Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. Das in Calcutta, who requested me to accept, for eventual analysis, a mountain of anthropometric data he had accumulated, as he was ill and did not expect to survive to analyse it himself. The data provided the astonishment; twenty-two anthropometric characters recorded every six months or a year, over a period of 14 years, in a mixed longitudinal study of some 560 children, aged six months to twenty years. Most were in families with siblings also in the study, and every child was measured every time by S. R. Das himself. The archive was unique, combining the personal anthropometry of R. H. Whitehouse in the Harpenden Growth Study and the family approach of the Fels Growth Study. This was a study of which neither I, nor anyone of my acquaintance, had heard. Even in India, Prof. Das' work was scarcely known. It turned out Das was a scholarly man, quiet and unassuming, absolutely committed to his Sarsuna-Barisha Growth Study,just the obverse of the professional showman. Clearly this was not a request I could refuse, although I already had in hand enough projects to occupy Siva himself.

Uncertain physical, social, cultural, and economic situations limit access to nutritious intake and impact health both in the developing (e.g., wasting, stunting, underweight, and mineral/vitamin-related malnutrition) and developed (e.g., obesity) worlds. However, a healthy diet is not only determined by food safety and quality, but also by food education, nutritional practice during the life stages, and food availability and accessibility to the population. This is true in developing countries, but also in developed ones, where vulnerable groups (including people living in conditions of poverty, unemployment, lack of schooling, people with forms of addiction and ethnical minorities) struggle to adhere to healthy diets and choose cheaper, and often unhealthy options. As a consequence, excessive consumption of limited nutrients and inappropriate supplementation becomes as unhealthy as not consuming enough. In the past three years, the pandemic of COVID-19 has put significant troubles on human society, exacerbating hard life conditions in vulnerable groups and worsening their adherence to healthy lifestyles and food choices. Therefore, the nutrition and diet-related health issues in vulnerable people must be highlighted and followed up.

This report presents the first global assessment of food insecurity and malnutrition for 2020 and offers some indication of what hunger might look like by 2030 in a scenario further complicated by the enduring effects of the COVID-19 pandemic. It also includes new estimates of the cost and affordability of healthy diets, which provide an important link between the food security and nutrition indicators and the analysis of their trends. Altogether, the report highlights the need for a deeper reflection on how to better address the global food security and nutrition situation. To understand how hunger and malnutrition have reached these critical levels, this report draws on the analyses of the past four editions, which have produced a vast, evidence-based body of knowledge of the major drivers behind the recent changes in food security and nutrition. These drivers, which are increasing in frequency and intensity, include conflicts, climate variability and extremes, and economic slowdowns and downturns – all exacerbated by the underlying causes of poverty and very high and persistent levels of inequality. In addition, millions of people around the world suffer from food insecurity and different forms of malnutrition because they cannot afford the cost of healthy diets. From a synthesized understanding of this knowledge, updates and additional analyses are generated to create a holistic view of the combined effects of these drivers, both on each other and on food systems, and how they negatively affect food security and nutrition around the world. In turn, the evidence informs an in-depth look at how to move from silo solutions to integrated food systems solutions. In this regard, the report proposes transformative pathways that specifically address the challenges posed by the major drivers, also highlighting the types of policy and investment portfolios required to transform food systems for food security, improved nutrition, and affordable healthy diets for all. The report observes that, while the pandemic has caused major setbacks, there is much to be learned from the vulnerabilities and inequalities it has laid bare. If taken to heart, these new insights and wisdom can help get the world back on track towards the goal of ending hunger, food insecurity, and malnutrition in all its forms.
