Book excerpt: Crohn's disease and ulcerative colitis, collectively known as inflammatory bowel disease (IBD), are chronic autoimmune disorders of the digestive track most commonly affecting the small and /or large intestine. Little research has been conducted on parental stress in parents of children with IBD; however, researchers suggest this is a pressing concern. In the current study, the Pediatric Inventory for Parents (PIP) was used to assess parenting stress while controlling for general life stress as measured by the Social Readjustment Rating Scale. Parent gender, child disease severity, and child age were anticipated to be key factors associated with increased stress. Regression analyses supported these hypotheses. Specifically, mothers, high disease severity, and young children, we re predictors of parental stress. PIP subscores were also compared to those of other childhood illnesses (diabetes, sickle cell anemia, and cancer) as published in existing research. T-tests were conducted and revealed overall higher stress scores than those of the comparative illnesses. These results provide likely risk factors physicians may recognize and then address early in the child's IBD diagnosis. Results also demonstrate the importance of increased IBD awareness along with publicized parental support options as the current study's stress scores were overall higher than the comparative groups.

Book excerpt: Youth with inflammatory bowel disease (IBD) experience poor health-related quality of life (HRQOL; e.g., Cunningham et al., 2007; De Boer et al., 2005; Greenley et al., 2010). Previous research has demonstrated that factors such as problematic family functioning and increased general anxiety are associated with decreased HRQOL in pediatric IBD (e.g., Guilfoyle et al., 2014; Gumidyala & Greenley, 2014; Herzer et al., 2011a; Kilroy, Nolan, & Sarma, 2011). However, little was known about how specific aspects of the parent-child relationship are related to HRQOL. Furthermore, there was a gap in the literature about the relation of gastrointestinal (GI)-specific anxiety to HRQOL. Therefore, the purpose of the study was to identify more specific factors of the parent-child relationship and GI-specific anxiety that are associated with HRQOL in pediatric IBD. Thirty youth with IBD (Mage = 13.90 years, Range = 10 -- 18; 60% Male) and their primary caregivers or parents were recruited from a pediatric gastroenterology clinic in West Virginia. Parents and youth each completed a series of study questionnaires. Results indicated a significant association between GI-specific anxiety and both youth- and parent-reported total HRQOL scores. There were some significant associations among parent-child relationship dimensions and youth- and parent-report of HRQOL. However, most all of these were counterintuitive and may have been a function of study limitations. Exploratory analyses with specific parent-child relationship dimensions as mediators or moderators of GI-specific anxiety and youth-reported HRQOL were non-significant. The small sample and number of analyses performed each contributed to a likely increase in Type I error. Further, the sample was underpowered to detect medium-to-large effect sizes. However, data suggests that GI-specific anxiety is an important consideration in HRQOL in this population. Future directions include expanding data collection for this study to increase statistical power through a larger sample and potentially greater generalizability of data, as well as investigate targeted interventions to improve GI-specific anxiety in this population.

Previously characterized as diseases primarily affected Caucasians in industrialized countries, there has been an explosion of growth amongst the African American and Hispanic populations in the United States, and well as in pockets of these and other ethnic groups worldwide. The economics of IBD has been changing greatly, given the issues raised above, as well as major changes in the United States Medicare program, and other insurance changes. The growing proportion of the GNP that healthcare costs are consuming in the US and worldwide has shifted attention to decreasing utilization of health care services, and other cost-savings measures. Again, up to this point this has been an exclusive part of our IBD book. New findings in the genetics of IBD have been recently published. Our first edition had the finding of the first Crohn’s disease gene inserted just before going to press; so much has been published about the phenotypes and other genotypes that have emerged. Breakthroughs in new endoscopic evaluations of the bowel (ie. Capsule endoscopy, double-balloon enteroscopy, endoscopic ultrasound) and their use in the inflammatory bowel diseases will be a key highlight of this new book. Major advances in novel radiographic techniques employed in patients with inflammatory bowel disease (ie. CT enterography, MR enterography, advanced ultrasounds, etc.) will also an important component of our work. There have been numerous areas related to the development and detection of colorectal cancer in patients with IBD that have emerged in the past few years. Techniques of better detection (ie. Narrow band imaging; special mucosal staining; endoscopic histological evaluations) as well as research in the putative role of mesalamine and other agents for colorectal cancer prevention are all hot topics that will attract much interest from a variety of interest groups I described above. Identification of additional disease states that probably belong in the IBD family, such as microscopic colitis (including lymphocytic colitis), and collagenous colitis add to the diseases that will be discussed in this new edition.
