Book excerpt: Women Veterans are the largest growing population Veterans, yet have a significant mental health disparity, greater than both civilian women and Veteran men. This disparity continues in the mental health outpatient treatment options for women Veterans. Veteran Administration (VA) healthcare services may not be suited to their needs as women, and civilian services are not well suited to manage their needs as Veterans. Therefore, this dissertation study used constructivist Grounded Theory methods to explore the experiences of women Veterans when accessing mental health outpatient services, their decision-making process when make the choice to enter mental health service, and aspects of the experience that are important or meaningful to them. Twelve women Veterans revealed meaningful, personal stories on their experiences of trauma and their use of mental health outpatient services. While addressing each of these factors, what emerged from the data was a broader Grounded Theory Process model of how women Veterans process trauma, and the categories of Trauma, Transitions, Identity and Structure. Women Veterans who participated in the study used mental health outpatient services to assist in reestablishing identity after trauma and to propel forward in their lives. This research provides key insight into how women Veterans make healthcare related choices and process traumatic events, like military sexual trauma (MST). This has implications for research, practice, and policy to improve the provision of care for women Veterans.

Book excerpt: Objective : Assess pt perceptions of quality and access to gynecological care through the Veterans Health Administration (VHA) versus outside the VHA. Design : Anonymous cross sectional surveys distributed to VHA patients presenting for gynecological care and collected after visits. Patients : A sample of 356 female Veterans presenting for care the Mission Valley VA Women's Clinic between Nov 16, 2010 and April 10, 2013. Measures : Patients rated the quality and access of current care and over a 12 month period, both within and outside the VHA system. Analysis : The answers to questions regarding quality and access to care at the VA were summarized. Answers to questions comparing the VA versus outside care was dichotomized (better at the VA versus the same or worse) and was analyzed with respect to age (18-39 versus >̲40) and race (White, Black, Asian, Hispanic and other). Results : Gynecological care through the VA was rated as better than outside care by 70% and easier to access by 62%. There was no statistical significant difference reported with respect to age or race.

Book excerpt: Approximately 4 million U.S. service members took part in the wars in Afghanistan and Iraq. Shortly after troops started returning from their deployments, some active-duty service members and veterans began experiencing mental health problems. Given the stressors associated with war, it is not surprising that some service members developed such mental health conditions as posttraumatic stress disorder, depression, and substance use disorder. Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

Book excerpt: The United States military service members dedicate a period of their lives, some their entire life, to protecting and upholding the United States Constitution. While the United States military service members go through many transitions throughout their military career, all but their transition to civilian life include mentors and have a step-by-step action plan or checklist to prepare them for their next position or move. Often, female Veterans and their needs during reintegration into civilian life are overlooked. The purpose of this research study aimed to examine the relationships between perceived stress, satisfaction with life, and reintegration into civilian life for United States female Veterans. The study utilized feminist theory through the critical theory lens and relational-cultural theory (RCT). Data was collected through an anonymous, online survey via SurveyMonkey. The sample consisted of 155 female Veterans. Feminist theory and RCT focus on connections and relationships that guided this study to find whether relationships between female Veterans' reintegration into civilian life were perceived with stress and their level of satisfaction with life. The results indicated that United States female Veterans struggle with reintegration into civilian life with increased perceived stress and reduced feelings of satisfaction with their lives. Based on the research results from this study, additional research is recommended. Future research would benefit from utilizing a mixed-method approach incorporating qualitative and quantitative methods to better understand female Veterans' unique needs.

Book excerpt: Previous research has explored potential barriers to utilizing military sexual trauma(MST)-related care for male and female veterans. These potential barriers include knowledge,stigma, and gender-related barriers. Although providers play a vital role in veterans’ access to MST-related care, little research has examined provider-related factors that may be potential barriers to accessing MST-related care. Specifically, provider gender has been shown to be important to veterans. Provider gender mismatch, or not receiving a provider of the gender of one’s preference, may deter veterans from accessing MST-related care. The current study aimed to: 1) examine provider gender preferences and provider gender mismatch rates among veterans who screened positive for MST; and 2) explore how not receiving a provider of one’s gender preference is related to veterans’ comfort with VHA providers, their perception of the provider competency, and their endorsement of perceived provider barriers when communicating about MST. Approximately 2,500 male and female VHA care utilizing veterans who screened positive for MST were recruited into the study. Data were collected using a cross-sectional design via a mailed survey. A total of 618 male veterans and 1,290 female veterans were included in the final analyses. Data were analyzed using descriptive statistics, one-way MANCOVAs, and factorial MANOVAs stratified by gender. As found in previous research, veterans had significant provider gender preferences regarding discussing MST. The majority of women had a preference for female providers; men had more mixed preferences. Not seeing a gender-preferred provider was associated with more negative perceptions of providers when discussing MST. For both men and women, provider gender preference mismatch was related to greater perceived barriers and lower reported comfort with providers. Results from this study contribute to the literature by providing further data on veterans’ provider gender preferences and how these preferences may be related to veterans’ experiences and perceptions of provider barriers when discussing MST. It is hoped that these findings can be used to improve patient-centered MST-care and inform further research.

Book excerpt: The purpose of this study was to determine the difference in perceived barriers accessing medical care between female and male veterans, to determine the perceptions of satisfaction with healthcare services received, and healthcare differences between dualinsured and single insured-female veterans. One hundred thirty-eight female and male veterans submitted responses to a modified Patient Satisfaction Questionnaire-III electronic survey and fourteen veterans agreed to a face-to-face interview. A one-way ANOVA, a Chi square analysis and a Qualitative Content-Thematic Analysis were utilized to determine the differences in accessing medical care and general satisfaction of medical care received. The results of the study indicated there were no significant differences in medical care access or perceived differences in medical care satisfaction.
