Book excerpt: Although electronic health record (EHR) has been used for decades and progress has been made in its adoption, paper or document based forms are still in wide use in various clinical settings such as epilepsy center, movement disorder program, and cancer center. Effective data management approaches and systems are needed to be able to make use of the data generated from patient care in order to improve outcomes. In this thesis, we present the design, implementation, and evaluation results from three live clinical web systems: Trial Prospector, Ontology-driven Patient Information Capture system for epilepsy (OPIC), and DataBase system for Deep Brain Stimulation (DBSDB). These systems are for different clinical programs and have different application purposes, but the challenges faced in these programs are common and the methods and solutions introduced for these systems are general and interrelated. These methods work together to effectively complete clinical data management tasks, such as data capture, navigation, query, and visualization. The contributions of the thesis consist of: a generic configurable role-based access control management module to systematically authorize different group of users to the system according to their specific responsibilities; a design methodology of ontology-guided data capture; an ontology-driven interactive interface to build comprehensive queries for patient cohort identification; a new approach for data organization using a technique called active dashboard; and an improved method for agile software development. Trial Prospector manages 85 active clinical trials and has matched more than 5,374 patients to these trials to find eligible participants; OPIC has generated 1140 discharge summaries; DBSDB has captured more than 1,000 clinical data forms for 264 patients. DBSDB has 16 active users, Trial Prospector has 68 active users, and OPIC has active 100 users. Feedback from these users indicate that our systems are robust and user friendly.

Book excerpt: In low-resource settings, the prioritization of clinical care funding is often determined by immediate health priorities. As a result, investment directed towards the development of standards for clinical data representation and exchange are rare and accordingly, data management systems are often redundant. Open-source systems such as OpenMRS and OpenClinica provide an opportunity to leverage available systems to improve standards and increase interoperability. Nevertheless, continuity of care and data sharing between these systems remains a challenge, particularly in populations with changing health needs, and inconsistent access to health resources. The overarching goal of this project is to enable sharing of data across low cost systems like OpenMRS and OpenClinica using ontologies. The project consists of three aims: 1) describing clinical research and visit data related to the treatment and care of HIV/AIDS patients, 2) developing a prototype data integration system between electronic medical record and electronic data capture systems, and 3) evaluating the utility of the prototype system using simulated and real-world data. In the first aim, I developed a patient identifier and a HIV/AIDS treatment and care ontology to represent the types of data and information created and used by clinicians. This was achieved by gathering data forms used in HIV/AIDS clinics in low-resource settings. From these forms, the patient identifier and HIV/AIDS variables were extracted and used to create the ontologies. In aim 2, the ontologies from aim 1, along with simulated data, were used to develop a prototype data integration system that improves the ability of developers to implement integration systems that meet the needs of users, based on previously created use cases. In the third aim, I evaluated whether the matching algorithm used in the prototype can correctly identify matching patients, and whether the prototype is generalizable to clinical care and research data collected in a real world setting. This work contributes two ontologies to the medical and public health fields that are useful in providing standardization of data elements. Additionally, I provide a prototype data integration system that is useful in facilitating access to previously siloed data and helps reduce the burden of integrating future systems.

Book excerpt: While the use of database technology is ubiquitous throughout IT (and health IT in particular), it is not generally appreciated that, as a database increases in scope, certain designs are far superior to others. In biomedical domains, new knowledge is being generated continually, and the databases that must support areas such as clinical care and research must also be able to evolve while requiring minimal or no logical / physical redesign. Appropriately designed metadata, and software designed to utilize it effectively, can provide significant insulation against change. Many of the larger EMR or clinical research database vendors have realized this, but their designs are proprietary and not described in the literature. Consequently, numerous misconceptions abound among individuals who have not had to work with large-scale biomedical systems, and graduates of a health or bioinformatics program may find that they need to unlearn what they were taught in database and software design classes in order to work productively with such systems. A working knowledge of such systems is also important for individuals who are not primarily software developers, such as health informaticians, medical information officers and data analysts. This book is, in a sense, intended to prepare all of the above individuals for the real world.

This dissertation introduces an ontology-driven architecture for data federation. Ontological systems such as SNOMED CT have been traditionally used for data exchange, to preserve meaning for data elements across systems. We present a novel method of using ontologies to (1) enable data integration from distributed sources (2) facilitate a federated query interface. The contributions of this dissertation are trifold. First, we developed a unified model consisting of a sleep domain ontology and a units ontology into which data sources can be mapped using an intuitive Data Source to Ontology Mapper (DSOM). The DSOM allows for terminology standardization while accommodating local nomenclature. The domain ontology enriches the expressiveness of complex queries sent across data sources, based on graph traversal and ontological equivalence relationships. We provide further extensions to the ontology framework using formulae, for the purpose of inferring, discovering, and validating data across disparate sources. Second, we developed a powerful query interface called the VISual Aggregator and Explorer (VISAGE) for building and executing federated queries across mapped data sources. The interface streamlines the process of identifying case-control patient populations for study design through the use of embedded statistical tools. A framework is provided for the researcher to choose matching criteria and allows the researcher to create control sets based on frequency matching. Third, we implemented a fully operational and user-friendly system based on the ontology-driven architecture, as part of Physio-MIMI, an NCRR-funded multi-CTSA-site project. Using the Agile methodology to facilitate multi-site collaboration, our implementation leverages a suite of latest software development technologies: Ruby On Rails, Prototype JavaScript, AJAX, and Highcharts for a dynamic Web 2.0 interface, Mutual SSL for security and privacy, semantic web ontologies using OWL, and ontology development environments such as Protege. Preliminary comparative analyses and user evaluations of aspects of the system are provided, demonstrating superior results in several usability categories.
