Book excerpt: Excerpt from Observations Upon the Natural History of Epidemic Diarrhoea The causative agencies from which springs the plentiful harvest of child mortality in large cities may be conceived as a felted mass of rootlets almost inextricably intertwined. Those of poverty, bad housing, bad feeding, and neglect, may be severally recognized, but the extent of their interrelations with actual respiratory and alimentary disease - those to which the greater part of the mortality is referred - can be traced only with difficulty. It is the object Of this work to aid ln the labour of cutting away the matrix and entangling fibres and to lay bare the hidden ramifications of at least one important causative agency - epidemic diarrhoea. This affection - if we accept provisionally the more novel and generally favoured conception. As to its nature - is revealed as something very like an ordinary infectious disease, and one which permeates all classes, while the excessive mortality it gathers round itself in urban centres must be regarded as something superadded, owing to the vicious circle it forms with those baneful conditions of slum life mentioned above. On the other hand, its peculiarly intimate association with the circum stances of domestic life, from the continual faecal pollution of the interior of the household by infants and others, tends to make it more so than other affections of the kind peculiarly a class disease, and an especial scourge of dirty neighbourhoods. Dirty towns may however be saved from excessive mortality by a high percentage of breast feeding. A notable point in the interesting comparison that can be drawn between diarrhoea and typhoid fever is that, in accordance with their peculiarly opposite age-incidence curves, the marked and habitual depositing of infectious excreta within the household in the former disease may make the question of water-closet versus conservancy pan a matter of far less importance than in the latter. He cites an experiment with an infant Of 9 kg, who was in the chamber for one hour and ten minutes, and gives protocols for another experiment, one hour in length, presumably with the same infant. In summing up his averages he refers to numerous experiments on infants weighing from 6 to 9 kg, and includes Observations made with environmental temperatures rang ing from 18 C. To 25 C. He concludes that the infant produces on the average 4 calories per kilogram of body - weight per hour. Book excerpt: Excerpt from American Journal of Diseases of Children, Vol. 21 In the course of our metabolism observations on young children many interesting facts have been brought out with respect to the general food requirements as to total calories, distribution of the calories as fat, carbohydrate and protein, and other factors in the diet which must be considered. One notes in the literature on the food requirements of growing children a great difference of opinion among the various observers who have studied the subject. Many writers have taken up this problem from only a single point of view, such as the total caloric requirement, or the basal requirement, or the food needs in respect to one or another of the accessory food substances, the so-called "vitamins." We have, therefore, thought it desirable to consider in its different aspects the whole subject of the food requirements of the child during the entire period of growth. We shall discuss in this paper the total caloric requirements of children and the different factors which must be reckoned with in estimating the total calories. In succeeding papers we shall take up the proper distribution of the calories of the food as fat, carbohydrate and protein, also other important factors in the diet such as roughage, the difference in growth-promoting properties of the different proteins, the supply of essential inorganic salts and the accessory food substances or vitamins. At various times and by various authors each of the different factors mentioned has been discussed, but as yet no one has attempted to correlate them all and study the food needs of the child in the entire aspect. The estimates of total caloric requirements of children made by some authors have been purely hypothetical, while others have been based on the requirements of adults. The stretching out and manipulating of the legs and arms is among the earliest reflexes of the new-born. Because muscular exercise is so constantly a part of daily life and so obvious in its practical application, medical practitioners have come to regard its physiology as an established truth, into the true nature of which search is unnecessary. The subject of muscular exercise should be of more than mere academic interest to the pediatrist. A clearer understanding of the physiology of this subject and the establishment and effective application of physiologic standards will lead to a higher degree of efficiency of our school children and will increase our knowledge of diagnosis in children from a functional point of view. Only on the basis of quantitative scientific investigation of the physical capacity of normal children of all ages and of the effect of muscular activity of all types on the circulatory system, can we ever hope adequately to provide for physical education and successfully to cope with the prevention of those diseases in children which are dependent on muscular deficiency. When we recall that 30 per cent. of our soldiers were physically defective; when we learn from a survey of 20,000 New York school children that from 20 to 30 per cent. have spinal curvature, that about 10 per cent. have other postural defects, that from 6 to 30 per cent. suffer from malnutrition, that 5 per cent. exhibit other signs of muscular insufficiency; when we understand that in a survey of a public school in Minneapolis, 8 per cent. of the children were found to be asthenic, the analogue of soldiers with "effort syndrome"; when we bear in mind that all of these conditions are preventable and curable by general physical education and properly administered corrective exercises, we must acknowledge the practical importance of a fundamental study of the effect of various types of work and play under all obtaining conditions on the circulatory system of the child.
