However, this issue extends beyond the realm of the individual clinician's office. This text presents quality-of-life research from various fields, including psychology, public health, and general health care; discusses how a patient-centered approach can be applied to basic oral and craniofacial research, clinical dental practice, community dental health issues, and dental education; and addresses how oral health-related quality of life relates to treating and understanding different patient populations, such as children with special needs, medically compromised patients, patients with oral cancer, and patients with chronic facial pain. Also discussed is how factors such as race/ethnicity, gender, and age can affect oral health-related quality-of-life concerns and treatment strategies. Finally, the book offers an outlook on the role that oral health-related quality of life will play in future research and dental education.

Book excerpt: "Purpose: This study sought to bring awareness to the scientific community of the need for regular dental care in skilled nursing facilities (SNF) by proving access to oral care has an effect on oral health and may have an impact on the SNF residents' daily performance and Quality of Life (QoL). Methods: This study was conducted at a private SNF in a southern metropolitan area with a sample size of 30 (N=30). The PI and two research assistants conducted interviews and oral exams using a modified 10 item Oral Impacts on Daily Performance (OIDP) with two free response (QoL) questions, OHI-S, and DMFT indices to collect data. Results: Spearman's rank coefficient analyzed correlations between OHI-S, DMFT, and OIDP indices and found a significant relationship between the OHI-S and OIDP (p=0.036) implying oral disease from plaque and calculus buildup in the mouth impact daily performance. Furthermore the strongly suggested relationship between the DMFT and OIDP (p=0.054) alludes to the fact the number and condition of the teeth has an impact on how these subjects lived their daily lives. Considering the strongly suggested relationship between the Missing (M) in DMFT scores and the OIDP (p= 0.059), the number of teeth has more potential to impact daily performance than whether the teeth are decayed or filled. Qualitative data analysis found four themes related to the social, psychological, physical, and financial well-being of the subjects and their oral health. Conclusion: Oral health has the potential to impact daily performance resulting in an effect on quality of life. The health care professions are charged with developing health care provider models and collaborative interprofessional health care teams to meet the future needs of aging adults"--P. v.

Book excerpt: " Despite concerns regarding the oral health of rural Canadians, estimates of oral health-related quality of life do not exist in published provincial data. The objective of this population-based cross-sectional study was to estimate rural-urban differences in oral health-related quality of life (OHRQoL). Using Andersen's behavioral model for health services utilization as the conceptual framework, data from 1,788 parents/caregivers of schoolchildren from 8 regions of Quebec, Canada were collected through the employment of a two-stage sampling technique. Place of residency was defined according to Statistics Canada's Census Metropolitan Area and Census Agglomeration Influenced Zone classification. The outcome of interest was OHRQoL, measured using a validated Oral Health Impact Profile-14 (OHIP-14) questionnaire. The prevalence, extent and severity of negative oral health impacts were calculated after applying data heightening. Descriptive statistics, bivariate analyses and binary logistic regressions were performed using SPSS version 22. Our results showed that residents of rural areas had poorer oral health-related quality of life than those living in urban zones (P=0.02). The advantages of fluoridated water, dental insurance, greater awareness of preventive oral healthcare measures, and more restorative dental services, have helped this generation of adults maintain their teeth into their advanced years. The demand for oral health care services will be greater and more complex that of previous generations. Challenges for the older adult, and the healthcare system providing for them, are complex. Increasing evidence of a link between oral health and overall health underscores the need to promote public health awareness of the need for dental care to be part of overall health care. Understanding the attitudes, expectations, intentions, and current level of oral and overall health knowledge, is vital to helping these individuals' transition into increasing levels of dependency with dignity and a high level of oral health related quality of life. This study used a qualitative approach to better understand these issues from the perspective of the older adult. A purposeful sample of 26 independent living adults age 65 and older in Northwest Arkansas was selected for this investigation. Semi structured interviews were recorded, transcribed, and analyzed using constant comparative thematic analysis typical of qualitative methods to code, categorize, and develop themes. Results revealed the emergence of five themes: 1) Difficulties accessing dental care, 2) Stoic independence, 3) Taking care of your mouth as part of overall health, 4) Relationships affecting oral health related quality of life, and 5) Supporting roles. Conclusions reveal a need to increase oral health literacy of the older adult population with attention to reducing the modifiable risk factors associated with dental diseases. Additionally, there is a need to reevaluate public policy related to providing dental services in both the private and public sector.
