Book excerpt: Where do you begin to look for a recent, authoritative article on the diagnosis or management of a particular malignancy? The few general oncology textbooks are generally out of date. Single papers in specialized journals are informative but seldom comprehensive; these are more often preliminary reports on a very limited number of patients. Certain general journals frequently publish good in depth reviews of cancer topics, and published symposium lectures are often the best overviews available. Unfortunately, these reviews and supplements appear sporadically, and the reader can never be sure when a topic of special interest will be covered. Cancer Treatment and Research is a series of authoritative volumes which aim to meet this need. It is an attempt to establish a critical mass of oncology literature covering virtually all oncology topics, revised frequently to keep the coverage up to date, easily available on a single library shelf or by a single personal subscription. We have approached the problem in the following fashion. First, by dividing the oncology literature into specific subdivisions such as lung cancer, geni tourinary cancer, pediatric oncology, etc. Second, by asking eminent authorities in each of these areas to edit a volume on the specific topic on an annual or biannual basis. Each topic and tumor type is covered in a volume appearing frequently and predictably, discussing current diagnosis, staging, markers, all forms of treatment modalities, basic biology, and more.

The original vol umes were the result of an initial call for papers dedicated to progress in the field of surgical pathology with contributors from all over the world. The pa pers published in these volumes have represented examples of classical clin ical pathologic correlations within the discipline of surgical pathology; other papers reflect the work being done at the interface between classical diagnos tic surgical pathology and research in the realm of immunology, molecular biology, cell biology, etc. These papers illustrate what is possible utilizing all of the advances made in basic biology, while remembering that the patholo gist remains an essential, crucial figure in the analysis of tissues, both with respect to their diagnosis as weil as the analysis of the dynamic interactions between cells. There have also been papers that may be characterized as philosophical or historical, which Iook at aspects of surgical pathology in a unique way. Five volumes have been published since 1980. The last of these was pub lished in 1983. For those of you who have been our loyal readers, you may wonder why there has been a gap in the publication of these volumes. This has been due to reorganization both among ourselves as weil as with the pub lisher.

Book excerpt: The development of monoclonal antibodies to human tumor associated antigens has greatly facilitated the application of immunohistochemical techniques to analyze surgically removed tissues. During the last few years this approach has been utilized by a progressively increasing number of investigators to analyze malignant cells. Although monoclonal antibodies to tumor associated antigens have not become yet routine reagents in immunopathology, they have provided new information which could not be obtained with conventional antisera or histochemical procedures. The following are representative examples. TUmor associated antigens have been identified which display a restricted distribution in normal tissues and therefore may represent useful markers for radio imaging and appropriate targets for immunotherapy. In spite of undetec table differences with conventional histopathological approaches hetero geneity has been found in the antigenic profile of tumor cells within a lesion, in autologous lesions removed from different anatomic sites from a given patient and in lesions removed from different patients. Phenotypes of tumor cells have been identified which correlate with the biology of tumor cells and with the clinical course of the disease. From a practical view point the use of monoclonal antibodies in immunopathology has enhanced interactions between pathologists and immunologists, as exemplified by the present book. Such interactions have contributed to the application of basic research to clinical problems. The chapter of this book discuss investigations performed with monoclonal antibodies to antigens expressed by various types of normal and malignant human cells.

Pregnant women who have even a single drink routinely face openly voiced reproach. Yet fetal alcohol syndrome (FAS) in infants and children is notoriously difficult to diagnose, and the relationship between alcohol and adverse birth outcomes is riddled with puzzles and paradoxes. Sociologist Elizabeth M. Armstrong uses fetal alcohol syndrome and the problem of drinking during pregnancy to examine the assumed relationship between somatic and social disorder, the ways in which social problems are individualized, and the intertwining of health and morality that characterizes American society. She traces the evolution of medical knowledge about the effects of alcohol on fetal development, from nineteenth-century debates about drinking and heredity to the modern diagnosis of FAS and its kindred syndromes. She argues that issues of race, class, and gender have influenced medical findings about alcohol and reproduction and that these findings have always reflected broader social and moral preoccupations and, in particular, concerns about women's roles and place in society, as well as the fitness of future generations. Medical beliefs about drinking during pregnancy have often ignored the poverty, chaos, and insufficiency of some women's lives—factors that may be more responsible than alcohol for adverse outcomes in babies and children. Using primary sources and interviews to explore relationships between doctors and patients and women and their unborn children, Armstrong offers a provocative and detailed analysis of how drinking during pregnancy came to be considered a pervasive social problem, despite the uncertainties surrounding the epidemiology and etiology of fetal alcohol syndrome.
