There are cumulative effects of oral diseases over time that may impact other health issues for older adults. Regular utilization of dental care is key to maintaining good oral health in later life; however, older adults identified oral health care as the most common health care service they need, which has significant barriers. The purpose of this study was to explore oral health experiences from the perspective of older adults' living in community dwellings: to identify facilitators and barriers to oral health care, and to determine how oral health services utilization compares to utilization of other healthcare services. An interpretive descriptive methodology was employed with a purposive sample of 12 adults, aged 70 years or older. The inclusion criterion was English-speaking seniors residing in community dwellings. Community dwellings were defined as any housing outside of long-term care or other supportive living facilities. Semi-structured interviews were 30-80 minutes, audio-recorded and transcribed verbatim. Three researchers participated in the comparative analysis process to develop codes, generate categories, interpret patterns, and construct themes. Three central themes surfacing from the data were: 1) life course influences on oral health; 2) transparency in delivery of oral health services; and 3) interrelationships between oral health and overall health. Study findings were interpreted within the context of current literature and a theory of aging. Perspectives of older adults in this study helped inform suggestions for improving dental and dental hygiene clinical interventions in the following areas: recognizing the value of establishing collaborative and trusting relationships between oral health practitioners and older adults; including oral health services as part of interdisciplinary care; and funding more research to expand research related to life course influences on oral health for older adults.

Book excerpt: Abstract : Although the United States has experienced population improvements in oral health over the past 60 years, oral diseases remain among the most common chronic conditions across the lifespan. Moreover, low-income, racial/ethnic minority, and elderly members of the population experience barriers to oral health opportunities. These barriers result in a lack of oral health awareness and dental hygiene, fewer contacts with the oral health care system, and poorer oral health outcomes. This dissertation research integrates different data sources and methods toward an enhanced understanding of the barriers faced in accessing dental care among racial/ethnic minority older adults in an urban context, and identifying opportunities for reducing barriers to oral health and health care. Qualitative data were analyzed from focus groups with community-dwelling older adults who self-identify as African American, Dominican, or Puerto Rican to elicit the lived experiences of older adults in accessing dental care in their northern Manhattan neighborhoods. To do so, this dissertation leveraged an approach to systematically extricating and representing factors associated with the decision-making process in finding and visiting dentists. The qualitative data analysis revealed that older adults want more information about oral health and their dental care options, particularly affordable dental care in their neighborhoods. The results from this qualitative analysis support the need for community-based oral health promotion and care. To further contextualize these findings, spatial data and analysis techniques were used to assess information about older adult populations and community resources for oral health promotion activities and dental care in the study area. The analysis of older adults’ lived experiences is thus situated in the community context. Therefore, this dissertation contributes approaches that may serve to inform context-specific strategies to promote oral health equity.

Book excerpt: Unlike previous generations, baby boomers are keeping their own natural teeth as they enter into older age. There is a growing body of research demonstrating the link between oral diseases and common chronic diseases. Therefore, it is imperative that older adults possess the necessary skills and knowledge, and participate in preventive oral health behaviours to remain healthy. The purpose of this exploratory, mixed-methods online study was to explore the influence of oral health literacy (OHL) and oral health chronic disease knowledge (OHCDK) on oral care behaviours (OCBs). A convenience sample of 69 community dwelling men (n=19) and women (n=50) was recruited to participate in the Oral Health Chronic Disease Knowledge Study. This Internet based study utilized 4 separate questionnaires: The Oral Health Literacy-Adult Questionnaire, The Oral Health Chronic Disease Knowledge Questionnaire, The Background Information Questionnaire, and The Oral Care Behaviours Survey. Findings from The Oral Care Behaviours Survey were further categorized to create the dependent variable: dental care behaviours, and the independent predictor variable: home dental aid use behaviour group. In addition, qualitative data was collected through the use of 9 open-ended questions. Participants ranged in age from 50 to 69 years, and were further subdivided into early boomers (n=34) with an average age of 63.9, and late boomers (n=35) with an average age of 54.4. All participants had achieved higher than high school education; over half of all participants (52 %) reported living in an urban setting, and 46 % had an annual household income of greater than $75,000. All participants had some natural teeth and 72 % had access to private dental insurance. The mean OHL score for the total sample was 13.36, 95% CI [12.62, 14.10] and the mean OHCDK score was 10.01, 95% CI [9.13, 10.89]. Based on a Pearson product moment correlation co-efficient procedure there was a low but positive correlation between total population OHL and OHCDK scores [r = 0.31, n =69, p = 0.008]. A dental care behaviours score was determined by adding together individual's total points related to the self-reported frequency of dental visits plus frequency of brushing and frequency of flossing. Scores could range from a maximum of 10 to a minimum score of 3, and higher scores denote less participation in positive dental care behaviours. The mean dental care behaviours score was 5.60, 95% CI [5.36, 5.84]. A backward step regression analysis determined the significant predictor model for dental care behaviours included: (1.46) dental insurance + (0.85) age cohort + (0.6) level of education. Book excerpt: The purpose of the study is to describe the impact of oral health-related quality of life (OHRQoL) on the lives of pre-seniors and seniors living in Nova Scotia, Canada. This cross-sectional study involved 1461 participants, grouped by age (pre-seniors [45-64] and seniors [65+]) and residential status (long-term care facility [LTC] or community). OHRQoL was measured using the 14-item Oral Health Impact Profile questionnaire. Approximately one in four pre-seniors and seniors reported at least one OHRQoL impact 'fairly/very often'. Of those residing in the community, pre-seniors (28.8%) reported significantly more impacts than seniors (22.0%). Logistic regression revealed that for the community dwelling sample, those who were dissatisfied with their teeth or dentures were 5.16 times more likely to report an impact 'fairly/very often', which was the strongest indicator. Among the LTC sample, those who have poor perceived mouth health were 9.87 times more likely to report an impact.

This population is undergoing a rapid transformation, becoming more affluent and better educated on average than in the past. Aging of the population presents a special challenge to dentists. Public health measures should be given priority and implemented. Promotional and preventive educational strategies involving media, self-care, self-monitoring as well as direct delivery by the dental professional, can improve oral health status of elders. These strategies minimize root caries; reduce gingivitis, bone loss, control denture stomatitis and aid in the early detection of oral diseases. It is important for the dentist to be aware of practical problem-oriented approaches that helps in patient management, maintaining & improving dental health as part of total healthcare services available to the elderly.
