Book excerpt: The Dedicated Education Unit (DEU) is a model of clinical teaching which has gained widespread acceptance and national recognition as an innovative method of clinical education for undergraduate nursing students (Robert Wood Johnson Foundation, 2010; Warner & Burton, 2009). The primary goals and benefits of the DEU for schools of nursing are expansion of educational capacity within a nursing faculty shortage and increased clinical placements while providing a high quality experiential learning environment (Murray, Crain, Meyer, McDonough, & Schweiss, 2010). Registered nurses (RNs) are the key component in the DEU clinical teaching model and serve as preceptors and valuable role models. Nurse preceptors are caught in challenging situations where they experience significant pressures to deliver quality nursing care under heavy workloads and are responsible for teaching and supervising students. The current quasi-experimental, investigational study reveals the relationships between the DEU model and the quality of patient care. Utilizing the nursing sensitive indicators of nursing hours per patient day --- Registered Nurse, hospital acquired pressure ulcer rates, patient fall rates, and medication error rates, the study found no negative impact on the quality of patient care provided on DEUs despite the increased workload for precepting registered nurses.

At more than 3 million in number, nurses make up the single largest segment of the health care work force. They also spend the greatest amount of time in delivering patient care as a profession. Nurses therefore have valuable insights and unique abilities to contribute as partners with other health care professionals in improving the quality and safety of care as envisioned in the Affordable Care Act (ACA) enacted this year. Nurses should be fully engaged with other health professionals and assume leadership roles in redesigning care in the United States. To ensure its members are well-prepared, the profession should institute residency training for nurses, increase the percentage of nurses who attain a bachelor's degree to 80 percent by 2020, and double the number who pursue doctorates. Furthermore, regulatory and institutional obstacles-including limits on nurses' scope of practice-should be removed so that the health system can reap the full benefit of nurses' training, skills, and knowledge in patient care. In this book, the Institute of Medicine makes recommendations for an action-oriented blueprint for the future of nursing.

Book excerpt: The changes in the healthcare environment, safety concerns of the practice setting and patient acuity has supported reform and research to identify areas for improvement (IOM, 2001, 2003). The Carnegie Foundation's Educating Nurses, A Call for Radical Transformation (Benner, P., Sutphen, M., Leonard, V. and Day, L., 2010) explored the state of American nursing education. Among the findings are: patient safety issues, higher patient acuity, the increased complexity of nurse's work, shortages of nursing faculty and clinical learning sites, the current and predicted shortages of registered nurses, and the chaotic, fragmented hospital work environment. The call to action is to improve patient care through transforming the education of undergraduate nursing students. This study is a qualitative exploration of how nursing students develop practice knowledge in their undergraduate clinical experience. Clinical practice knowledge development is explored using the epistemological concepts of the discipline of nursing- empirics, aesthetics, ethics and personal knowing as described by Carper (1978), unknowing by Munhall (1984) and sociopolitical knowing as described by White (1995). The study utilized individual interviews exploring the learning processes of developing nursing practice knowledge by undergraduate baccalaureate nursing students from UMASS at Amherst who have had clinical experiences in both a Dedicated Education Unit (DEU) and non-DEU clinical settings. Change in the education of nurses must be guided by research to support best practices. Clinical education is a crucial aspect of the practice development of student nurses. The development of nursing knowledge comes together in a model of Synergistic Clinical Education, incorporating the identified attributes supporting learning: the student, learning environments and relationships. This study supports the utilization of Dedicated Education Units as a clinical education model providing an optimal learning environment in which the development of nursing knowledge and clinical practice is more likely to happen than in any other clinical experience setting. Book excerpt: This research examines the experience and self-regulated learning capacities of second-career nursing students who have received clinical teaching in the VA Hospital's dedicated education unit (DEU). Grounded in Kuiper's self-regulated learning (SRL) theory (2005), the study focuses on the apsect of behavioral self-regulation and self-monitoring in which second-career students acquire nursing competency. Under these premises, the clinical practicum setting and the role of nurse preceptors play a critical part in students' learning process. Two unstructured, in-depth focus groups with six and seven participants respectively have been completed. The focus group transcripts are improted for deductive analysis using NVivo 9 computer software. Prior to the focus group, a brief demographic survy was administered anonymously to those who attended the focus group. The focus group dialogues are summarized in a matrix format. Three dominant themes have been identified from the focus group discussion: 1) The DEU is conducive to learning; 2) Real learning takes place in clinical settings; 3) Barriers. The associated threads under each theme are presented. The study concludes that the DEU has a positive impact on the clinical learning experience of second-career students. The development of clinical competency is affected by the self-regulated learning capacity of the students. The environment of the DEU, which provides a conducive physical context, supportive preceptorship, and relevant structure and organization, facilitates self-regulated learning. Placement in the DEU does not provide an answer to all the problems. Therefore, recommendations for practice are made based on the research findings including: 1) Pre-enrollment training as a pre-requisite; 2) Maximizing clinical learning opportunities while enrolled by taking advantage of simulation learning. This study provided preliminary insight into second-career clinical learning. Limitations are also identified and proposals are made to facilitate future research for better understanding of the competency development of this unique student population.

The Support Instrument for Nurses to Facilitate the Learning of Others (SINFLO) was used to survey learning guides in the states who are members of the Southern Regional Education Board, plus Portland, Oregon, to determine the support services for learning guide respondents with academic appointment (that is, adjunct faculty status) versus those without academic appointment. The SINFLO was interrelated with the Roy Adaptation Model (RAM) theory of nursing to determine the proportions of respondents with and without academic appointment that achieved adaptation in the RAM areas of role performance, nursing care group, interdependence, and self-concept.
