Book excerpt: Increasing public investments in health care services for low-income and special needs children and adolescents in the United States have raised questions about whether these efforts improve their health outcomes. Yet it is difficult to assess the general health status and health care quality for younger populations, especially those at risk of poor health outcomes, because the United States has no national information system that can provide timely, comprehensive, and reliable indicators in these areas for children and adolescents. Without such a system in place, it is difficult to know whether and how selected health care initiatives and programs contribute to children's health status. Child and Adolescent Health and Health Care Quality identifies key advances in the development of pediatric health and health care quality measures, examines the capacity of existing federal data sets to support these measures, and considers related research activities focused on the development of new measures to address current gaps. This book posits the need for a comprehensive strategy to make better use of existing data, to integrate different data sources, and to develop new data sources and collection methods for unique populations. Child and Adolescent Health and Health Care Quality looks closely at three areas: the nature, scope, and quality of existing data sources; gaps in measurement areas; and methodological areas that deserve attention. They are: health status, barriers to access, workforce, organizational response, and the role of the American Academy of Pediatrics (AAP). Recommendations are included for each of these components. Health status indicators for minority children include: premature death and disability caused by controllable illnesses and high infant mortality, differential rates of immunization, teenage pregnancy, and injuries associated with violence. Minority children also encounter barriers to health care access. Among these barriers are: economic factors, such as a lack of financial resources and inadequate insurance, and geographic factors that result in limited availability of providers and facilities. Other barriers are: poverty and lack of education, which often translate into delayed health care and poor compliance with treatment, cultural insensitivity, racism, and classism. Workforce factors that affect minority children's health care access include the relative absence of minority group pediatricians and the geographic maldistribution of practitioners. The AAP Task Force recognizes the organizational response of other groups that have addressed this issue. The Academy's role is to ensure that the perspectives of minority pediatricians and children are considered in AAP educational programs, policy, developmental advocacy and research. Book excerpt: Since 1965 the foreign-born population of the United States has swelled from 9.6 million or 5 percent of the population to 45 million or 14 percent in 2015. Today, about one-quarter of the U.S. population consists of immigrants or the children of immigrants. Given the sizable representation of immigrants in the U.S. population, their health is a major influence on the health of the population as a whole. On average, immigrants are healthier than native-born Americans. Yet, immigrants also are subject to the systematic marginalization and discrimination that often lead to the creation of health disparities. To explore the link between immigration and health disparities, the Roundtable on the Promotion of Health Equity held a workshop in Oakland, California, on November 28, 2017. This summary of that workshop highlights the presentations and discussions of the workshop.
