Book excerpt: Introduction : le médecin généraliste tient un rôle de pivot dans la prise en charge globale du patient ainsi qu'un rôle essentiel dans ce réseau structuré de l'urgence. Il est souvent le « premier maillon médical de la chaîne des secours », un facteur important dans l'alerte et dans la prise en charge initiale d'une urgence relative ou vitale. L'objectif de ce travail était d'évaluer les besoins et le ressenti des nouveaux médecins généralistes face aux gestes de premiers secours. Méthodes : une étude épidémiologique descriptive de type enquête transversale a été réalisée auprès des 376 anciens internes de médecine générale des promotions 2017 et 2018 inscrits à la Faculté d'Aix Marseille à l'aide d'un auto-questionnaire. Résultats : 119 sujets ont été inclus, 45,4% des nouveaux médecins généralistes n'ont pas renouvelé leur formation et environ 70% n'a jamais pratiqué de gestes de premiers secours. La moitié (48,7%) estime que leur principale difficulté est due à un manque d'expérience. Près de 95% ont estimé la formation actuelle perfectible et souhaitent pour une très large majorité une formation mixte pratique et théorique (54,6%) ou seulement pratique (38,7%) après la fin de leur internat. Discussion : les situations sanitaires exceptionnelles ainsi que la crise du système de santé que nous vivons actuellement, soulignent l'importance de préparer l'ensemble des médecins à réagir rapidement et efficacement dans de telles circonstances. En adéquation avec les besoins ressentis des médecins généralistes, il semble nécessaire d'envisager une formation obligatoire aux gestes de premiers secours en fin de cursus ou au début de leur carrière, grâce à un support adapté comme le e-learning ainsi qu'au moyen d'une nouvelle méthodologie comme le relais pédagogique ou la formation interprofessionnelle.

Book excerpt: Introduction Le Diplôme d'Etudes Spécialisées (DES) de médecine d'urgence va être créé en 2017 et séparé de la formation des internes de médecine générale (IMG). L'objectif de ce travail est de réaliser une fiche d'objectifs pédagogiques pour les IMG en stage des urgences en Ile-de-France (IDF) afin de rapprocher les propositions de formation par les médecins urgentistes des attentes des Départements de Médecine Générale (DMG). Méthode La méthode utilisée a été une étude qualitative par entretiens semi-directifs avec les sept représentants des DMG d'IDF et sept représentants urgentistes d'IDF. Résultats Cinq DMG ont répondu aux entretiens. Ils ont rappelé que la formation des IMG se faisait par l'acquisition de compétences et non par objectifs. Ils n'étaient pas opposés à la réalisation d'une fiche, mais ont affiché leur crainte d'une fiche réductrice des savoirs médicaux. Sept urgentistes ont pu être interrogés. Tous étaient favorables à la réalisation d'une fiche, quelle qu'en soit la forme. Discussion Bien que le projet ait intéressé tous les intervenants, il n'a pas été possible de réaliser une "fiche d'objectifs pédagogiques" en stage des urgences : ces objectifs seraient allés à l'encontre de la formation des IMG par l'acquisition de compétences. En revanche, un prototype de "fiche d'évaluation" a été créé, adaptable aux souhaits de formation des services et des internes. Conclusion Un prototype de "fiche d'évaluation" a été créé en tentant de répondre aux demandes des DMG et urgentistes, en tenant compte des contraintes d'une utilisation pratique réelle. Cette étude se révèle prometteuse pour une meilleure collaboration entre enseignants de différentes spécialités.

Book excerpt: La notion d'urgence médicale est souvent mal perçue par les patients, il s'agit pour eux d'un sentiment très subjectif (notion d'urgence réelle et d'urgence ressentie). L'objectif de ce travail est d'évaluer la perception du degré d'urgence médicale par les patients adultes en étudiant la façon dont ils réagissent face aux symptômes qu'ils présentent. Il s'agit d'une enquête transversale, descriptive de type populationnelle ouverte, multicentrique, réalisée entre le 30 mai et le 30 août 2013. Un questionnaire à destinée des patients a été élaboré, composé de 10 items reprenant chacun une situation clinique évoquant une discipline médicale différente: gastro-entérite aiguë virale bénigne (n°1), rhinopharyngite (n°2), traumatisme crânien avec perte de connaissance (n°3), ventre chirurgical (n°4), douleur thoracique avec dyspnée (n°5), éruption cutanée bénigne type eczéma (n°6), pyélonéphrite (n°7), suspicion d'AVC (n°8), syndrome dépressif majeur avec critères de dangerosité (n°9), et brûlure étendue du membre supérieur (n°10). Cinq réponses étaient systématiquement proposées : faire le 15, aller aux urgences, consulter son médecin traitant dans la journée, s'auto-médiquer ou ne rien faire. Les questionnaires étaient disposés dans les salles d'attente de 7 cabinets de médecine générale. Le critère de jugement principal est la bonne perception du degré d'urgence médicale, évalué par le pourcentage de bonnes réponses pour l'ensemble des patients à chaque item. Les critères de jugement secondaires étaient la bonne perception ou non du degré d'urgence médicale en fonction du sexe, de l'âge et du type de cabinet (urbain, semi-rural ou rural). 295 patients, d'âge moyen 53 ans, dont 65,8% de femmes, ont été inclus. 4 des 10 items affichent des taux de réponses adéquates supérieurs à 90% ; 4 autres items affichent ce même taux compris entre 80% et 90% ; et les 2 derniers ont un taux compris entre 60 et 80%. Les sous-résultats permettent de dégager des grandes tendances : Les patients ne composent pas assez le 15 pour des pathologies graves telles que la douleur thoracique avec dyspnée et la suspicion d'AVC. Il y a un défaut de recours au système de soins pour des pathologies nécessitant un avis médical dans la journée telles que le TC avec PC, le ventre chirurgical, la pyélonéphrite et le syndrome dépressif avec critères de dangerosité. Ce phénomène est encore plus marqué pour la brûlure étendue de l'avant-bras. Il y a un excès de consultations chez le médecin généraliste pour des pathologies bénignes telles que la gastro-entérite aigüe virale bénigne, la rhino-pharyngite, et l'éruption cutanée bénigne de type eczéma. On n'observe pas de différence dans la perception du degré d'urgence médicale en fonction du sexe des patients ou du type de cabinet dans lequel ils consultent (urbain, semi-rural ou rural), mais on note une différence significative de réponses adaptées pour plusieurs pathologies en fonction de l'âge des patients. La perception du degré d'urgence médicale par les patients adultes est diversement appréciée. Il convient donc à chaque professionnel de santé de continuer à éduquer les patients au quotidien, mais il convient aussi aux pouvoirs publics de développer encore davantage les campagnes sanitaires destinées au grand public. Par ailleurs, d'autres pistes pourraient être explorées, comme le développement de "l'éducation sanitaire" dans les écoles, voire un meilleur balisage du parcours de soins pour mieux organiser le système de santé en France.

Book excerpt: Contexte et objectifs: au lendemain de la publication des dernières recommandations internationales sur la prise en charge de l'arrêt cardiaque (ILCOR 2015) nous avons souhaité évaluer le ressenti des médecins libéraux landais, spécialistes et généralistes, sur la prise en charge de cette urgence vitale. Les objectifs secondaires étaient d'évaluer leur formation aux gestes de réanimation cardio-pulmonaire, leur connaissance sur le defibrillateur et leur désir de formation dans ce domaine. Méthode: Une enquête épidémiologique descriptive transversale a été réalisée via un questionnaire d'auto-évaluation présenté directement au cabinet de 174 médecins libéraux exerçant dans l'est des Landes entre le 1er octobre 2016 et le 27 février 2017. Résultats: 164 questionnaires analysés ont permis de mettre en évidence que 60% des praticiens libéraux reconnaissent n'être pas à l'aise dans la prise en charge de l'arrêt cardiaque. Les facteurs associés à ce manque d'aisance sont la pratique d'une spécialité médicale autre que la médecine générale, l'absence d'expérience de prise en charge d'un arrêt cardiaque, l'absence de formation aux gestes d'urgence et d'utilisation du defibrillateur. Concernant la formation, on constate que 35 % des médecins libéraux n'ont jamais été formés aux gestes des premiers secours. Ils sont encore plus nombreux à n'avoir jamais utilisé de defibrillateur (46%). On retrouve un désir prononcé de formation exprimé par 81% des professionnels de santé. Conclusion: Cette étude a montré que le corps médical libéral n'est pas à l'aise dans la gestion de l'urgence vitale que représente l'arrêt cardio-pulmonaire. Bien que rare à l'echelle individuelle du médecin, chaque praticien est supposé être apte à initier une réanimation cardio-pulmonaire. Promouvoir l'instauration de formation pratiques aux gestes de premiers secours apparaît aujourd'hui comme une véritable necessité pour les professionnels de santé.

Book excerpt: In the late 1950s, like tens of thousands of young men of his generation, Pierre Bourdieu, having recently passed the agrégation in philosophy, found himself immersed in the Algerian war. Motivated by an impulse that, as he himself says, ‘was civic rather than political’, nothing seemed more important to him than to understand the Algerian situation and provide the elements that would enable others to come to an informed judgement about it. In extremely tough conditions and along with a small group of students, Bourdieu undertook a series of studies across an Algeria that was tightly patrolled by the army, leading him to discover the shocking reality of the resettlement camps and to analyse the mechanisms of destruction of Algerian society of which they were emblematic. To achieve the objectives he had set himself, Bourdieu had to carry out a genuine intellectual conversion, acquiring an ethnographic understanding of Algerian society, learning sociological analysis at a breakneck pace and inventing new instruments - both theoretical and empirical - that would enable him to understand the relations of domination specific to colonialism. These new tools also enabled him to analyse the nature of the crisis that the war had both produced and manifested.
