Book excerpt: Excerpt from On Disorders of Digestion, Their Consequences and Treatment When I was requested by the Medical Society to publish the Lettsomian Lectures which I had the honour to deliver before it, I intended to print them alone. But on looking over them I found that in many places I had referred for fuller particulars to papers which I had formerly published, and it occurred to me that some who might wish to consult these papers would be unable to obtain them, and others might like to be spared the trouble of hunting them up. I therefore decided to reprint some of them along with the Lettsomian Lectures. These papers were chiefly read at different times and before different audiences, and contain the same ideas repeated again and again, sometimes in nearly the same words, almost ad nauseam. Indeed, if any one should attempt to read this book straight through hewill probably throw it aside in utter disgust. But if he will only take the book up now and then at a spare moment, and read a single paper at a time, he may perhaps excuse the repetition, as it makes each paper more complete in itself. Moreover, the reason why certain ideas are repeated again and again is, that they seem to me very important, and I have decided to let the papers remain as they were originally published, without attempting to lessen the repetition, because I hope that the constant recurrence of certain ideas may obtain for them in the mind of the reader the same importance which they have in that of the writer. Book excerpt: Excerpt from On Disorders of Assimilation, Digestion, Etc The fact that such digestion actually does occur, and the observa tion that microbes have the power of modifying the enzymes they produce in accordance with the media upon which they grow, appear to afford an indication that the phenomenon of infective disease and microbes are due to digestion within the tissues by the enzymes formed by microbes. Further, that the increasing virulence of microbes growing in favourable media is due to an increased power of producing enzymes, whilst the acquired resistance of the tissues is probably due to an extension to them of the power of resistance to toxic substances naturally possessed by the intestine and liver. The paper on atropine in cholera shows the practical result of an attempt to cure infective disease by the use of an alkaloid antagonistic to one which, as I pointed out twenty-five years ago, could produce symptoms similar to the disease, and which alkaloid has now been shown to be a common product of the bacterial decom position of proteid substances. With the assistance of Dr. Rhodes, I again took up the question of glycolytic enzyme in muscle, and we have been able to confirm my earlier results by more perfect experiments, but we have not yet succeeded in isolating this enzyme. The clumping of bacilli as a question of re-action is interesting both theoretically and practically. Most of the other papers are clinical and practical in character. Had time and opportunity permitted, 1 should have liked to combine these fragmentary researches into a coherent whole by other experiments, but other demands upon my time prevent my doing more than collecting them into one book from the scattered publications in which they have appeared. The leading idea of many of them is the action of enzymes, and the recognition of their importance I owe to my teacher and friend, Professor Kiihne, who impressed it upon me both by his lectures and conversation when I was working in his laboratory in Amsterdam in 1868 - 69. At that time he had clearly grasped the idea not only of enzymes, but of zymogens, and stated that both in the stomach and pancreas substances existed, which, though not themselves enzymes or ferments, were capable of yielding active ferments. How much I owe to his teaching it is impossible for me to express, and although the world at large must mourn his loss, only those who knew intimately his person and work know how great that loss is. The major topics covered in my courses were fatty acids, prostaglandins, leukotrienes, phospholipids, glyco lipids, triacylglycerols, cholesterol, bile acids, and plasma lipoproteins. Empha sis was placed on the regulation and disorders of lipid metabolism. The latter included hyperlipidemias, atherosclerosis, and alcohol-induced liver damage. In this volume, I have chosen to focus on the disorders of lipid metabolism at a level appropriate both for medical students and for graduate and undergradu ate students majoring in the biological sciences. The biochemistry, nutrition, genetics, and cell biology aspects of lipids and lipid metabolism will be covered as they relate to lipid disorders. I am not aware of any textbook that integrates the disorders of lipid metabolism in this manner. Chapter 1 includes a brief discussion of the basic structures, properties, and metabolism of lipids. This chapter is not very detailed, since the material covered is available in basic textbooks on biochemistry. The major fpcus of this volume is the various lipid disorders, with emphasis on polyunsaturated fatty acids, the molecular biology and pathogenesis of the hyperlipidemias, dietary and drug therapy for the hyperlipidemias, and alcohol-induced liver damage. The material presented has been obtained from several textbooks on biochemistry and from a variety of recent articles in the scientific literature.
