Book excerpt: Based on research studies and documented professional experiences there appears to be an issue with high new graduate nurse (NGRN) turnover rates within the nursing profession. This poses a problem within the profession as health care organizations prepare for the anticipated nursing shortage projected by the Bureau of Labor Statistics. High turnover rates have been linked to poor transition into practice. Inadequate transition can cause nursing errors that jeopardize the quality and safety of patient care in addition to causing health care organizations to lose money. This indicates the need for effective transition into practice programs as NGRNs join the workforce. One solution is to implement nurse externships. Externships have been common practice for many professions as a successful way to prepare students for the roles and responsibilities required by a given profession. Implementing externships within the nursing profession would be an effective means in preparing nursing students for the profession and ultimately lower NGRN turnover rates (Redding and Flatley, 2003). Currently, it is common practice for health care organizations to adopt new graduate programs (NGP) to help NGRNs transition into practice. While these programs have been successful in lowering NGRN turnover rates to some degree, they are costly to organizations and the issue still remains, justifying the need to explore other options. The author of this paper explores the concept of nurse externships as a proposal to produce better prepared NGRNs and reduce NGRN turnover rates; identifying an appropriate plan for implementation, evaluation, and dissemination to ensure timely and appropriate integration into practice while utilizing current research to support key claims.
