The genesis of this Conference came from the work of the Subcommittee on Nutrition and Fertility of the Committee on International Nutrition Programs of the National Research Council. The purpose of the Conference was to assemble scientists and program planners from a broad range of fields including nutrition, epidemiology, demography, endocrinology, sociology, economics, anthropology, biostatistics and public health. Each individual brought his or her analytical skills and perspective to the meeting, with the goal of developing a more coherent picture of the many facets of nutrition and reproduction. The approach was to get a more comprehensive view by: 1. Clarifying terminology and definitions. 2. Reviewing recent and current work on the biological basis for nutrition-fertility interactions. 3. Reviewing biomedical and socioeconomic factors related to breast-feeding to assess how this practice relates to maternal and infant nutrition and fertility. 4. Assessing some current analytical models for defining nutrition-fertility interrelationships. 5. Reviewing recent field studies from Africa, Asia and Latin America which are examining the interrelationships of nutrition and reproduction.

While it is evident that hormonal adjustments affect nutrient requirements, these are rarely considered when nutrient recommen dations are made for pregnant or lactating women, and often neglected during evaluation of nutritional status. In addition, changes in nutrient metabolism during the stages of pregnancy and oflactation are usually considered separately, while in reality events during pregnancy can have a major influence on nutritional status and nutrient requirements during lactation. The purpose of this volume is to describe changes in the metabolism of important nutrients during pregnancy and lactation, including the physiological basis for these changes and their implications for nutrient requirements and assessment. Authors have considered such issues as inter-relationships between endocrine changes and nutrient metabolism at the tissue, cellular and molecular level; alterations in nutrient binding proteins; the efficiency of nutrient absorp tion and retention; and the impact on maternal as well as fetal nutritional status. Another unique aspect of this book is the focus on pregnancy and lactation as a continuum.

Book excerpt: A review of 120 studies published since 2006 was undertaken to examine the relationship between food insecurity at the household or individual level and the following nutrition indicators: child stunting, child wasting, low birth weight, exclusive breastfeeding of infants < 6 months of age, anaemia in women of reproductive age, child overweight and adult obesity. While there is some evidence of a direct association between food insecurity and stunting for children in lower-middle and upper-middle income countries, evidence of links between food insecurity and either child wasting or overweight is almost absent, with the exception of an association with overweight among girls in middle- and high-income countries. The obesity–food insecurity link is most predominant among women in high-income countries, while it is almost absent in men. In addition, food insecurity increases the risk for low birth weight in infants and anaemia in women. Methodological concerns that pose challenges for valid comparison of results relate to study design, data analysis techniques, use of different indicators of household/individual food security and malnutrition, and the limited availability of high-quality micro-level data from large-scale surveys. Most studies report correlation rather than causal associations between food insecurity and nutrition indicators; longitudinal micro-level data from large-scale surveys can help establish causal association and capture the dynamic nature of food insecurity. Food insecurity emerges as a predictor of undernutrition as well as overweight and obesity, highlighting the need for multisectoral strategies and policies to combat food insecurity and multiple forms of malnutrition.

Health outcomes have also improved rapidly in recent years..."Several reforms were attempted during the health sector's recuperative years. Some were successful, others failed. To increase the chances for success in the future, policymakers in Peru must face three key challenges: • continue to reduce the large gap between the health status of the poor and that of the nonpoor; • increase the resources assigned to provide care for the poor; • increase the efficiency in the use of these resources. This report is meant to help the Government continue to develop its agenda to improve health care for the poor. Its focus is on poverty, and particularly on ways to improve the Ministry of Health's primary health subprogram, rather than on sector-wide reforms, to increase efficiency. The report emphasizes that there is much to be gained by sustaining and deepening the reforms directed toward improving health care for the poor while using the shift in focus and responding to the key challenges. The executive summary and introduction are provided in both English and Spanish.
