillion per year by 2027, policy makers and a wide range of stakeholders are searching for plausible actions the nation can take to slow this rise and keep health expenditures from consuming an ever greater portion of U.S. economic output. While health care services are essential to heath, there is growing recognition that social determinants of health are important influences on population health. Supporting this idea are estimates that while health care accounts for some 10 to 20 percent of the determinants of health, socioeconomic factors and factors related to the physical environment are estimated to account for up to 50 percent of the determinants of health. Challenges related to the social determinants of health at the individual level include housing insecurity and poor housing quality, food insecurity, limitations in access to transportation, and lack of social support. These social needs affect access to care and health care utilization as well as health outcomes. Health care systems have begun exploring ways to address non-medical, health-related social needs as a way to reduce health care costs. To explore the potential effect of addressing non-medical health-related social needs on improving population health and reducing health care spending in a value-driven health care delivery system, the National Academies of Science, Engineering, and Medicine held a full-day public workshop titled Investing in Interventions that Address Non-Medical, Health-Related Social Needs on April 26, 2019, in Washington, DC. The objectives of the workshop were to explore effective practices and the supporting evidence base for addressing the non-medical health-related social needs of individuals, such as housing and food insecurities; review assessments of return on investment (ROI) for payers, healthy systems, and communities; and identify gaps and opportunities for research and steps that could help to further the understanding of the ROI on addressing non-medical health-related social needs. This publication summarizes the presentations and discussions from the workshop.

Book excerpt: This document presents the Queer Students of Architecture, Planning, and Preservation's (QSAPP) research into housing for LGBTQ youth experiencing homelessness in New York City. Based at Columbia GSAPP, QSAPP's interdisciplinary project looks at this issue from the various disciplines of the built environment represented at the school: architecture, real estate, planning, and preservation. The book draws on a range of sources--including data from government and social service organizations, operating models of existing organizations in New York, and interviews with service providers and experts in the field--and perspectives in sociology, public health, and advocacy. Funding is often cited as one of the biggest barriers to solving this housing crisis, but an analysis of funding models and strategies does not currently exist. In addition, housing is a design problem but there are no published reports that analyze LGBTQ youth housing from a spatial perspective. QSAPP hopes that by visualizing this issue and highlighting ways in which these shelters fit into specific planning and real estate systems in the city, we can further shed light on the specific needs of LGBTQ youth and help advise on ways forward with these concerns in mind.
