Book excerpt: Areas of the brain which process olfactory information (entorhinal cortex, prepiriform cortex, and anterior olfactory nucleus) contain plaques, tangles, and cell loss in patients diagnosed with Alzheimer's disease (AD). These abnormalities are also associated with increasing loss of olfactory function in AD. Previous research on AD has shown marked impairment in olfactory functioning with respect to detection, recognition memory, identification, and quality discrimination. Diagnosis of definitive AD is only possible at autopsy, therefore during life, only a probable diagnosis is possible. As a result, better techniques for diagnosing AD are essential in order to provide a more definitive diagnosis of AD during life. Event-related potential (ERP) studies in the auditory and visual modalities have demonstrated significant differences between AD and normal controls including increased P2 and P3 latencies in AD patients. In addition, ERPs in these two modalities have proven useful in the study of early AD and as a predictor of mortality. The present study examines the olfactory ERP, its utility and usefulness in AD assessment, and ability to differentiate AD from normal controls, compared to auditory ERPs. Olfactory ERPs (OERP) and auditory ERPs were recorded from the Fz, Cz, and Pz electrode sites in 12 AD patients and 12 matched normal controls in a single-stimulus paradigm with a 45 sec inter-trial interval, using Amyl Acetate as the olfactory stimulus, and in a separate session a 500 Hz tone as the auditory stimulus. The results of the present study indicate: (1) a simple and short olfactory OERP single-stimulus paradigm is very sensitive to mild changes in dementia status and olfactory function in AD. OERP P2 & P3 latencies were significantly longer in AD patients than normal controls; (2) olfactory ERP latency measures correlated significantly with dementia status as measured by the DRS indicating that as participants performed worse (increase dementia) on the DRS, OERP latencies increased; (3) olfactory ERP latency measures better differentiate AD patients from normal controls than auditory ERP latency measures and correctly classify about 92% of participants; (4) and finally, this study demonstrated the replicability and usefulness of olfactory identification measures in the assessment of AD.

Book excerpt: The sense of olfaction declines with age and negatively impacts the lives of individuals. This decline can be seen in measurements of threshold, identification, and memory. Olfactory deficits beyond that of normal aging are present in Alzheimer's disease (AD). The earliest stages of neurodegeneration occur in olfactory processing areas of the brain. The Apolipoprotein (ApoE) [lower case epsilon]4 allele is a genetic risk factor for AD, and is associated with olfactory deficits. Presence of the allele is also associated with a more profound decrease in olfaction with age. This study examined differences in activation of olfactory and visual brain areas, reflected in event-related potential (ERPs) latencies and amplitudes, based on age group and ApoE [lower case epsilon]4 status. This measure was also compared to traditional olfactory behavioral tests. Latencies and amplitudes were compared across the N1, P2, N2, and P3 peaks to explore which peaks are most effective at differentiating among groups. Significant differences were found in olfactory ERPs based on age, ApoE [lower case epsilon]4 status, and their interaction. Visual ERPs were only significantly different by age. P3 was found to be the most effective peak to differentiate between ApoE[lower case epsilon]4 positives and negatives for the young and middle age groups, whereas N1 was the most effective peak for the older age group. Overall, the results suggest that olfactory ERPs may be able to detect more subtle differences due to aging and AD risk than traditional behavioral tests.
