Available in PDF, EPUB and Kindle. Book excerpt: Background: Oligodontia (congenial absence of six or more teeth) affects functional, emotional and social aspects of an individual's life. Few published studies have evaluated oral health-related quality of life (OHRQoL) in children with oligodontia. Methods: Thirty-five 8-to-18-year-old patients with oligodontia recruited from The Hospital for Sick Children and Holland Bloorview Kids Rehabilitation Hospital, and their parents, completed the short format of Child Perception Questionnaire (CPQ11-14) and Parent Child Perception Questionnaire (P-CPQ), respectively. Results: Children reported significantly higher overall CPQ score than their parents. Correlations between children's and parents' overall CPQ score, oral symptoms and functional limitations, and social well-being were not statistically significant. However, as children's emotional well-being score increased, parents' score also increased. There was no association between child CPQ score and age, gender, number and location of permanent tooth agenesis in this sample. Conclusion: Children with oligodontia had poorer OHRQoL compared to what was perceived by their parents.

Abstract: Abstract of thesis entitled CHANGES IN CHILDREN'S ORAL HEALTH RELATED QUALITY OF LIFE FOLLOWING ORTHODONTIC TREATMENT Submitted by ZHANG MAN For the degree of Doctor of Philosophy at The University of Hong Kong in September 2007 Malocclusion is a common oral disorder and the demand for orthodontic care is increasing among children. There is a dearth of information as to how malocclusion and its treatment impacts on the life quality of children, which has obvious implications in assessing orthodontic treatment need and evaluating outcomes from orthodontic care. This prospective longitudinal study was conducted to identify associations between orthodontic treatment need and child oral health-related quality of life (COHQoL); to determine the level of agreement between parents' and children's perceptions of COHQoL; to determine changes in COHQoL as a result of undergoing fixed orthodontic appliance therapy and to determine changes in COHQoL following orthodontic treatment. Children (aged 11-14) seeking orthodontic treatment were recruited in this study and followed-up over a three year period. Children's parents were also recruited at baseline. Children self-completed the Child Perceptions Questionnaire (CPQ) at baseline (pre-treatment status and expectations of treatment), during orthodontic treatment (at one week, one month, three months, six months, twelve months, and eighteen months after insertion of fixed appliance), and 6-months post-treatment. Their parents completed the Parental Perception Questionnaire (PPQ) and Family Impact scale (FIS) prior to treatment. Children's pretreatment occlusal models were assessed by three commonly used occlusal indices: Index of Orthodontic Treatment Need (IOTN) including Aesthetic Components (AC) and Dental Health Components (DHC), Index of Complexity, Outcome and Need (ICON) and the Dental Aesthetic Index (DAI). Improvements in occlusion following orthodontic treatment was determined by assessing differences in ICON scores obtained from pre- and post- treatment orthodontic casts. CPQ scores and AC scores were significant correlated (r=0.27, p0.05). Various health conditions, personal, social, and environmental factors, are all thought to contribute to individual's quality of life. However, the impact of orthodontic treatment on Oral Health-related Quality of Life (OH-QOL) outcomes in children has not yet been systematically studied. Hence, this research was planned to assess the effect orthodontic treatment has on pediatric OH-QOL outcomes. Further, the important moderational role of children's psychological assets on OH-QOL reports is explored.Following completion of a preliminary study to confirm the psychometric properties of the Child Perception Questionnaire (CPQ11-14), the current two-phase study was undertaken. This consisted of a cross-sectional study examining the relationship among Self-Esteem (SE), malocclusion, and OH-QOL, and a longitudinal study examining the influence of orthodontics and children's Psychological Wellbeing (PWB) on OH-QOL reports.This PhD dissertation is presented in the "Publishable Style". The journals which hold the copyrights for the papers published from this thesis have given permission for the reproduction of the text and figures for this dissertation.The preliminary data confirmed that the CPQ11-14 is sensitive to change when used with children receiving orthodontic treatment. Our cross-sectional findings indicated that the impact of malocclusion on OH-QOL is substantial in children with low SE and identified SE as a salient determinant of OH-QOL in children seeking orthodontic treatment. Longitudinal data, on the other hand, detected significant improvement of OH-QOL outcomes after orthodontic treatment. As postulated, these improvements were most evident for the social and emotional domains of OH-QOL. However, covariate analysis emphasized the important role psychological factors play in moderating OH-QOL reports, as children with better PWB were more likely to report better OH-QOL regardless of their orthodontic treatment status.These results substantiate the validity of contemporary models of patient-based outcomes linking biological, personal, social, and environmental factors. Researchers and clinicians are encouraged to adopt this forward thinking approach when dealing with children with oro-facial conditions. Further studies with larger samples and longer follow-ups would be of value to expand on these findings.

Book excerpt: The purpose of this study was to assess the baseline oral health quality of life of high-risk children and examine differences in oral health quality of life according to the child's treatment needs at an oral screening exam. We hypothesized that a child's oral health related Quality of Life (QOL) would be correlated with dental treatment needs. This study was a cross-sectional design with subjects who were 8-14 year old patients of Virginia Commonwealth University and/or members of the Boys and Girls Club of Metro Richmond. The oral health quality of life was measured using the Child Perception Questionnaire (CPQ). It is a multidimensional scale that includes items concerning functional aspects including oral pain and discomfort, but also includes psychological and social aspects. The four domains tested are: oral symptoms, functional limitations, emotional well-being and social well-being. The children were asked to report on these domains as they related to their oral health in the past 3 months. The dental treatment needs of the children were categorized as (0) no future needs, or (1) restorative or orthodontic needs. Those with no future needs were scheduled for a 6 months periodic exam and prophylaxis (cleaning). The children in need of restorative treatment were scheduled for such (restorations, stainless steel crowns, extractions), and those in need of orthodontics were referred for consult to an orthodontist. The final sample population consisted of 107 children, predominately African American (84%). The intra-oral screenings revealed 75% of children required future dental treatment, specifically 84% restorative and 12% orthodontic work and 25% in need of preventative recall (cleaning and exam at 6 months). Overall, children reported high levels of oral health quality of life. When asked about the overall health of their teeth, lips, jaws and mouth, 39 % (n=42) reported excellent or very good, 50% (n=53) reported good, and 9% (n=10) reported fair or poor. When asked how "overall life" was affected by oral health, 72% (n=77) reported very little or not at all, 13% (n=14) reported some, and 15% (n=16) reported a lot or very much. There were no significant correlations between the oral health quality of life domains and the children's dental treatment needs.
