Book excerpt: This thesis aims to give insight into the clinical status quo of primary pediatric liver tumors and investigate signaling pathways that may be of importance for liver tumorigenesis. The most recent series of the International Childhood Liver Tumors Strategy Group (SIOPEL) of hepatocellular carcinoma (HCC) and its distinct variant fibrolamellar hepatocellular carcinoma (FL-HCC) are discussed. For both tumors, complete resection is at present the only curative option. FL-HCC does not have a better prognosis than HCC. A potential role for the Delta-Notch signaling pathway in hepatoblastoma is explored and although this cannot be detected, interestingly, tissue-specific Notch 2 deletion is found to lead to bile duct agenesis. The role of the DCUN1D1-5 proteins and the neddylation and ubiquitination signaling pathways in hepatotumorigenesis was studied. The paralogues DCUN1D3 and DCUN1D5 were characterized; DCUN1D3 is shown to function as a tumor suppressor, whereas DCUN1D5 functions as an oncogene exhibiting an oncogene addiction phenotype. DCUN1D paralogue expression is found to correlate with function in liver cancer tissue and increased function correlates with low LD50 and IC50 of the neddylation inhibiting compound MLN4924. The oncogene addiction phenotype observed for several paralogues may thus be exploited to select liver tumors for treatment with such a drug..

Book excerpt: Myeloid Derived Suppressor Cells (MDSCs) are a heterogeneous population of immature myeloid cells that can suppress the function of multiple immune cells and in particular, T cells, through various mechanisms. MDSCs can be divided into two major subtypes based on their cell surface phenotype and morphology: polymorphonuclear MDSC (PMN-MDSC or G-MDSC) and monocytic MDSC (M-MDSC). Additional subtypes have been proposed, such as the early MDSC (e-MDSC) that lack both macrophage and granulocyte markers. There is still considerable ambiguity about the phenotype of these cells that corresponds to their immunosuppressive function and there are on-going challenges on how to identify, purify and/or potentially generate and expand these cells in vitro. MDSCs were first discovered in cancer patients where they have been most extensively studied as components of the immunosuppressive tumor microenvironment. In the last several years, however, the importance of their immunomodulatory role in many other disease and clinical settings has emerged. Hepatocellular carcinoma (HCC) is the third most common cause of cancer related death in the United States. Treatment options are limited. Viral hepatitis is one of the major risk factors for HCC, which represents a typical “inflammation-induced” cancer. Immune-based treatment approaches have revolutionized oncology in recent years. Various treatment strategies have received FDA approval including dendritic cell vaccination, for prostate cancer as well as immune checkpoint inhibition targeting the CTLA4 or the PD1/PDL1 axis in melanoma, lung, and kidney cancer. Additionally, cell based therapies (adoptive T cell therapy, CAR T cells and TCR transduced T cells) have demonstrated significant efficacy in patients with B cell malignancies and melanoma. Immune checkpoint inhibitors in particular have generated enormous excitement across the entire field of oncology, providing a significant benefit to a minority of patients.

Book excerpt: Diabetes is a metabolic disorder of chronic hyperglycemia that causes numerous complications. The International Diabetes Federation and World Health Organization report that approximately 537 million people worldwide have diabetes in 2021, which may increase to 700 million by 2045. Diabetes and its complications significantly lower the quality of life of patients and cause more than 4 million deaths yearly. Diabetes and its complications also increase premature mortality, increasing society's economic burden. Numerous studies reveal that the chronic multisystem injury and dysfunction can be caused by diabetes, including cardiac and cerebral vascular lesions, nephropathy, retinopathy and neuropathy. In addition, diabetes usually co-occurs with other diseases that may have certain risk factors in common. For example, patients with diabetes are more likely to have hypertension, chronic obstructive pulmonary disease, or asthma. However, there is still a lack of knowledge on the relationship between diabetes and other potential comorbidities such as hepatitis infection, hearing impairment and periodontal disease. Furthermore, common risk factors contributing to the pathogenesis of diabetes and comorbidities are far from fully understood. Several comorbidities (e.g., cardiovascular and cerebrovascular diseases) raise the extra chances of death in patients with diabetes, but the association between diabetes and mortality of other diseases, such as endometrial or kidney cancer, is inconsistent. Further research is still needed to reveal the excess mortality associated with diabetic comorbidities.
