Les conséquences des troubles du langage sur la vie quotidienne des enfants puis des adultes, sur l'insertion scolaire puis professionnelle peuvent être prévenues ou atténuées par des prises en charge précoces adaptées. Pour cela, un outil d'aide au repérage validé et adapté aux soins premiers est nécessaire. Objectif : Lister les outils de repérage des troubles du langage de l'enfant, utilisables en consultation de médecine générale non programmée. Evaluer leur validité et faisabilité. Méthode : Revue systématique de la littérature réalisée à partir des bases de données Pubmed, Lilacs, Psyclnfo, SUDOC et The Cochrane Library. Seuls les articles évaluant la validité ou la faisabilité d'un outil de repérage en soins premiers ont été inclus dans l'analyse. Résultats : 2941 articles ont été sélectionnés, 29 études ont été incluses dans l'analyse. 30 tests ont été référencés : 12 tests directs et 18 questionnaires parentaux. L'ERTL4 et les IDFC étaient les 2 seuls outils en langue française. Ils permettaient le repérage des troubles du langage à 4 ans ou à 12-18-24 mois. Leur temps de passation était inférieur à 10 minutes mais ils étaient globalement méconnus des médecins généralistes interrogés. La qualité et la comparabilité des études étaient limitées par les différences de critères de validité, par l'hétérogénéité des populations, des recrutements et des langues évaluées. Aucun test ne permettait le repérage des troubles du langage écrit. Conclusion : Parmi les tests directs et questionnaires parentaux disponibles, I'ERTL4 et les IDFC version courte apparaissent adaptés aux soins primaires où une évaluation rapide du langage de l'enfant est nécessaire. Bien qu'il y ait encore certains freins à une utilisation aisée lors d'une consultation non programmée, il semble maintenant nécessaire d'évaluer leur acceptabilité sur de larges échantillons de médecins généralistes.
