Insurance companies routinely use an individual's medical history and family medical history in determining eligibility for life insurance; this is part of the process of medical underwriting. Insurers have also long used genetic information, often derived from family history, in underwriting. But rapid advances in gene identification and genetic testing are changing the way we look at genetic information. Should the results of genetic testing (which might identify a predisposition toward disease not related to medical history) be available to life insurance medical underwriters? Few if any life insurers currently require genetic testing, but there are no laws or regulations prohibiting its use. Genetics and Life Insurance examines the complex economic, legal, and social issues surrounding the use of genetic information in life insurance underwriting. Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: The year in which this first number of "Annals of Life Insurance Medicine" goes to press happens to be the Fiftieth Anniversary of the Swiss Reinsurance Com pany's activity in the field of underwriting and reassuring those risks which later became known as "substandard lives". In retrospect, it is a far cry from the old days when life assurance proposals were either accepted or rejected on medical grounds to the modern principles and methods of rating substandard cases both medically and actuarially. It can be assumed that in the course of the last few decades solutions, or at least approxi mate solutions sufficiently accurate for practical purposes, have been found to most of the numerous and often rather tricky actuarial problems relating to substandard policies, adequate premiums and reserves. No Life Assurer to-day however can fail to recognize that actuarial skill may only be applied to of medical assessment. Even the lay under substandard life risks on the basis writer certainly realizes that the medical and statistical problems inherent in the underwriting of substandard risks are infinitely more complex than any actuarial consequences of a calculated or assumed extramortality. It is primarily this basic fact which has stimulated the Swiss Reinsurance Company's plans to intensify and develop its research work in the field of the medical assessment of substandard lives.
