Book excerpt: Adult skeletal muscle has a remarkable capacity to initiate a rapid and extensive repair process after damage due to injury or degenerative disease. Although satellite cells are the primary skeletal muscle stem cells, there are many reports of non-satellite cell populations with myogenic capacity resident within skeletal muscle. The activity of muscle-resident stem cells during the regeneration process is tightly controlled through the dynamic interactions between intrinsic factors within the cells and extrinsic factors constituting the muscle stem cell niche. The extracellular matrix (ECM) in skeletal muscle plays an integral role in force transmission, structural maintenance, and regulation of stem cell niche. ECM interacts with stem cells either directly by binding cell surface receptors or indirectly through growth factor presentation, and maintains a balance between their quiescence, self-renewal, and differentiation. These interactions are reciprocal since the stem cells can remodel the niche and secrete or degrade ECM components. Natural ECM scaffolds, derived from decellularized tissues can influence stem cell activity both in vitro and in vivo and are widely being investigated for skeletal muscle repair. In this chapter, we discuss the regenerative potential of stem cell populations and ECM bioscaffolds in the treatment of skeletal muscle injury and disease.

Book excerpt: Stem cells are defined as undifferentiated cells that are able to unlimitedly renew themselves within controlled conditions and to differentiate into a multitude of mature cell types. Skeletal muscle stem cells, represented predominantly by satellite cells, show a variable capability of self-renewal and myogenic differentiation. They were found to be involved not only in the growth of myofibers during neonatal and juvenile life but also in the regeneration of skeletal muscles after an injury. The microenvironment in which stem cells are nourished and maintained dormant preceding division and differentiation is known as ,Äúniche.,Äù The niche consists of myofibers, which are believed to modulate the active/inactive state of the stem cells, extracellular matrix, neural networks, blood vessels, and a multitude of soluble molecules. It was observed that changes in the composition of the niche have an impact on the stem cell functions and hierarchy. Furthermore, it seems that its layout is variable throughout the entire life, translating into a decrease in the regenerative capacity of satellite cells in aged tissues. The scope of this chapter is to provide a detailed view of the changes that occur in the skeletal stem cell niche during life and to analyze their implications on tissue regeneration. Future studies should focus on developing new therapeutic tools for diseases involving muscle atrophy.

The cellular communication processes and mediator molecules in the regenerative microenvironment that control muscle stem cell functions are poorly characterized. Muscle resident fibro/adipogenic progenitors (FAPs) are responsible of the fibrotic and adipose degeneration of muscle in pathological situations. In contrast with this function, here we demonstrate that blood-infiltrating neutrophils cooperate with FAPs to promote muscle regeneration after injury. By cell-type specific gene deletion and transplantation experiments, here we show the requirement of the soluble form of interleukin 6 (IL-6) receptor released by neutrophils and FAP-derived IL-6 to activate IL-6 trans-signaling for efficient muscle stem cell proliferative and regenerative functions. Exploitation of this physiological mechanism may be of therapeutic importance in pathologies coursing with poor regeneration of skeletal muscle.

An intriguing characteristic of skeletal muscle is its remarkable regenerative capacity: a highly orchestrated cellular process, in which the resident muscle stem cells (MuSCs) play a central role. Upon tissue injury, quiescent MuSCs are activated and give rise to a population of primary myoblasts (pMBs). pMBs undergo several rounds of division and ultimately fuse with one another to form multi-nucleated myofibers and repair the muscle. As observed in a wide range of conditions such as muscular dystrophies and aging, disruptions in the repair process can lend to impaired regeneration and progressive muscle wasting. Strategies to restore strength and function to pathological muscle include cell-based therapies to replace defective myogenic cells, and treatments to restore the endogenous repair process. Despite substantial advances, these treatment options are still in the early stages of clinical translation. The specific goal of this thesis is to improve upon currently available strategies as well as to lay the groundwork for the emergence of new therapeutic entry-points for skeletal muscle treatments. In the area of cell transplantation therapy, challenges remain in producing clinically-relevant numbers of cells that, as a population, possess high regenerative potency to produce skeletal muscle and repopulate the stem cell niche. We demonstrate that by using a bioactive hydrogel (HAMC) as the cell delivery vehicle, it is possible to improve MuSC transplantation outcomes, and thereby reduce the overall number of required MuSCs. Next, we address the issue of producing sufficient numbers of highly regenerative myogenic cells. Using a high-throughput drug screen and in-vivo intramuscular transplantation assay validation, we identify epidermal growth factor receptor (Egfr) and vascular endothelial growth factor receptor 2 (Vegfr2, Kdr) as new druggable targets, that upon inhibition, produce a population of cultured MuSCs with greater regenerative potency than control treated. Finally, using single-cell RNA sequencing, we shed light on the diversity and intercommunication of cells present in skeletal muscle. This dataset serves as a valuable resource through which new regulators of MuSCs and other cells in skeletal muscle can be evaluated with an eye towards skeletal muscle regenerative medicine applications.
