Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

Book excerpt: Africa is not on track to meeting the Sustainable Development Goal (SDG) 2 targets to end hunger and ensure access by all people to safe, nutritious and sufficient food all year round and to end all forms of malnutrition. The number of hungry people on the continent has risen by 47.9 million since 2014 and now stands at 250.3 million, or nearly one-fifth of the population. The 2017, 2018 and 2019 editions of this report explain that this gradual deterioration of food security was due to conflict, weather extremes, and economic slowdowns and downturns, often overlapping. A continued worsening of food security is expected also for 2020 as a result of the COVID-19 pandemic. In addition to hunger, across all countries in Africa millions of people suffer from widespread micronutrient deficiencies, and overweight and obesity are emerging as significant health concerns in many countries. This report shows that the food system in Africa does not provide food at a cost that makes nutritious food affordable to a majority of the population, and this is reflected in the high disease burden associated with maternal and child malnutrition, high body-mass, micronutrient deficiencies and dietary risk factors. The report also shows that current food consumption patterns impose high health and environmental costs, which are not reflected in food prices. The findings presented in this report highlight the importance of prioritizing the transformation of food systems to ensure access to affordable and healthy diets for all, produced in a sustainable manner.

Book excerpt: At STIAS, the ‘Health in Transition’ theme includes a programme to address the epidemic rise in the incidence of non-communicable diseases (NCDs) such as Type 2 diabetes, hypertension, obesity, coronary heart disease and stroke in Africa. The aim is to advance awareness, research capacity and knowledge translation of science related to the Developmental Origins of Health and Disease (DOHaD) as a means of preventing NCDs in future generations. Application of DOHaD science is a promising avenue for prevention, as this field is identifying how health and nutrition from conception through the first 1 000 days of life can dramatically impact a developing individual’s future life course, and specifically predicate whether or not they are programmed in infancy to develop NCDs in later life. Prevention of NCDs is an essential strategy as, if unchecked, the burden of caring for a growing and ageing population with these diseases threatens to consume entire health budgets, as well as negatively impact the quality of life of millions. Africa in particular needs specific, focussed endeavors to realize the maximal preventive potential of DOHaD science, and a means of generating governmental and public awareness about the links between health in infancy and disease in adult life. This volume summarizes the expertise and experience of a leading group of international scientists led by Abdallah Daar brought together at STIAS as part of the ‘Health in Transition’ programme.

In many African countries, large proportions of the population rely on agriculture not only for their food - but also for their livelihoods. A transformed agricultural and food system is thus a necessary condition for addressing this double-triple challenge. Additionally, post harvest and food waste and losses reduce the availability of sufficient quantities of safe, edible and preferable foods. At least one third of food produced at farm level is lost due to inappropriate storage, infrastructure and agro-processing technologies in developing countries; and one third of food purchased is wasted at household and retail level.

Book excerpt: In the 2017 and 2018 editions of the Africa Regional Overview of Food Security and Nutrition, FAO reported that the prevalence of undernourishment was rising in the region. The latest data shows that the deterioration has slowed, but there remain 256 million hungry people in Africa today. The report further documents that although many African countries are making progress towards reducing malnutrition, progress is too slow to meet six key nutrition targets, which form part of the Sustainable Development Goals (SDGs) monitoring framework and the World Health Assembly global nutrition targets. Food insecurity has been rising in Africa in recent years and the continent is not on track to eliminate hunger by 2030. The 2017, 2018 and this year’s report identify and report in detail on conflict, climate extremes and economic slowdowns and downturns as the key drivers of the rise in food insecurity. In most cases, the economic slowdowns and downturns that contributed to rising undernourishment in 2014–2018 were the result of commodity price falls. Many effective policy tools are available, but their adoption will depend on the availability of fiscal space to effect the desired policy action. In the longer-term, countries must develop policies and invest to achieve a more diversified economy and achieve an inclusive structural transformation. However, sustained economic growth is not enough: reducing inequalities, including gender-based and spatial inequalities, is essential to strengthening household resilience, laying the path to inclusive growth and reducing food insecurity and tackling the multiple forms of malnutrition.
