Book excerpt: Excerpt from The Malarial Fevers of Baltimore: An Analysis of 616 Cases of Malarial Fever, With Special Reference to the Relations Existing Between Different Types of Haematozoa and Different Types of Fever During the five years between-june 14th, 1889, and August Ist, 1894, 616 cases of malarial fever were treated in the wards ahd in the medical out - patient department of the Johns Hopkins Hospital. In a considerable number of cases occurring in the out patient department, hasty or Insufficient examinations of the blood failed to Show the specific micro - organisms, while in other instances examination of the blood was omitted on account of lack of time. Most of these cases are classified in the hospital records as malarial fever they have not been included in our analysis. Excepting two or three instances where the patients entered the hospital during convalescence, the specific micro-organism was found in every case of malarial fever treated in the wards. The writers of this article have observed practically all the cases here classified after October, 1890. In a number of those observed before this time careful notes or drawings were made, which enabled us to draw definite conclusions with regard to the type of the organism. In other cases, however, the entries upon the records were so vague that, while the charts might justify a more or less decided opinion as to the nature of the infection, we have not included them among those cases Where the type was definitely differentiated. As a result of our observations, we believe that we have been able to recognize certain distinct types of parasites which appear to be related in turn to certain almost equally characteristic types of fever. It is the principal object of this paper to determine by a careful analysis of all of our cases in how far our impressions may be justified, and to compare our results with those of the numerous recent observers in this and in other countries. We have prefaced our analysis by an historical summary of the more important literature concerning the malarial parasite. It deals with a much-neglected subject in the text-books upon medicine, and one which is of the utmost importance to the profession. It will be noticed that I have not considered the subject of hemoglobinuric fever at all. This is not an oversight on my part, but because I believe that in a work such as this the subject should be confined as much as possible. As the etiology of hemoglobinuric fever is still somewhat in doubt, and as there are many excellent treatises upon it, I have thought it best to ignore it entirely. Today, however, these drugs no longer work against the deadliest form of malaria that exists throughout the world. Malaria deaths in sub-Saharan Africaâ€"currently just over one million per yearâ€"are rising because of increased resistance to the old, inexpensive drugs. Although effective new drugs called "artemisinins" are available, they are unaffordable for the majority of the affected population, even at a cost of one dollar per course. Saving Lives, Buying Time: Economics of Malaria Drugs in an Age of Resistance examines the history of malaria treatments, provides an overview of the current drug crisis, and offers recommendations on maximizing access to and effectiveness of antimalarial drugs. The book finds that most people in endemic countries will not have access to currently effective combination treatments, which should include an artemisinin, without financing from the global community. Without funding for effective treatment, malaria mortality could double over the next 10 to 20 years and transmission will intensify.
