Book excerpt: On July 26, 2010, a 59-year-old male volunteer fire chief (victim 1) and a 67-year-old male volunteer fire fighter (victim 2) died from injuries sustained after they were ejected when their engine was involved in a crash and rolled over. The engine, with its lights and siren activated, was responding to a mutual aid residential structure fire. The crash occurred when the engine entered an intersection with a red light and was struck by a sport utility vehicle. The engine rolled over and both victims were ejected. Victim 1 was transported to a local hospital and pronounced dead. Victim 2 was pronounced dead at the scene. Both victims were reported to not be wearing their seat belts.

The tanker crossed the double-yellow center-line of a two-lane state roadway, and as victim #1 steered to the right, the tanker began skidding sideways. The tanker ran off the road, then skidded back onto the roadway and overturned several times before coming to rest sitting upside-down on the roadway and the shoulder of the road. Victim #1 was ejected and victim #2 was partially ejected from the cab of the tanker. Both of the victims were found unresponsive, and were pronounced dead at the scene by Emergency Medical Services (EMS).

The victim was en route to a vehicle fire and was approximately 1.4 miles from the fire station when the incident occurred. The engine left the roadway shoulder after misjudging the clearance to a large farm tractor stopped in the oncoming lane. The victim was not wearing a seat belt and was ejected from the vehicle. Another fire fighter onboard the apparatus crawled from the wreckage and began administering first aid to the seriously injured victim. The victim was airlifted to a medical center and was later pronounced dead. It was later determined that the victim was driving with a blood alcohol level in excess of the legal limit.

The Victim was traveling alone on a narrow, unmarked, asphalt road that had a bend to the right at the bottom of a slight grade. While attempting to negotiate the bend, the Victim lost control of the truck which was equipped with a slip-on unit, ran off the left side of the roadway and landed in a ditch embankment against a tree. The slip-on unit detached from the truck bed and landed near the front of the truck. Fire department personnel arrived quickly on the scene and began preparing the truck to remove the Victim who was alert and communicating. However, once extricated and transported to a local hospital he succumbed to his injuries.

The victim was entrapped and had to be extricated by fire fighters. The victim was transported to a regional medical center where she was pronounced dead upon arrival.

They arrived on scene to find a camper fully involved with fire and flames impinging on an adjacent mobile home. The occupants of the camper and mobile home escaped without injury, prior to the fire department's arrival. The victims entered the mobile home through the front door with a charged 1-in hoseline. Within 5 to 10 minutes of them entering, the pump operator sounded the evacuation alarm when he noticed his tank water was low. The victims did not evacuate from the structure. Fire fighters on scene attempted to contact the victims via radio and by yelling into the mobile home. The fire chief and a fire fighter tugged on the 1-in hoseline several times with no response.

He overcompensated, and the engine traveled across both lanes, left the road, and struck a large tree. The passenger was ejected from the cab of the engine and landed approximately 30 feet away. At 1537 hours, Central Dispatch requested all volunteers to respond to the scene to assist in the extrication of the victim. The victim was trapped in the engine and was removed approximately 1 hours later. The victim and the passenger were taken by ambulance to an area hospital. The passenger was hospitalized for 5 days, and the victim died 3 days after the incident.
