Book excerpt: ABSTRACT: The impact of the HIV/AIDS epidemic on the lives of older individuals continues, with older adults comprising 10-12% of all diagnosed AIDS cases, acting as caregivers to infected individuals, sharing facilities and resources with infected individuals, and acting as a political force in a society struggling to deal with HIV/AIDS. Although it is clear older adults are in fact impacted by the HIV/AIDS epidemic, various studies show older adults exhibit less knowledge about HIV/AIDS. In order to effectively modify intervention and educational efforts, this research examines various factors believed to impact older individuals' HIV/AIDS-related knowledge. Utilizing the 1994 AIDS Supplement of the National Health Interview Survey, the following sets of hypotheses are tested: older individuals exhibit lower levels of knowledge; within the older adult population, men, individuals with lower levels of education, lower levels of income, members of racial/ethnic groups, individuals living outside of metropolis areas, married individuals, individuals who have less frequent contact with physicians, and individuals who report not knowing someone with HIV/AIDS exhibit lower knowledge scores. T-tests performed between older adults (age 50 and over) and younger adults support the first hypothesis. Regression analysis conducted on the remaining hypotheses show varied support for the remaining hypothesis, and provide focus for modifying intervention and education, as well as further research.

Book excerpt: Despite decades of attention on building a global HIV research and programming agenda, HIV in older populations has generally been neglected until recently. This new book focuses on HIV and aging in the context of ageism with regard to prevention, treatment guidelines, funding, and the engagement of communities and health and social service organizations. The lack of perceived HIV risk in late adulthood among older people themselves, as well on the part of providers and society in general, has led to a lack of investment in education, testing, and programmatic responses. Ageism perpetuates the invisibility of older adults and, in turn, renders current medical and social service systems unprepared to respond to patients’ needs. While ageism may lead to some advantages – discounts for services, for example – it is the negative aspects that must be addressed when determining the appropriate community-level response to the epidemic.

Book excerpt: Abstract: The purpose of this study was to investigate older adults' knowledge and perceived susceptibility to HIV/AIDS and their perception of self-efficacy in avoiding HIV/AIDS. Older adults (n = 33) from a local senior center completed the AIDS Questionnaire measuring these concepts. Findings indicated no correlation between knowledge and perceived susceptibility, however, knowledge was strongly correlated with self-efficacy. The findings indicated that knowledge of HIV/AIDS was a significant predictor of self-efficacy. In this sample, additional findings indicated a statistically significant difference between ethnicity and HIV/AIDS knowledge. Asians reported lower knowledge, yet had higher perceived susceptibility and self-efficacy than other ethnic groups.

The epidemic continues to expand, with an estimated doubling time of 10 years, making AIDS the leading infectious cause of death ahead of tuberculosis and malaria. Even in the U.S.A. where the death rate from AIDS is declining as a result of effective drug therapies, HIV infection rates continue to climb in several population groups. The prevalence of AIDS among people over the age of 50 is steadily increasing, and most older people are unprepared to address it for a number of reasons, including the widespread discomfort with matters sexual and homosexual and the belief that elderly people are not sexually active and therefore not at risk. This guide for care providers seeks to educate and inform readers about the difficulties and complications that accompany the disease in older people. Thus, while the appendix includes technical descriptions of methodology, data, and results, the narratives in the chapters describing the findings and their practical implications are written in layman's language. Method: An exploratory and descriptive study with a qualitative approach, conducted through interviews with 17 elderly participants in the Living Center for Senior Citizens in Teresina-PI. The data analysis allowed us to establish three categories: knowledge of elderly HIV / AIDS; methods used by the elderly in the prevention of HIV / AIDS and health education in the prevention of HIV / AIDS. Results: The elderly hold little knowledge about HIV / AIDS, with an emphasis on prevention, transmission and susceptibility of contracting HIV at any stage of life. Few subjects reported on the use of an adequate method of prevention. Conclusion: We emphasize the importance of Health Education as a strategy to improve the knowledge of the elderly population about important aspects of HIV / AIDS, mainly with regard to the myths related to the mode of transmission

Book excerpt: With the development of effective antiretroviral therapies (ART) in the mid-1990s, HIV became a treatable although serious condition, and people who are adherent to HIV medications can attain normal or near-normal life expectancies. Because of the success of ART, people 50 and older now make up a majority of people with HIV in high-income countries and other places where ART is accessible. The aging of the HIV epidemic is a global trend that is also being observed in low- and middle-income countries, including countries in sub-Saharan Africa, where the greatest number of older people with HIV reside (3.7 million). While globally over half of older adults with HIV are in sub-Saharan Africa, we have little information about the circumstances, needs, and resiliencies of this population, which limits our ability to craft effective policy and programmatic responses to aging with HIV in this region. At present, our understanding of HIV and aging is dominated by information from the U.S. and Western Europe, where the epidemiology of HIV and the infrastructure to provide social care are markedly different than in sub-Saharan Africa. Aging with HIV in Sub-Saharan Africa addresses this gap in our knowledge by providing current research and perspectives on a range of health and psychosocial topics concerning these older adults from across this region. This volume provides a unique and timely overview of growing older with HIV in a sub-Saharan African context, covering such topics as epidemiology, health and functioning, and social support, as well as policy and program implications to support those growing older with HIV. There are very few published volumes that address HIV and aging, and this is the first book to consider HIV and aging in sub-Saharan Africa. Most publications in this area focus on HIV and aging in Uganda and South Africa. This volume broadens the scope with contributions from authors working in West Africa, Botswana, and Kenya. The range of topics covered here will be useful to professionals in a range of disciplines including psychology, epidemiology, gerontology, sociology, health care, public health, and social work.

Book excerpt: Older adults account for 20% of new HIV infections in the U.S and 50% of people living with the disease. In part, this is due to people living longer with HIV, greater sexual activity in later life, a lack of condom use, and little understanding about HIV/AIDS. Evidence suggests that healthcare providers can play a key role in the dissemination of information about HIV/AIDS to older adults. The purpose of this study was to examine the impact of patient-provider communication of HIV/AIDS on older adults’ intentions to use condoms. The mediating relationships of knowledge of HIV/AIDS, perceived threat, subjective norms, and self-efficacy were also examined. This study utilized a cross-sectional, web-based survey, administered to adults aged 50 years and older (N=427). Forty-one percent of participants reported no communication with the healthcare provider about HIV/AIDS, yet 87% reported future intentions to use a condom with a new sexual partner. Participants demonstrated high knowledge of HIV/AIDS (M=14.7 out of 18), believed their sexual partner would be agreeable to using condoms (M=24 out of 36), and 91% believed they had the ability to properly use a condom. However, participants did not believe HIV/AIDS posed a threat to their health (M=8 out of 36). Mediation analysis yielded non-significant results. Healthcare providers can provide tailored prevention recommendations to higher risk older adults in order to curb the number of new infections. Additional research is needed with older adults who identify as LGBT, MSM, and older adults of color.
