The purpose of this symposium on "Fertiliza tion of the Human Egg in Vitro" was to bring together the stilllimited number of elinical specialists in the field and to stimulate another group of basic research people to exchange their experiences and knowledge, hopefully promoting elose cooperation between the two groups. It was a kind of scientific "first" that all research teams so far successful in achieving the birth of a healthy baby, fertilized in vitro came together at a workshop conference without a large audience of spectators and reporters, but with a small number of highly critical colleagues from the fields of basic reproductive physiology and comparative developmental biology. This atmosphere allowed for the elose exchange of results, hypotheses, diagnostic and therapeutic procedures, criticism, and respect, and created very productive discussions, all of which furthered the aim of the method: To help more childless couples to have their own babies by the ultima ratio procedure of in vitro fertilization and embryo replacement. The book that has emerged from this symposium will help to disseminate the great amount of information and experience gathered among the scientifically and clinically interested colleagues of many other hospitals and universities who could not be invited to the meeting. At the same time, it will prove that there is much more work to be done in the basic and clinical sciences of human embryology and reproductive biology.

The complex signals from the embryo to the ovary, endometrium and myometrium are now being revealed through studies in both primates and other mammalian species. This book addresses the interrelationship of pituitary and ovarian hormones in controlling ovulation and the preparation of the intrauterine environment for implantation. Once fertilisation has occurred and trophoblast has formed, the next vital step is the production of materials which signal the presence of the pregnancy to the rest of the body. Trophoblastic proteins and other early-pregnancy factors are prime candidates for this role. Recent studies have emphasised the importance of the intrauterine environment in implantation. Specific secretory products of the endometrium have great potential in this process. The prostaglandins also play an essential part. Immunological adjustments are now considered a condition for the successful establishment of pregnancy. The possible use of immuno therapy in the treatment of recurrent abortion has highlighted interest in this area. The use of immunological techniques for contraception are in their infancy but offer much hope for the future.

The section on diseases of the vagina has been enhanced by a greater emphasis on the clinical aspects. The chapter on DES induced lesions has been updated with the data of the DeSAD study and the section on adenocar cinoma of the cervix has been enlarged by the description of the undifferenti ated lesions including "glassy cell and signet cell" carcinoma. A departure from the traditional chapter approach has been made in the discussion of endometrial hyperplasia and carcinoma in order to present a conceptual view of these diseases. A similar presentation of diseases of the myometrium has been made. The subjects of lymphomas and mesenchymal tumors of the ovary have been enlarged upon and presented in a separate chapter. Fine needle aspiration in the diagnosis of ovarian tumors and of non malignant disorders of the ovary has become an increasingly useful technique. Two new chapters have been added to cover this subject. Many of the changes made in the second edition were in response to reviewers of the First Edition.
