Book excerpt: Abstract: The utilization of occupational therapy in pediatrics has increased in the last two decades due to the passing of Public Law 94-142, a law enacted to ensure adequate education for all disabled children and the new positions available in the neonatal units of hospitals (Hopkins & Smith, 1988). Despite the agreement among therapists that functional assessments for infants and young children are valuable, few standardized instruments have been developed for these age groups (Feldman et al, 1990). Recently, to answer the need for an assessment tool to identify the functional abilities in children, Tufts University School of Medicine under a grant from the National Institute of Disability and Rehabilitation Research, has developed the Pediatric Evaluation of Disability Inventory (PEDI) (Haley, Faas, Coster, Webster, & Gans, 1989). According to Keith (1984), "the endless spawning of new and poorly developed functional assessments is wasteful for medical rehabilitation." This is why it is important during the present standardization period of the PEDI that thorough reliability and validity investigations be conducted. Since initial validity studies have shown promising results, present efforts are now focusing on establishing the reliability of the PEDI.

Book excerpt: Abstract: The researchers explored how similar the development of functional behavior of children who have cerebral palsy, specifically spastic hemiplegia, are to children who do not have cerebral palsy. The subjects were recruited form Shriners Hospital, Springfield Unit, using a sample of convenience. The Pediatric Evaluation of Disabilities Inventory (PEDI) (Haley, Faas, Coster, Webster, & Gans, 1989) was administered in a parent report/structured interview format during scheduled hospital visits. The data was then analyzed using a FIT Score statistic. This enabled the researchers to generate data for the developers of the PEDI at the Department of Rehabilitation Medicine, Tufts University School of Medicine in Boston, Massachusetts. Analysis of the data indicated a scattering of functional behaviors, making it difficult to draw any conclusions as to the similarity of functional behaviors between the .sample and the normative group. Although this research can not be considered statistically significant due to the low number of subjects, the researchers believe the discrepancies in scores indicate a need for further research into setting normative scores for different populations of children with disabilities.

Book excerpt: Patient-reported outcomes (PROs) are measures of how patients feel or what they are able to do in the context of their health status; PROs are reports, usually on questionnaires, about a patient's health conditions, health behaviors, or experiences with health care that individuals report directly, without modification of responses by clinicians or others; thus, they directly reflect the voice of the patient. PROs cover domains such as physical health, mental and emotional health, functioning, symptoms and symptom burden, and health behaviors. They are relevant for many activities: helping patients and their clinicians make informed decisions about health care, monitoring the progress of care, setting policies for coverage and reimbursement of health services, improving the quality of health care services, and tracking or reporting on the performance of health care delivery organizations. We address the major methodological issues related to choosing, administering, and using PROs for these purposes, particularly in clinical practice settings. We include a framework for best practices in selecting PROs, focusing on choosing appropriate methods and modes for administering PRO measures to accommodate patients with diverse linguistic, cultural, educational, and functional skills, understanding measures developed through both classic and modern test theory, and addressing complex issues relating to scoring and analyzing PRO data.

Book excerpt: Abstract: One basic goal for any pediatric occupational therapist is to provide each child who receives services with the most effective treatment program possible. The treatment process should begin with the therapist performing an evaluation of the client in which two fundamental aspects of the child's life are examined: the functional skills that the child is able to utilize in performing activities of daily living, and the child's level of independence, or the amount of assistance required by the child in order to perform these tasks. Unfortunately there are few standardized instruments which can be used by pediatric occupational therapists for evaluation purposes (Campbell, 1989). Additionally, there is little clinical data that can provide therapists with a consistent definition of those components which truly constitute "independence", and it is not known whether the clinical definition of this concept should vary from population to population. In other words, the question should be raised as to whether or not those components which describe "independence" for disabled and non-disabled children include the same, similar, or different concerns. The Pediatric Evaluation of Disability Inventory (PEDI) is the first functional assessment designed specifically for the purpose of measuring a child's functional skill level combined with an assessment of that child's level of independence. This test records children's functional performance in a variety of daily living skills and social behaviors. It is the opinion of the investigators that a relationship exists between functional ability, as measured by the Functional Skills/Behaviors section of the PEDI (Part One), and level of independence, as measured by the Caregiver Assistance/Modifications section of the PEDI (Part Two). This investigation will analyze the nature of this relationship. It is our belief that if the PEDI can describe the relationship between functional skill and independence accurately, this concept can be used to better evaluate and assess the needs of the clients who receive pediatric occupational therapy services.
