The special interest and importance, in a surgical sense, of the great European War lies not so much in the fact that examples of every form of gross lesion of organs and limbs have been seen, for if we read the older writers we find little in the moderns that is new in this respect, but is to be found in the enormous mass of clinical material which has been presented to us and in the production of evidence sufficient to eliminate sources of error in determining important conclusions. For the first time also in any campaign the labours of the surgeon and the physician have had the aid of the bacteriologist, the pathologist, the physiologist, and indeed of every form of scientific assistance, in the solution of their respective problems. The clinician entered upon the great war armed with all the resources which the advances of fifty years had made available. If the surgical problems of modern war can be said not to differ sensibly from the campaigns of the past, the form in which they have been presented is certainly as different as are the methods of their solution. The achievements in the field of discovery of the chemist, the physicist, and the biologist have given the military surgeon an advantage in diagnosis and treatment which was denied to his predecessors, and we are able to measure the effects of these advantages when we come to appraise the results which have been attained. For this reason the author has selected examples of different types of usual fracture pathology and endeavored to bring them before the reader's eye by means of line drawings which illus trate the essential points. Every illustration of this character is a careful reproduction of a tracing made from a Roentgenogmm of an actual case. The Artist, Mr. Streedain, and the author have worked for nearly two years preparing these drawings, checking and rechecking each one' until it conveyed the author's conception of the lesion. No one can read all roentgenograms with positive assurance. The author's obligations to Dr. Blaine, Roentgenologist to the Cook County Hos pital, for his friendly cooperation in this part of the work are gratefully recorded. The Artist has attempted to differentiate the shadows of bones more distant from the observer by lighter lines, and where two shadows overlap has indicated the one more remote by dotted lines. Cutaneous landmarks, lines of incisions with skin clips and plaster dressings have been occasionally indicated for obvious reasons. The profession at large has become reawakened to the problems of accident surgery. And. Incidentally, has come into a new relationship with the injured because of the operation of compensation and allied laws; likewise, the victims of accident, and civic, judicial, legal, and other agencies are exacting from the physician a higher grade of care and placing on him an added burden of responsibility. The writer has long been of the opinion that cases of injury have not received the same care and attention accorded other surgical patients, and has often realized that a properly treated Pott's fracture or infection of the hand is a far greater manifestation of the surgical art than the successful removal of an interval appendix. What follows is purposely didactic, and much of it relating to frac tures has hitherto been the subject of clinical lectures to successive groups of matriculants at the post-graduate Medical School. The text also aims to state the measures which the writer has found most practical in his own experience, and an effort has been made to unify and standardize the treatment of such common injuries as wounds, infections, burns. And the usual fractures. It will be noted that stress is placed on the routine use of but few antiseptics, the drainage of all wounds.
