Book excerpt: Rising consumer demand for fresh fruits and vegetables has led to the employment of five million Mexican-born workers on North American farms during a typical year. The migration of Mexican workers within and from Mexico has implications for North American agriculture, labor, and economic development. For instance, the guest worker systems of Canada and the US allow Mexican workers to earn five times more in six months than they could earn in a year at home, fueling the construction of trophy homes in rural Mexico but not necessarily spurring economic development. The expansion of export agriculture encourages internal migration from south-to-north within Mexico, which moves migrants to areas that offer higher wages but may subject some migrants to exploitation. In Bracero 2.0, Philip Martin draws on decades of research and experience to explore the role of rural Mexicans in North American agriculture, as well as the implications for farm employers and farm workers, consumers, and the economies of North America. Martin assesses the historical and current demand for and supply of farm labor and the operation of farm labor markets in Canada, Mexico, and the US. He also uses statistical and survey data to provide the most reliable portrait of the five million people who work for wages on North American farms and explores alternatives to US farm workers in major fruits and vegetables, showing how changing consumer preferences can speed or slow mechanization. Bracero 2.0 concludes with options to improve protections for farm workers, highlighting the need for systems that ensure continuous labor law compliance--as with food safety--rather than compliance only for government or private audits.

The unskilled immigrants who came to the United States from southern, central, and eastern Europe in the late 19th and early 20th centuries largely realized that vision. Within a few generations, their descendants rose to the middle class and beyond. But can today's unskilled immigrant arrivals—especially Mexicans, the nation's most numerous immigrant group—expect to achieve the same for their descendants? Social scientists disagree on this question, basing their arguments primarily on how well contemporary arrivals are faring. In Italians Then, Mexicans Now, Joel Perlmann uses the latest immigration data as well as 100 years of historical census data to compare the progress of unskilled immigrants and their American-born children both then and now. The crucial difference between the immigrant experience a hundred years ago and today is that relatively well-paid jobs were plentiful for workers with little education a hundred years ago, while today's immigrants arrive in an increasingly unequal America. Perlmann finds that while this change over time is real, its impact has not been as strong as many scholars have argued. In particular, these changes have not been great enough to force today's Mexican second generation into an inner-city "underclass." Perlmann emphasizes that high school dropout rates among second-generation Mexicans are alarmingly high, and are likely to have a strong impact on the group's well-being. Yet despite their high dropout rates, Mexican Americans earn at least as much as African Americans, and they fare better on social measures such as unwed childbearing and incarceration, which often lead to economic hardship. Perlmann concludes that inter-generational progress, though likely to be slower than it was for the European immigrants a century ago, is a reality, and could be enhanced if policy interventions are taken to boost high school graduation rates for Mexican children. Rich with historical data, Italians Then, Mexicans Now persuasively argues that today's Mexican immigrants are making slow but steady socio-economic progress and may one day reach parity with earlier immigrant groups who moved up into the heart of the American middle class. Although it is a national program, it adjusts payments to hospitals and health care practitioners according to the geographic location in which they provide service, acknowledging that the cost of doing business varies around the country. Under the adjustment systems, payments in high-cost areas are increased relative to the national average, and payments in low-cost areas are reduced. In July 2010, the Department of Health and Human Services, which oversees Medicare, commissioned the IOM to conduct a two-part study to recommend corrections of inaccuracies and inequities in geographic adjustments to Medicare payments. The first report examined the data sources and methods used to adjust payments, and recommended a number of changes. Geographic Adjustment in Medicare Payment - Phase II:Implications for Access, Quality, and Efficiency applies the first report's recommendations in order to determine their potential effect on Medicare payments to hospitals and clinical practitioners. This report also offers recommendations to improve access to efficient and appropriate levels of care.
