Book excerpt: "The GAO first issued a report on this subject in September, 1975. That report indicated that large quantities of drugs could not be accounted for in VA hospitals, especially in those hospitals using the conventional ward stock system. The report recommended that the unit dose system be adopted to provide for better control of these dangerous drugs. At the time of the 1975 report, only seven VA hospitals had installed the unit dose system despite VA's endorsement of it. The bottom line of the report was that the VA should take immediate action to strengthen its control of drugs throughout the system. Subsequent to this report, this subcommittee conducted an oversight hearing at the Brecksville Division of the Cleveland VA Medical Center on April 17, 1979. Witnesses at that hearing made several references to the easy availability of illegal drugs within both divisions of this medical center. The Wade Park Division was the target of much of the criticism of the witnesses. The loss of these drugs can be attributed to at least three things. They are: first, pilferage of drugs on the wards; second, theft of prescription pads and forging of a physician's signature; and third, sale of illegal drugs by drug pushers who enter the hospital solely for the purpose of peddling illegal substances. Both divisions were criticized for the lack of security to prevent these outsiders from pushing drugs within the hospitals"--Page 1.
