Book Pain Control in Ambulatory Surgery Centers

Download or read book Pain Control in Ambulatory Surgery Centers written by Kanishka Rajput and published by Springer Nature. This book was released on 2021-03-08 with total page 429 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book provides a comprehensive review of the challenges, risk stratification, approaches and techniques needed to improve pain control in ambulatory surgery centers (ASCs). It addresses not only the management of acute perioperative pain but also describes modalities that could potentially reduce the risk of evolution of acute pain into chronic pain, in addition to weaning protocols and follow ups with primary surgical specialties and pain physicians as needed. Organized into five sections, the book begins with the foundations of managing ASCs, with specific attention paid to the current opioid epidemic and U.S. policies relating to prescribing opioids to patients. Section two and three then explore facets of multimodal analgesia and non-operating room locations, including the use of ultrasounds, sedation in specific procedures, regional anesthesia, ketamine infusions, and the management of perioperative nausea and intractable pain in outpatient surgery. Section four examines the unique challenges physicians face with certain patient demographics, such as the pediatric population, those suffering from sleep apnea, and those with a history of substance abuse. The book closes with information on discharge considerations, ambulatory surgery protocols, recovery room protocols, and mandatory pain management services. An invaluable reference for all health personnel and allied specialties, Pain Control in Ambulatory Surgery Centers (ASCs) meets the unmet need for a resource that covers optimum pain control in patients undergoing outpatient surgery as well as the urgent ASCs challenges that are presented on an immense scale with national and international impact.

Book Pain Management and the Opioid Epidemic

Download or read book Pain Management and the Opioid Epidemic written by National Academies of Sciences, Engineering, and Medicine and published by National Academies Press. This book was released on 2017-09-28 with total page 483 pages. Available in PDF, EPUB and Kindle. Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Book Perioperative Pain Control Tools for Surgeons

Download or read book Perioperative Pain Control Tools for Surgeons written by Peter F. Svider and published by Springer Nature. This book was released on 2021-01-21 with total page 352 pages. Available in PDF, EPUB and Kindle. Book excerpt: According to the Centers for Disease Control and Prevention (CDC), there were nearly 40,000 opioid-related deaths in the U.S in the last year alone. These numbers have increased considerably for a variety of reasons, and the growing role of prescription drug abuse has been widely recognized. There has also been increasing consciousness of the lack of dedicated and formalized training dedicated to prescription of analgesics, particularly among surgical trainees and practicing surgeons. Recent studies demonstrate that only a small minority of training programs mandates any opioid prescribing education, despite the fact that nearly all programs allow trainee prescription of opioids on an outpatient basis. Therefore, it is not surprising that despite a growing body of literature illustrating the value of opioid alternatives and adjuncts, practicing surgeons in all specialties largely ignore these evidence-based practices and continue to prescribe opioids in situations for which alternatives are proven, safe, and effective. There are many reasons for preparing a concise and practical resource describing evidence-based practices targeting surgeons and surgical subspecialists, including but not limited to: (1) The exponential increase in prescription drug abuse associated with the “opioid epidemic,” as surgeons play an important role in managing perioperative and chronic pain in a variety of practice setting; this issue bears importance from the patient, physician, and policymaker standpoint; (2) With increasing recognition of the role of quality improvement in patient safety initiatives, and literature demonstrating inadequate opioid prescribing education (OPE) in surgical training programs, it is only a matter of time before accreditation bodies including the ACGME incorporate strong recommendations and mandates for OPE; (3) Myriad literature demonstrating the existence of evidence-based alternatives to opioids – All surgeons and surgical subspecialists would benefit from a practical guide concisely laying this literature out. We feel that surgeons of all experience levels may benefit, ranging from the first year surgical intern to an experienced practitioner trying to remain compliant with the evolving opioid regulatory environment. Hence, the appropriate target audience for this text would be for the tens of thousands of current surgical trainees in many specialties, including general surgery, thoracic surgery, otolaryngology, plastic surgery, urology, gynecology, vascular surgery, neurosurgery, and orthopedic surgery. Furthermore, surgeons out of training would be another audience of interest, as there are increasing courses for continuing medical education based on perioperative and chronic pain management. Currently, there are no comparable and competitive guides with these target audiences; all of the existing literature about perioperative and chronic pain is targeted either specifically for patients, anesthesiologists, or pain medicine physicians. This will be the only up to date guide focusing on evidence-based practices for perioperative pain control, and each section will also include information on chronic pain sequelae relevant to each surgical specialty. The editors envision this text being an interdisciplinary endeavor, incorporating surgeons from multiple specialties, anesthesiologists, pain medicine physicians, and palliative physicians as appropriate.

Book Pain Management Education Quality Improvement Project

Download or read book Pain Management Education Quality Improvement Project written by Adriana Torres and published by . This book was released on 2020 with total page pages. Available in PDF, EPUB and Kindle. Book excerpt: Section I: Abstract Problem: Thirty percent of patients who have ambulatory surgery describe their pain as moderate to severe on postoperative phone calls in this Northern California Hospital. Patients have expressed dissatisfaction on the topic of subsequent pain on the Outpatient and Ambulatory Surgery-Consumer Assessment of Healthcare Providers and Systems surveys. Patients receive pain management education after recovery via oral and written format, but the content is not standardized. The aim of this project is to decrease the pain experienced by patients from 30% to less than 25% with a secondary goal to increase patient satisfaction with pain education from 10th % to over 20% on OAS-CAHPS. Context: The recovery room has over fifty patient care bays. An average of 25 outpatient surgeries are performed a day. The vast majority of surgeries performed are same day discharge with an average recovery time of ninety minutes. Staff members deliver discharge instructions written by the surgeon which includes pain management. There is variability in content and delivery of pain management education. Unit stakeholders support an educational quality improvement project to address the patient's needs for better pain control and satisfaction with their instructions. Interventions: The American College of Surgeons Safe and Effective Pain Control After Surgery brochure was used to manage the content and delivery of educational material patients received to assess their pain and treat it at home. The brochure meets the Joint Commission requirements of 2018 related to pain education upon discharge (Joint Commission,2017). Inclusion criteria for the intervention group were adult English-speaking patients having scheduled elective surgery with exclusion of eye, ear/nose/throat surgery, add-on cases, and dementia. Staff engagement and buy-in was established with huddles sharing OAS-CAHPS scores and patient postoperative call verbatims. The brochure was presented in the preoperative period for patient to read, and it was reviewed in greater detail in the recovery phase before discharge. A pain score education flyer was added to help patients describe their pain to the nurse on the postoperative call. Measures: Process measures included, increasing the amount of postoperative calls performed, auditing of nursing staff educating with the brochure, and auditing of documentation in electronic medical record. The unit informaticist pulled data from the electronic medical record to support audit accuracy. The two outcome measures included pain statements from the 24- hour postoperative phone calls and the OAS-CAHPS score on the question of subsequent pain delivered by the patient care experience coordinator. Results: The outcome measure results as of June 11,2020 show that although the quantity of patients that complained of moderate to severe pain went down from 30% to 27%. The change was not statistically significant. At the time of this analysis, the OAS-CAHPS score regarding subsequent pain education showed improvement from 10% to 49% for March. Preliminary results for April sit at 46% and May at 99%. Conclusion: There were extenuating circumstances that were in effect at the roll-out of this project. Covid-19 became a global pandemic. The volume of cases went down significantly from the average 25 cases a day to less than 10 a day. Workflows and priorities changed within the unit with a focus on infection control and safety. Based on the results of this project, the pain management educational brochure will be part of every patient's discharge instruction packet pending funding approval. Studies have found that the more education the patient receives about their pain management at home the better they do. They have less complications, report less postoperative pain, and have a better surgical care experience (Sawhney,Wilson,&McGillion,2017).

Book Ambulatory Multidisciplinary Management of Postoperative Pain

Download or read book Ambulatory Multidisciplinary Management of Postoperative Pain written by Jaime Ruiz-Tovar and published by . This book was released on 2022-03-16 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Postoperative pain is a common problem in clinical practice that becomes chronic in approximately 10% to 30% of adults who undergo surgery, being severe in 5 to 10% of them. It is one of the main causes of chronic pain, which makes it a major public health problem. It generates in patients who suffer it a worsening of their quality of life, their psychological well-being, and disability with the consequent incapacity for work. Given that both its etiology and its consequences are complex and multifactorial, postoperative pain must be approached from a multimodal perspective: preventive, pharmacological, nutritional, rehabilitative, nursing intervention, psychological and, in those refractory cases, through surgical approaches or nerve block techniques. This book aims at first to analyze the pathophysiological and psychological bases that underlie its etiology. Next, it aims to carry out an updated review of the multidisciplinary programs for the treatment of postoperative pain that have been carried out and what their results have been. Moreover, the contribution of each of these disciplines in the ambulatory management of postoperative pain will be reviewed. Finally, we want to study the peculiarities of the management of this type of pain in the oncological patient, as well as the personal and socioeconomic consequences and the increase in other comorbidities of this important reason for consultation.

Book Capstone Project

Download or read book Capstone Project written by Sara B. Maich (RN) and published by . This book was released on 2015 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: When a patient reaches the post-anesthesia care unit (PACU), to the surgical team, their journey is over, but to the patient, it has only begun. Although the procedure is behind them, the pain that greets them in consciousness is what many patients fear more than any other part of the surgical process. Currently, patients in the Ximed Ambulatory Surgery Center (ASC) are coming in for increasingly complex procedures. Concurrently, many of these patients have comorbidities that make pain management a complex issue. Pain control in the PACU is the first step towards the patient's recovery. The use of narcotics can increase length of stay, and easily leads to side effects that can keep the patient in the unit longer than expected. Pain management is a complex element of any patient's recovery. Managing pain in the post-operative setting can be difficult because pain is subjective. At the same time, it is arguably one of the most important factors of discharge criteria. With the increasing concerns of meeting benchmarks and effective use of staff, time is of the essence for nurses in PACU. The use of opioid pain medication in PACU comes with a host of potential problems including adverse drug effects, delayed transfer, and delayed ambulation. The use of non-narcotic medication greatly lowers the risk of adverse drug effects and has little to no impact on ambulation after surgery as it relates to fall risk. It is proposed that by giving all patients in the ASC one gram of IV Acetaminophen in the final stages of surgery (id est wound closure and dressing), the peak onset of the drug will occur before the patient reaches the PACU. The peak onset is one hour after administration and lasts four to six hours. It is anticipated that this addition to current pain control therapies will decrease the need for rescue narcotics, decrease adverse narcotic effects, and decrease length of stay related to pain control as shown to be the case in numerous studies. To evaluate the effectiveness of the proposed change there are several tools that can be used. By having a baseline survey the implementation team will be able to determine if the change has been quickly effective or requires more work to show the greatest patient benefit. As an essential communication tool, it is important to survey clinicians and patients. Staff Education assessment tools will be included in the facilities established Learning Management System (LMS) module with a post test and survey. This will provide a baseline understanding of the material presented and give participants an opportunity to respond with questions and comments that will be addressed before the implementation date. Constant communication through all stages between clinicians and management are essential for a successful launch of the new protocol. The cost of the IV Acetaminophen is offset by a decrease in narcotic use and associated adverse drug events, and anticipated decrease in length of stay in the PACU, thereby decreasing the number of nursing hours. Disseminating results of any implemented project is important to many groups of people, and it is important to distribute it in a way that all of these people can access the information.

Book Improving Acute Postoperative Pain Management Using an Educational Intervention

Download or read book Improving Acute Postoperative Pain Management Using an Educational Intervention written by Andrena Patrice Wright and published by . This book was released on 2017 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: "Over the last three decades, the volume of ambulatory and day surgery has more than tripled to nearly 54 million procedures annually (Hollenbeck et al., 2014). Rapid turnover of ambulatory and day surgery patients requires the proper management of their pain level. Acute postoperative pain management is one of the most important roles and functions of the perioperative nurse. Healthcare providers that integrate evidence-based guidelines into practice make informed decisions that are more concise and predictable. Although clinical guidelines on acute postoperative pain management have been published by the American Society of PeriAnesthesia Nurses, the Association of Perioperative Registered Nurses, the American Pain Society and the American Society of Anesthesiologist Task Force on Acute Pain Management, patients continue to experience inadequate pain control due to inconsistent pain management practices. Educational interventions can increase clinical knowledge and decrease negative patient outcomes post-surgically. The purpose of this EBP change project was to reduce acute postoperative patient pain through the implementation of an educational intervention aimed to educate the staff on evidence-based standardized pain management guidelines in an Ambulatory Surgery Center and Post Anesthesia Care Unit. The benchmark of 80% post intervention and 70% post implementation was achieved. There was an overall 22% reduction rate in pain scores from 34.91% in August 2017 to 27.36% in October 2017. Although the rate in October 2017 did not meet the 22% internal benchmark, there was a decrease in the rate of pain. Key words: acute postoperative pain management, pain management guidelines, educational intervention. " -- Abstract

Book Perioperative Pain Management

Download or read book Perioperative Pain Management written by Richard D. Urman and published by Oxford University Press. This book was released on 2013-04-08 with total page 144 pages. Available in PDF, EPUB and Kindle. Book excerpt: Millions of procedures, surgical and non-surgical, are performed around the world each year. Management of perioperative pain is of great importance to patients, and a critical management issue for physicians and other health professionals who provide perioperative care. Healthcare facilities and national accrediting organizations have established standards surrounding proper management of perioperative pain. There is now an increased burden on every hospital and training program to ensure that healthcare providers understand the essentials of pain management and are able to recognize and treat pain in a timely fashion. Poorly controlled pain leads to patient dissatisfaction and contributes to increased morbidity and mortality, such as myocardial infarction, pneumonia, and emotional effects. Proper management, including appropriate pharmacologic management and regional analgesic techniques, can improve function and shorten length of hospital stay. Patients who are undergoing procedures under sedation or anesthesia need to receive adequate pain relief with drugs or regional anesthetic techniques. In the recovery room following the procedure, the staff taking care of the patient needs to be properly trained to diagnose and treat post-procedural pain. Part of the Oxford American Pain Library, this concise, evidence-based clinical guide serves as a tool for every clinician who wishes to understand the basic mechanisms, pharmacology, invasive and noninvasive treatment modalities, guidelines and development of pain management protocols. The authors address new technologies, chronic pain issues, running an acute pain service, opioid and non-opioid pharmacology (including newly approved drugs), epidural and other regional anesthesia, and special populations such as pediatric patients, the elderly, and patients with a co-existing disease.
