Book excerpt: Excerpt from The Etiology, Pathology, and Treatment of Diseases of the Hip Joint, Vol. 42 In the present state of Pathology it is impossible to present any classification of diseases of the hip joint which shall be exact, or which shall be sanctioned by authority weighty enough to give it pre-eminence. It is essentially a time of transition in the history of the pathology of bone and joint disease, a time when tuberculosis has sprung into such prominence that the tendency is to refer to it the great mass of all chronic bone and joint disease. Coming as it did upon a pathology in which tuberculosis played a minor part, this change has disturbed the former arrangement; and enough time has not yet elapsed for the evolution of a new and permanent basis of classification. Consequently, each one who deals with these subjects has to formulate for himself, as best he can under the existing conditions, the phenomena occurring in diseased joints. The classification given here is merely a scheme of abnormal conditions, which are to be clearly recognized in the hip joint, and which, it is hoped, has at least the merit of being practical. It follows no authority, because there is none to follow; and it must stand on its own merits as a practical working classification. It does not enter into the refinements of subdivision, and so far as is practicable it rests upon a pathological basis. It has not been possible, for obvious reasons, to follow this out rigidly; but, so far as possible, a pathological rather than an etiological basis has been adopted. That epilepsy, catalepsy, eclampsy, St. Vitus's dance, tetanus, trismus, spasms, and scores of other pathological phenomena, are the visible effects of reflex action, is no longer doubtful; besides, we know a vast number of circumstances under which reflex action occurs, and stimuli that produce them; but still our knowledge, as to the histology and the function of the spinal cord, and its ganglionic appendices, is far from perfect and infallible. We may, however, try to point out some analo gies in physiology and pathology, the comparison of which may lead to some conclusions important both to science and practice. Book excerpt: Excerpt from Hip Disease in Childhood: With Special Reference to Its Treatment by Excision The greater part of the present work has been already published in America, in the Archives of Pediatrics; and most of the cases in Appendix I., with some of those in Appendix II., have appeared in the annual numbers of Abstracts, edited by the staff of the General Hospital for Sick Children, Manchester. The text has, however, been much revised and expanded from the American issue, into which many mistakes crept from my having no opportunity of correcting proof sheets. The list of excisions has been enlarged, and later records of the condition of many of the cases obtained. That this record is not more complete is due to the practical impossibility of tracing many patients after their discharge from hospital. Most of the cases, however, have been followed up for considerable periods. I have thought it worth while to publish the work in its present form since hip disease is one of the subjects on which the most directly conflicting views are still held, especially as to the question of excision, and the results of an experience of several hundred cases cannot be without interest. The present uncertainty in the matter is, in my opinion, to no small extent due to the habit of assuming that all joints of the body are affected by disease in the same way, while, in fact, the effects of disease are as different as is the construction of the several joints, and the hip stands much by itself in its pathology.
