Severe malnutrition can cause significant alteration in immune response, but even subclinical deficits may be associated with an impaired immune response, and an increased risk of infection. Infectious diseases have accounted for more off-duty days during major wars than combat wounds or nonbattle injuries. Combined stressors may reduce the normal ability of soldiers to resist pathogens, increase their susceptibility to biological warfare agents, and reduce the effectiveness of vaccines intended to protect them. Considerable emphasis on prevention of new infections and treatment of those living with HIV will be needed to make this goal achievable. With nearly 37 million people now living with HIV, it is a communicable disease that behaves like a noncommunicable disease. Nutritional management is integral to comprehensive HIV care and treatment. Improved nutritional status and weight gain can increase recovery and strength of individuals living with HIV/AIDS, improve dietary diversity and caloric intake, and improve quality of life. This book highlights evidence-based research linking nutrition and HIV and identifies research gaps to inform the development of guidelines and policies for the United Nations’ Sustainable Development Goals. A comprehensive approach that includes nutritional interventions is likely to maximize the benefit of antiretroviral therapy in preventing HIV disease progression and other adverse outcomes in HIV-infected men and women. Modification of nutritional status has been shown to enhance the quality of life of those suffering HIV/AIDS, both physically in terms of improved body mass index and immunological markers, and psychologically, by improving symptoms of depression. While the primary focus for those infected should remain on antiretroviral treatment and increasing its availability and coverage, improvement of nutritional status plays a complementary role in the management of HIV infection.

Its authors have been courageous in bringing together in one text a triad of topics that cover such large tracts of biomedical sciences as epidemiology, biochemistry, immunology, and clinical medicine. Malnutrition and infection are known to be closely linked, the one promoting the other. The adaptive immune system forms a part of the link since it is responsible for a good deal of defense against infection, and it may be affected adversely by malnutrition and indeed by infection itself. Knowledge in this complex field is of great potential importance because malnutri tion and infection are such dominant features of the ill-health of many of the world's underprivileged people. As this book shows, there is no lack of technical facets for study. There are now so many components of the immune response which can be measured or assessed and so many aspects of nutritional biochemistry which can be studied that the problem is to select what to study and where to begin. Moreover, the great number of variables in the nature of nutritional deficiencies, in types of infections or multiple infections and in the genetic, environmental, and social background of the affected people, all combine to make interpretation and application of findings a speculative business. There are several high-risk scenarios for the development of malnutrition, which are the focus of the current publication: Atopic diseases, gastrointestinal disorders, and preterm delivery. For the pediatric allergist it is important to understand the mechanisms regulating IgE responses to food proteins since they may also be the earliest markers for the atopic march. As breastfeeding seems to have only limited effects regarding the atopic march, other measures to modulate infantile immune responses have to be taken, including the use of hypoallergenic formulae or the addition of probiotics or prebiotics to infant formulae. The second part of this book highlights the functional properties of nutrition with regard to diseases of the gastrointestinal tract and the ensuing chronic alterations of gastrointestinal function. Topics addressed include the molecular basis of some diseases, main causes of and nutritional measures in chronic enteropathy, including the role of parenteral and enteral nutrition, stressed mucosa and the role of nutrition in cholestatic liver disease. Nowadays, smaller and more immature infants are surviving - but the smaller the infant, the greater the accrued deficit as nutritional needs change with advancing maturity, and one formulation may not meet all requirements. Furthermore, there are no sensitive, accurate and precise measures of nutritional outcome. The net effect of these uncertainties is that all very-low-birth-weight infants are growth retarded at hospital discharge. Strategies for improving growth in these high-risk infants are at the center of the last part of this publication.
