Book excerpt: The purpose of this descriptive correlational study was to determine if the nurses and patients on the medical surgical units of an acute care facility perceived caring behaviors in the same way. A convenience sample of 120 nurse and 120 patient surveys was distributed. Forty-six percent of the nurse surveys and 40% of the patient surveys were returned. Nursing behaviors perceived as most caring by both nurses and patients focused on human needs assistance and the monitoring of patient conditions and demonstration of professional competence. Results of the study indicate that female patients perceived all caring behaviors to be more important than men did, especially in relation to the expression of feelings and teaching/learning behaviors. It was also noted that the longer a nurse had been practicing, the less important they perceived teaching to be. The difference noted in educational levels of nurses was that BSNs and PNs perceived teaching/learning behaviors as more important than did ADN nurses. Future research is recommended which would study ways in which caring behaviors affect patients' outcomes, especially those related to teaching and learning behaviors.

Book excerpt: Aim: The purpose of this qualitative study was to explore and describe the level of understanding of PU development and prevention and perceptions of implementing PU prevention measures in nurses caring for patients in medical-surgical units. Method: The study was conducted in three (3) community hospitals within the same healthcare system using an Interpretive Description approach. Six (6) focus groups were conducted including thirty (30) participants. Findings: Participants described their knowledge of PU development and prevention as good and perceived continuing education as important in maintaining knowledge levels. Two (2) methods of identifying patients at risk for PU development were described: the Braden Scale and common patient diagnoses and conditions known to be associated with patients at risk for PU. Factors perceived to facilitate nurses' implementation of PU prevention measures included personal motivation, use of evidence-based treatment protocols, availability of expert consultants, and leadership support. Barriers to nurses' implementation of PU prevention measures perceived by study participants included the need to prioritize competing patient needs; lack of equipment and supplies; inadequate numbers and competency of staff; patient's lack of or inability to cooperate; and family involvement. Finally, regulatory mandates limiting payment for hospital acquired PU were perceived by participants as a necessary measure and positive motivator in their implementation of PU prevention measures. Implications: Additional studies are needed to further describe differences between medical versus surgical patients and settings, the influence of patient families on nurses' ability to provide care, and the influence of nurse leader and organizational culture on nurse motivation and performance. Organizations should implement evidence-based practice protocols, continue to provide ongoing education regarding PU prevention measures, and seriously consider adoption of the Wound Care Nurse role.

Book excerpt: The purpose of this project was to create a profile of nurses who utilize Jean Watson's caritas processes and acknowledge caring by forming transpersonal caring relationships with all patients. In an acute care, 468-bed healthcare organization located in southeastern United States, on five adult medical surgical units, an overall score of 52% existed on the nurse communication domain within the Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey. This score was well below the national top box of the 76th percentile set for the domain of nurse communication. Within the five units selected, 67 nurse participants completed a pre survey using Nyberg's Caring Assessment Scale. An education presentation based on Jean Watson's Theory of Human Caring was presented to front line nurses within the five units selected. After the presentation, nurses were encouraged to incorporate Watson's caritas processes and establish caring moments with their patients. Four weeks following the education presentation and implementation, post surveys using the Caring Assessment Scale were administered to determine if scores had increased from the pretest. In healthcare, lack of communication can lead medical failures to patient errors, and even patient death. Communication failures account to 70% of 2455 annual sentinel events, and 76% of persons having a sentinel event and with subsequent death. Nurse-physician collaboration is defined as “an interpersonal process where physicians and nurses present with shared objects”. Both physicians and nurses need to have equity in decision-making, and in the responsibility of managing the patient in order to decrease patient mortality and improve quality of patient. In order to evoke effective communication between both parties, there needs to be elements of respect and mutual trust in the relationship. The purpose of this grant proposal is to describe research to explore and measure the differences in perception of collaboration amongst physician and nurses on the medical surgical, and intermediate care units of a southern California hospital. A healthcare association between the distinctive units regarding nurse-physician collaboration differences may suggest interventions to enhance collaboration. Enhanced collaboration may increase the nurse’s job satisfaction, decrease nurse burnout, facilitate retention of nurses and decrease hospital financial strain resulting from high staff turnover rates. Ultimately patient’s benefit from decreased medical errors resulting from miscommunications amongst physicians and nurses.
