Book excerpt: Adolescents frequently experience perceived and actual barriers to sexual and reproductive healthcare related to confidentiality, access, and their relationship with healthcare providers. Advanced Practice Registered Nurses (APRNs) are well positioned to work with adolescents in a variety of non-acute care settings to provide preventative sexual healthcare services. No studies have recorded Nurse Practitioners' (NPs) and Certified Nurse Midwives'/Certified Midwives' (CNMs/CMs) adolescent sexual health knowledge, attitudes, and beliefs. The purpose of this study was to determine whether there were any differences between Women's Health Nurse Practitioners (WHNPs) and CNMs/CMs whose educational programs have a concentration in sexuality, and APRNs from other specialties (ANPs, FNPs, PNPs, and PMHNPs) regarding knowledge, attitudes, and beliefs toward adolescents' sexual health. The study used a cross-sectional, descriptive comparative design. A convenience sample was recruited through the National Association of Nurse Practitioners in Women's Health (NPWH) and the Nurse Practitioner Association- New York State (NPA) e-newsletters and surveyed online with The Sex Knowledge Attitude Test (SKAT), the Sexuality Attitudes and Beliefs Survey (SABS), and the National Violence Against Women Survey (NVAWS) to assess sexual health knowledge, sexuality attitudes, beliefs about addressing adolescent's sexual health, and personal experience of sexual violence. One open ended question elicited responses of qualitative data. Demographic data of participants (n = 204) were analysed and 171 participants finished the online survey. Multiple logistic regression analysis shows that the WHNP/CNM/CM group have significantly more positive beliefs about addressing adolescents' sexual health than the Other NPs group, [beta] = -3.36, p = .003. Book excerpt: ABSTRACT Background: Sexual orientation is comprised of distinct components, including sexual identity, sexual attraction and sexual behavior. Lesbian, gay and bisexual adolescents are at an increased risk of experiencing poor health outcomes compared to non-sexual minority youth. Health care professional organizations recommend that health care providers discuss each component of sexual orientation at every adolescent health supervision visits in order to best assess the adolescent’s health risks and needs for intervention and education. Objective: This survey assessed the frequency with which nurse practitioners (NPs) in the state of Vermont discussed sexual identity, attraction and behavior with adolescents during annual health supervision visits. Design: A cross sectional study that analyzed descriptive statistics of a small convenience sample of Vermont NPs. Setting and Participants: Attendees of the Vermont Nurse Practitioner Association 2015 annual conference. Participants in the study were licensed, practicing NPs in the state of Vermont responsible for the health supervision of adolescents. Results: Participants were overwhelmingly female (93%), with a median age between 40-49 years old, and a median length of years in practice of six to ten years. Sixty-two percent of respondents specialized in family practice. Respondents reported that they always asked adolescents about the sex of sexual partners at 49% of health supervision visits. Respondents always discussed sexual attraction and sexual identity at 31% and 24% of health supervision visits, respectively. Twenty percent of respondents reported rarely or never discussing sexual attraction, and 38% reported rarely or never discussing sexual identity. Conclusions: The Vermont NPs who participated in this survey were demographically similar to national NP cohorts. Vermont NPs discussed the adolescent’s sexual behavior at health supervision visits as frequently as health care providers nationally, and Vermont NPs discussed sexual attraction and sexual identity more frequently than providers nationally. However, Vermont NPs discussed sexual attraction and identity much less frequently than they discussed sexual behavior. Results of this survey illustrate that there is substantial room for improvement regarding the frequency with which Vermont NPs discuss the three components of sexual orientation with adolescents, particularly the components of sexual identity and attraction.

Book excerpt: Risk-taking may be an expectation of adolescent development, but negative outcomes related to risky behaviors such as use of drugs, alcohol, and sexual activity require attention from healthcare professionals. For the population of adolescents with chronic medical conditions, the motivation to engage in risk behaviors and outcomes of participation may be complicated by the chronic illness. Adolescents with chronic conditions may not receive routine screenings, due to a disease focus, lack of a primary care provider, or for other reasons. Nurse Practitioners (NP) providing care for adolescents with chronic conditions have a critical role in screening. Despite frequent contact with healthcare professionals, adolescents with chronic conditions are not less likely than other adolescents to participate in risky behaviors but may be less likely to receive routine screening. This descriptive phenomenological study describes the NPs’ experiences of screening for risk behaviors in adolescents with chronic medical conditions. Interviews were conducted with NPs employed in outpatient settings and whose patient population included adolescents with chronic medical conditions. Data analysis followed Colaizzi’s method. Three integrated themes emerged: the act of screening with skills and resources to facilitate the practice, influence of the “delicate and complex” relationship between NP and adolescent, and the perceived “responsibility,” or role of the NP. Screening practice varied. Routine screening occurred when skills and resources, the relationship, and NP perception of the role intersected. Approximately half of NPs incorporated consistent screening. For others, decision-making was guided by multiple factors, including the priority of the medical condition, NP comfort with risk behaviors, time, risk of the condition or treatment, the NP-patient relationship, and the NP’s view of the responsibility of screening. The findings of the study demonstrate that screening is a complex process. Given the prevalence of risk behaviors, routine screening should take place. NPs caring for adolescents with chronic conditions are in a position to incorporate screening. Implications for practice include increasing awareness of risk behaviors in adolescents with chronic conditions, provision of tools and skills to facilitate screening, and institutional support. Implications for research include additional studies evaluating the impact of screening on risk behavior outcomes.

Book excerpt: The purpose of this study to examine certified nurse practitioners' learning needs related to sexual health concerns, to assess frequency of sexual health interventions, and to evaluate the perceived barriers to the application of sexual health interventions in health care. Descriptive study administered via internet-based questionnaire. A convenience sample of 574 certified nurse practitioners (CNP) was obtained from Ohio Board of Nursing. The Learning Needs for Addressing Patients' Sexual Health Concerns was used to assess knowledge. The Nursing Interventions on Sexual Health Scale was used to assess the frequency of sexual health interventions used in nursing practice. The perceived barriers to the utilization of sexual health interventions among CNPs were assessed using a checklist based on the literature pertaining to sexual health care in nursing. "The influence of treatment on sexuality" was the highest learning need. "Biopsychosocial factors on altered sexual activity" was the lowest learning need. The frequency of nursing intervention in sexual health care among CNPs is average and the behavioral frequency of nursing intervention decreases from the permission level to the specific suggestion level when evaluating care based on the PLISSIT model. Barriers to sexual health care are similar to what is reflected in the literature. While nurse practitioners have the knowledge to manage sexual health concerns, there is a need for further research. CNPs require comprehensive sexual health education and specialized clinical preparation to manage sexual health concerns.

Adolescents are known to be at risk for unplanned pregnancy and associated negative consequences, but marginalized female adolescents are even more vulnerable. Prevention of pregnancy through access and availability of effective contraception is a key facet in meeting the sexual health care needs of this population. The purpose of this paper was to explore the nurse practitioner role in improving the sexual health of marginalized female adolescents through contraceptive prescribing practices, specifically through prescription and administration of the LNG-IUS. A review of current literature was undertaken to examine the prescribing practices of NPs and the use of the LNG-IUS in the identified population. NP prescribing trends and the potential for increasing access to contraception with positive health outcomes were identified themes in the literature, demonstrating that NPs are highly capable of improving the sexual health of marginalized female adolescents. Furthermore, the literature demonstrates that the LNG-IUS is an excellent and appropriate choice of contraception for this population and NPs can be secure in the decision to recommend, prescribe and insert the LNG-IUS for marginalized adolescent female patients requiring contraception."--P.2.
